[Adult T-cell leukemia/lymphoma of bone with multiple pathological fractures].

We report a 49-year-old woman who had adult T-cell leukemia/lymphoma (ATL) of the bone with multiple osteolytic lesions and pathological fractures of her extremities. The WBC count was 15,810/microliter with 0-1% of abnormal lymphocytes, but CT scan revealed no lymphadenopathy. Antibodies to HTLV-I were positive, and a right tibial bone biopsy showed infiltration of ATL cells. Monoclonal integration of HTLV-1 proviral DNA was demonstrated in bone tumor cells. Seven weeks later, she died of DIC due to severe infection during extensive chemotherapy.

Outcome of distal radius fractures in relation to bone mineral density.

The purpose of this study was to determine whether osteoporosis influenced the clinical results of distal radius fractures in women above the age of 40 years. Forty patients underwent bone mineral densitometry of the distal forearm, hip and lumbar spine. Radiographs of both wrists were taken at the time of fracture healing and the difference in ulnar variance, palmar tilt and radial inclination with the contralateral wrist was measured. Wrist mobility, grip strength and pain relief were determined in 35 patients with a follow-up of more than one year. We found that the clinical results correla

Surgical treatment of acetabular fractures in elderly patients with osteoporotic bone.

The incidence of acetabular fractures in the elderly has recently shown a marked increase due to the combination of greater longevity for the population as a whole and a relative decrease in the incidence of alcohol-related trauma in younger adults. The compromised physiologic reserve and the diminished healing capacity of the typical elderly patient have an adverse effect on the potential for a favorable clinical outcome. The presence of osteopenic bone or degenerative arthritis and the effects of previous radiation therapy to the hip and pelvis hamper diagnostic imaging and the utility of so

Tibial fractures with bone loss treated by primary reamed intramedullary nailing.

We reviewed the results of the treatment of 30 tibial fractures with minor to severe bone loss in 29 patients by early soft-tissue and bony debridement followed by primary locked intramedullary nailing. Subsequent definitive closure was obtained within the first 48 hours usually with a soft-tissue flap, and followed by bone-grafting procedures which were delayed for six to eight weeks after the primary surgery. The time to fracture union and the eventual functional outcome were related to the severity and extent of bone loss. Twenty-nine fractures were soundly united at a mean of 53.4 weeks, w

Management of petrous bone fractures in children: analysis of 127 cases.

Petrous bone fractures (PBF) in children are relatively frequent. They are mostly diagnosed after collisions and falls. The complications typically associated with PBF were different types of hearing disorders in 69.1% of the patients who had audiometry, liquorrhea in 16.5%, palsy of cranial nerves in 10.8% (facial nerve palsy in 9.4%), bacterial meningitis, stenosis of the external ear canal, and posttraumatic cholesteatoma in 0.7% of the fractures each. Most complications were transient; 8.6% of the patients underwent surgery because of PBF-related complications and 9.4% suffered from severe

[Pathomorphogenesis of radio-ulnar synostosis in fractures of forearm bone shafts].

Pathomorphogenesis of radioulnar synostosis which occurred in 4 out of 174 patients treated because of recent and inveterate fractures as well as pseudoarthroses within shafts of forearm bones has been presented. Fractures of both radius and ulna, located at the same level within proximal half, inveterate or neglected, after severe or multiple trauma are at the highest risk of developing radioulnar synostosis.

Reduced bone-mineral content is a risk factor for hip fractures.

To evaluate skeletal osteopenia as a risk factor for hip fractures, we measured bone-mineral content in 18 postmenopausal women (aged 50 to 85 years) within 5 days of sustaining a nontraumatic hip fracture. We compared these findings to those in 50 women without a hip fracture who were matched for age, postmenopausal years, height, and weight to the study participants. With the use of dual-photon absorptiometry, the hip fracture patients were found to have a significantly reduced mean bone-mineral content (P less than .005) at three sites in the unaffected hip and a marginally lower mean value

[Bone screw osteosynthesis of dens fractures. Technical surgical aspects and results].

Twenty cases of Anderson and d'Alonzo type II and "shallow" type III fractures of the dens were treated by anterior screw fixation: the results were reviewed and compared with previously published results obtained in series of such fractures treated non-surgically, by posterior C1-C2 arthrodesis or anterior screw fixation. The complication rate of 25% in our series is comparable to those reported in a previously published review of studies on posterior wiring for C1-C2 arthrodesis. Three of our cases in which complications occurred (15%) were recognized in retrospect as inappropriate for the u

Naso-ethmoid-orbital fractures: classification and role of primary bone grafting.

A detailed review of 80 patients with severe naso-ethmoid-orbital injuries has facilitated the classification of these injuries into five types. The recognition and diagnosis of each specific injury pattern will define the correct treatment choice in each instance. Special attention should be focused on injuries with comminution and bone loss in the medial wall and floor of the orbit, with loss of cartilaginous nasal support, and with orbital displacement and dystopia. An open, direct approach to these fractures with meticulous reduction, internal fixation, and repair of the medial canthal lig

Bone Metabolism-related Serum Biomarkers and Nutritional Markers for Bone Fractures in Living-donor Kidney Transplant Recipients.

BACKGROUND/AIM: The clinical importance of fracture prevention in patients with end-stage renal disease is well-established. We investigated the roles of bone metabolism-related serum biomarkers and nutritional markers for fractures in Japanese living-donor kidney transplant recipients. PATIENTS AND METHODS: We included 204 consecutive patients who underwent kidney transplantation at Nara Medical University between 2003 and 2022 and retrospectively reviewed their medical charts. The cumulative incidence of fractures was investigated by focusing on bone metabolism-related serum biomarkers and

The impact of bone histomorphometry, bone-related biomarkers, and FRAX® on fractures patients with predialysis CKD.

INTRODUCTION: Chronic kidney disease (CKD) patients are at increased risk of fracture. Whether the type of renal osteodystrophy (ROD) contributes to fracture risk is not currently established since bone biopsies are not frequently performed in clinical practice. We aimed to evaluate the association of ROD subtypes, bone biomarkers, and fracture risk assessed with the FRAX® tool with the occurrence of fractures in predialysis CKD patients. METHODS: Retrospective study with patients followed between 2014-2023. Blood tests, including bone-related biomarkers (BRB), and bone biopsies were performe

Surgical Reconstruction of Canine Nonunion Fractures Using Bone Morphogenetic Protein-2-loaded Alginate Microbeads and Bone Allografts.

BACKGROUND/AIM: Effective treatment of nonunion fractures is challenging as it requires a biological and mechanical environment to promote sufficient osteogenesis. Herein, we present a case series in which we evaluated the clinical efficacy of bone morphogenetic protein-2 (BMP-2)-loaded alginate microbeads and allografts in two dogs with nonunion fractures. CASE REPORT: A 3-year-old, 2.3-kg, spayed female Pomeranian (Case 1) presented with intermittent lameness of the left forelimb after radial and ulnar fracture repair 8 weeks prior. A 4-year-old, 4.8-kg, spayed female Pomeranian (Case 2) wa

MRI-based vertebral bone quality score effectively reflects bone quality in patients with osteoporotic vertebral compressive fractures.

OBJECTIVE: The present study is aimed to validate the ability of the vertebral bone quality (VBQ) score to evaluate bone quality in patients with osteoporotic vertebral compression fractures (OVCF) and to compare it with the ability of T-score by DXA. In addition, the sensitivity of VBQ score with cerebrospinal fluid (CSF) of L2 and L3 segments as baseline is evaluated. METHODS: 196 inpatients were collected and assigned into OVCF and Non-OVCF groups, respectively. For each patient, the VBQ score was calculated by the signal intensity of the L1-L4 vertebral bodies and CSF at L3 or L2 level fr

Relationship between Skeletal Muscle Mass, Bone Mineral Density, and Trabecular Bone Score in Osteoporotic Vertebral Compression Fractures.

STUDY DESIGN: A retrospective observational study was performed. PURPOSE: We investigated the relationships between skeletal muscle mass, bone mineral density (BMD), and trabecular bone score (TBS) in patients with osteoporotic vertebral compression fractures (VCFs). OVERVIEW OF LITERATURE: The TBS has attracted attention as a measurement of trabecular bone microarchitecture. It is derived from data obtained using dual-energy X-ray absorptiometry (DXA) and is a reported indicator of VCFs, and its addition to the Fracture Risk Assessment Tool increases the accuracy of fracture prediction. Role of APOE Gene in Bone Mineral Density and Incidence of Bone Fractures in Brazilian Postmenopausal Women.

Osteoporosis is one of the major diseases that affects mostly postmenopausal women. Despite being a multifactorial disease, some genes have been shown to play an important role in osteoporosis. Bone mineral density (BMD) is still largely used to diagnose it, although many other biomarkers are used to better follow the disease onset. It has been shown that the apolipoprotein E (APOE) gene could be a biomarker for risk of fractures as well as to predict lower BMD in patients with osteoporosis. The human APOE gene encodes 3 protein isoforms called ApoE2, ApoE3, and ApoE4, resulting in 4 possible
