Gait determinants of undulatory swimming via energy optimality principles.

Animals with a slender body such as eels progress by propagating body waves; this swimming mode is called anguilliform. Though anguilliform swimming has been studied extensively, its optimality and gait selection principles remain unknown. Here, through model-based optimal gait analysis, we propose that three dimensionless characteristic parameters -- the stride length, muscle mechanical efficiency and environmental force anisotropy -- shape the gait, and they respectively determine a whole or a fraction of body undulation, a more C-shaped or S-shaped body wave, and the overall body undulation…

Late Lyme neuroborreliosis presenting with tremor and gait impairment.

A 60-year-old woman presented with a 38-month history of progressive tremor and gait impairment. Examination was consistent with a myeloradiculopathy, cerebrospinal fluid (CSF) analysis was inflammatory and imaging showed meningeal and white matter disease. Revisiting the history identified long periods spent gardening in the months before symptom onset with significant tick exposure. Lyme testing was strikingly positive in serum and CSF: a diagnosis of late Lyme neuroborreliosis was confirmed. There was clinical and radiological response to appropriate antimicrobial treatment with ongoing imp…

Gait impairment associated with neuroimaging biomarkers in Alzheimer's disease.

Research links gait impairment in Alzheimer's disease (AD) to cognitive abnormalities, brain atrophy, or amyloid-β (Aβ) deposition, with the exact cause unclear. This study investigated the relationship between gait, neuroimaging biomarkers, and cognition across the AD spectrum. We recruited 48 AD dementia patients, 27 with prodromal AD, and 41 cognitively unimpaired individuals, analyzing associations among gait parameters, cognitive scores, Aβ deposition, and cortical atrophy. Path and receiver operating characteristic (ROC) analyses evaluated gait impairment's interdependent interactions an…

Inter-limb gait asymmetry in people with Parkinson's disease.

Gait asymmetry is a hallmark of Parkinson's disease (PD), contributing to instability, increased fall risk, and reduced quality of life. While spatiotemporal asymmetry has been widely studied, limited research has examined joint kinematic asymmetries throughout the gait cycle, particularly in both ON- and OFF-medication states. This study evaluated inter-limb asymmetry in lower limb angular kinematics during walking in individuals with PD compared to neurologically healthy older adults. Twenty-three individuals with PD and 18 healthy older adults participated. PD participants were evaluated in…

Recurrent syncopal events preceded by transient abnormal gait disturbance.

A previously fit and well patient in his 70s presented after recurrent transient loss of consciousness following dinner with alcohol consumption. Each event has been preceded by several minutes of transient abnormal posture and gait. More specifically, there were several minutes of stiffening of legs, shuffling gait and gait ataxia. Initial clinic investigations, including ECG, echocardiogram, CT and MRI head scans, vestibular tests and biochemistry for vitamins and electrolytes, proved inconclusive. However, a positive tilt table test demonstrated severe orthostatic hypotension (OH) with loss…

Spine kinematics during gait in paediatric Hereditary Spastic Paraparesis.

BACKGROUND: Many studies already addressed specific gait abnormalities in children affected by Hereditary Spastic Paraparesis (HSP). Some authors investigated the contribution of the upper body to walking pattern, but simplifying trunk and pelvis as two hinged rigid bodies. Recently, we developed a method to detail spinal kinematics in terms of anatomic curvatures and length; we were thus interested in applying such protocol to HSP. RESEARCH QUESTION: how HSP influences spinal kinematics during gait? METHODS: we enrolled ten HSP patients (5-17 years, 8 males) and twelve Healthy Children (HC,…

Biosignal-based Control of a Robotic Gait Training Lifter.

In this paper, we present a robotic walker that aims to encourage the patient's voluntary movement by enabling intention-based control of the mobile base. We proposed two variants of biosignal-based control methods for the robotic gait training lifter (GTL): one method to control the speed of the walker directly from Electromyography (EMG) signals of the muscles of the lower limb, and the other method to control the acceleration of the walker from EMG. We implemented these methods in a robotic walker and conducted experiments with one healthy male and one female participant to evaluate the fea…

Gait classification for growing children with Duchenne muscular dystrophy.

Classifying gait patterns into homogeneous groups could enhance communication among healthcare providers, clinical decision making and clinical trial designs in boys with Duchenne muscular dystrophy (DMD). Sutherland's classification has been developed 40 years ago. Ever since, the state-of-the-art medical care has improved and boys with DMD are now longer ambulatory. Therefore, the gait classification requires an update. The overall aim was to develop an up-to-date, valid DMD gait classification. A total of 137 three-dimensional gait analysis sessions were collected in 30 boys with DMD, aged …

Nonlinear Assessment of Gait Signal Complexity in Neurodegenerative Disorders.

The human gait cycle undergoes discernible alterations upon the onset of neurodegenerative diseases (NDD) such as Parkinson's, Huntington's, and Amyotrophic lateral sclerosis. Each specific neurodegenerative disorder imparts a distinct influence on human gait dynamics, and precise quantification of these changes holds the potential for accurate methods of NDD detection.Nonlinear entropy algorithms, such as sample entropy (SampEn), find widespread use in physiological signal analysis. SampEn gauges signal complexity by identifying pattern matches within windowed sub-segments of the signal. Howe…

Gait Velocity Alterations in Essential Tremor: a Meta-Analysis.

Essential tremor (ET) is a prevalent movement disorder that impairs gait function, including gait speed - a critical marker of mobility disability and adverse outcomes. This meta-analysis aimed to quantify differences in gait speed between individuals diagnosed with ET compared to people without a movement disorder diagnosis. Electronic databases were searched for studies comparing gait speed in ET patients and controls. Effect sizes were calculated using standardized mean differences (Hedges' g) and pooled using a random-effects model. Eight studies (390 ET, 227 controls) were included. ET pa…

Validation of an Ear-Worn Wearable Gait Analysis Device.

Wearable devices capable of measuring gait parameters may provide a means to more economical gait analysis compared to conventional equipment comprising of a motion capture system and a forced treadmill. Beflex Coach (Beflex, Republic of Korea) is one such device but worn on the ear as Bluetooth earphones, unlike other wearables worn on the wrist, feet, or torso. In this study, the validity of the device was examined against a motion capture system and a forced treadmill for walking and running parameters. Five walking parameters (cadence, single support time, double support time, vertical osc…

Gait analysis in chemotherapy-induced peripheral neurotoxicity rodent models.

Gait analysis could be used in animal models as an indicator of sensory ataxia due to chemotherapy-induced peripheral neurotoxicity (CIPN). Over the years, gait analysis in in vivo studies has evolved from simple observations carried out by a trained operator to computerised systems with machine learning that allow the quantification of any variable of interest and the establishment of algorithms for behavioural classification. However, there is not a consensus on gait analysis use in CIPN animal models; therefore, we carried out a systematic review. Of 987 potentially relevant studies, 14 wer…
