Towards monitoring of global health research: an exploratory analysis of transparency and stakeholder engagement.

OBJECTIVES: Monitoring systems exist for clinical research transparency in high-income countries, but systematic assessment of these practices in global health (GH) research (GHR) is limited. We evaluated methods for monitoring GHR transparency and engagement. DESIGN: Cross-sectional study. DATA SOURCES: Three sources were used: (a) ClinicalTrials.gov, (b) publications from 20 journals with 'international' or 'global' in the title and (c) outputs from selected GH funder websites. ELIGIBILITY CRITERIA: From ClinicalTrials.gov, we selected 200 interventional trials on maternal health and tube…

Update on the global burden of acute viral hepatitis in 2021: addressing health inequalities.

BACKGROUND: The strategy for eliminating viral hepatitis is at a critical juncture, necessitating an updated assessment of global incidence trends. METHODS: Data on the incidence of four types of acute viral hepatitis (AVH), namely, acute hepatitis A (AHA), acute hepatitis B (AHB), acute hepatitis C (AHC), and acute hepatitis E (AHE), were sourced from the Global Burden of Disease (GBD) Study 2021. The annual percentage change is utilized to elucidate temporal trends, whereas health inequalities and frontier analysis serve to evaluate global health equity and quantify disparities in burden am…

Structural Barriers and the Future of AI in Global Health: Lessons From Eradicable Diseases.

Despite unprecedented scientific and technological capacity, a substantial burden of eradicable and highly reducible diseases of poverty persists in low- and middle-income countries. These conditions continue to cause preventable mortality and morbidity. While artificial intelligence (AI) is widely promoted as a transformative solution to global health challenges, historical evidence demonstrates that technological innovation alone cannot overcome entrenched political, governance, and equity barriers. This narrative review synthesizes evidence from peer-reviewed literature, global health agenc…

The Origins and Genetic Diversity of HIV-1: Evolutionary Insights and Global Health Perspectives.

Human immunodeficiency virus (HIV), comprising two distinct types, HIV-1 and HIV-2, remains one of the most significant global health challenges, originating from multiple cross-species transmissions of simian immunodeficiency viruses (SIVs) in the early 20th century. This review traces the evolutionary trajectory of HIV from zoonotic spillover to its establishment as a global pandemic. HIV-1, the principal strain responsible for AIDS, emerged from SIVcpz in Central African chimpanzees, with phylogenetic evidence indicating initial human transmission between the 1920s and 1940s in present day …

Global health initiatives and the combat of HIV/AIDS on the African continent: a scoping review.

The aim is to identify, in the literature, the influence of Global Health Initiatives (GHI) on the HIV combat in Africa. A scoping review was carried out in March 2023 in the Web of Science Core Collection, Medical Literature Analysis and Retrieval System Online, Scopus, Excerpta Medica Data-base and Google Scholar databases. It included original articles published in English, Portuguese, Spanish and French that dealt specifically with the influence of GHI on this endeavour. 25 studies published between 2009 and 2019 were selected. The analysis resulted in two thematic categories: Contribution…

Unemployment and mental health: a global study of unemployment's influence on diverse mental disorders.

INTRODUCTION: Globally, one in five individuals faces unemployment, which substantially increases their risk of developing mental disorders. Understanding the relationship between unemployment and specific mental health outcomes is crucial for formulating effective policy interventions. METHODS: This study examines the relationship between unemployment and mental disorders across 201 countries from 1970 to 2020. Using a fixed-effects model, we analyze the impact of unemployment on various mental health outcomes, including anxiety, depression, bipolar disorder, drug use, and eating disorders,

Masculinities and sexual and reproductive health and rights: a global research priority setting exercise.

Engaging men and boys in sexual and reproductive health and rights (SRHR) and doing so in a way that challenges harmful masculinities, is both neglected and vital for improving the SRHR of both women and men. To address this gap, WHO commissioned a global research priority setting exercise on masculinities and SRHR. The exercise adapted the quantitative child health and nutrition research initiative priority setting method by combining it with qualitative methods. Influenced by feminist and decolonial perspectives, over 200 diverse stakeholders from 60 countries across all WHO regions particip

Equivocal diagnostics: Making a 'good' point-of-care test for elimination in global health.

What is a diagnostic test for? We might assume the answer to this question is straightforward. A good test would help identify what disease … through three configurational moments in the framing of onchocerciasis disease and reshaping of the global health innovation ecosystem. Efforts to build that ecosystem for diagnostics are often premised on the notion that public health needs for diagnostics are pre-given and stable; the challenge is seen to be how to incentivize investment and find a customer b …

The 2024 Public Health Emergency of International Concern: A Global Failure to Control Mpox.

On August 14, 2024, following a regional declaration by the Africa Centres for Disease Control and Prevention, the World Health Organization declared mpox a Public Health Emergency of International Concern, marking the second such declaration in two years. A series of outbreaks involving the more virulent clade I virus (compared to clade II, which caused a global outbreak in 2022), has now spread in 13 African countries, exposing the inadequacies of the public health infrastructure in these settings. There was significant investment during the 2022 global outbreak, but these efforts failed to

Association of health, nutrition, and socioeconomic variables with global antimicrobial resistance: a modelling study.

BACKGROUND: Although antimicrobial use is a key selector for antimicrobial resistance, recent studies have suggested that the ecological context in which antimicrobials are used might provide important factors for the prediction of the emergence and spread of antimicrobial resistance. METHODS: We used 1547 variables from the World Bank dataset consisting of socioeconomic, developmental, health, and nutritional indicators; data from a global sewage-based study on antimicrobial resistance (abundance of antimicrobial resistance genes [ARGs]); and data on antimicrobial usage computed from the ECD

Health workers' values and preferences regarding contraceptive methods globally: A systematic review.

OBJECTIVE: We sought to systematically review the literature on health workers' values and preferences related to contraceptive methods. STUDY DESIGN: As part of a larger review, we searched ten electronic databases for published articles from January 1, 2005 through July 27, 2020. We included studies that reported qualitative or quantitative data from the perspective of health workers providing family planning services globally. RESULTS: Forty-one studies met our inclusion criteria. These studies included 12,643 health workers and were conducted in 27 countries. Health worker values and pre

A Global Assessment of Eye Health and Quality of Life: A Systematic Review of Systematic Reviews.

IMPORTANCE: More than 1 billion people worldwide have vision impairment or blindness from potentially preventable or correctable causes. Quality of life, an important measure of physical, emotional, and social well-being, appears to be negatively associated with vision impairment, and increasingly, ophthalmic interventions are being assessed for their association with quality of life. OBJECTIVE: To examine the association between vision impairment or eye disease and quality of life, and the outcome of ophthalmic interventions on quality of life globally and across the life span, through an um

A global approach to addressing the policy, research and social challenges of male reproductive health.

Male infertility is a global health issue; yet to a large extent, our knowledge of its causes, impact and consequence is largely unknown. Recent data indicate that infertile men have an increased risk of somatic disorders such as cancer and die younger compared to fertile men. Moreover, several studies point to a significant adverse effect on the health of the offspring. From the startling lack of progress in male contraception combined with the paucity of improvements in the diagnosis of male infertility, we conclude there is a crisis in male reproductive health. The Male Reproductive Health …

Water insecurity and urban poverty in the Global South: Implications for health and human biology.

OBJECTIVES: Over half of the world's population (4 billion people) experience severe water scarcity at least one month per year, while half a billion people experience severe water scarcity throughout the year. Despite progress from national and global interventions, a staggering proportion of the Global South remains water insecure. Rapid urban growth and associated demographic changes, climate change, and governance failure have also fostered the growth and expansion of urban informal settlements and slums where widespread poverty and environmental hazards exacerbate the impact of water inse
