Sliding anterior hemitongue flap for posterior tongue defect reconstruction.

Posterior tongue defects present a unique reconstructive challenge. The various reconstructive options available for treating the defect created by a posterior hemiglossectomy frequently result in a distorted tongue and functional impairment. This paper describes a novel sliding anterior hemitongue flap to allow reconstruction of moderate resection defects (i.e. for T1-T2 tongue squamous cell carcinomas) of the posterior tongue. By mobilizing the anterior tongue, near normal mobility and tongue length are maintained. This surgical technique may be performed alone intraorally or in combination

Modification of flap design for total mobile tongue reconstruction using a sensitive antero-lateral thigh flap.

Reconstruction of large carcinologic tongue defect is mandatory to recover adequate speech and swallowing. Free flaps provide thin and pliable tissues needed to restore the shape and the volume of the tongue but their functional outcomes, especially in case of total mobile tongue reconstruction, are still limited. The authors describe a modification of flap design called the 'cathedral triptych' used with the antero-lateral thigh flap. This modified design recreates a near normal neotongue shape with more projected tip and vertical bulk that can contact the palate and the upper lip. Ten patien

Right middle and lower bilobectomy using a modified fissureless technique: a case report.

BACKGROUND: Fissureless lobectomy has been shown to prevent air leaks in patients with fused or incomplete interlobar fissures or emphysema. The technique is also beneficial in advanced lung cancers involving adjacent lobes, where interlobar manipulation is technically difficult. Although this technique is widely used in standard lobectomies, reports on bilobectomy remain scarce. CASE PRESENTATION: A 57-year-old woman with left-sided tongue cancer (pT2N0M0, Stage II) underwent subtotal glossectomy followed by postoperative concurrent chemoradiotherapy. Eighteen months after the surgery, imagi

Evaluation of speech outcomes in hemiglossectomy: Factors influencing long-term results.

BACKGROUND: Achieving optimal functional speech outcomes is essential in tongue reconstruction. Variables such as recipient artery, tumor size, and radiotherapy can impact tongue movement and speech outcome. Various tongue resection types and reconstruction methods exist. Our study examined post-hemiglossectomy speech outcomes by considering the reconstruction type and other relevant factors. METHODS: This retrospective study included 24 patients who underwent hemi-tongue reconstruction and speech rehabilitation. The evaluation included articulation, intelligibility, and specific syllabic cha

Influence of surgical tongue reduction on pressure from the tongue on the teeth.

Pressures from the tongue on the teeth were recorded in 21 children and adolescents before and after surgical reduction of the tongue. The recordings were made before surgery, and 6 and 12 months after the operation. Simultaneous measurements were made at the lingual surfaces of the maxillary and mandibular central incisors and at the left first molar, in the rest position and during chewing and swallowing. The method had been used in a previous study of normal cases, which served as a reference. Presurgical pressures recorded in the rest position at the maxillary incisors agreed with measurem

Prosthetic rehabilitation associated with the use of high-power laser following pelveglossectomy and radiotherapy for oral squamous cell carcinoma: A clinical report.

This clinical report documented the prosthodontic rehabilitation of a 70-year-old man with extensive postoperative sequelae (fibrosis and mucosal adhesions) following pelveglossectomy and radiotherapy for oral squamous cell carcinoma. To overcome severe anatomic limitations impeding oral rehabilitation, a multidisciplinary protocol was implemented featuring 10 surgical sessions using a 980-nm diode laser for minimally invasive procedures, including fibrotic band release, adhesion reduction, and vestibular sulcus deepening (vestibuloplasty). Custom surgical and healing guides were essential to

Predictors of success in combination of tongue base resection and lateral pharyngoplasty for obstructive sleep apnea.

The base of the tongue has been recognized as a significant site of obstruction in patients with obstructive sleep apnea (OSA). Our aim was to determine the independent predictors of surgical success in tongue base resection combined with lateral pharyngoplasty for OSA. Thirty-one OSA patients who underwent endoscopie-guided coblator or transoral robotic tongue base resection in combination with lateral pharyngoplasty for the treatment of retroglossal obstruction between March 2012 and December 2015 were enrolled in this study. Retroglossal obstruction was identified by preoperative nasopharyn

Bilateral gustatory sweating in the submandibular region after bilateral neck dissection successfully treated with botulinum toxin.

Gustatory sweating (or chorda tympani syndrome) in the submandibular region is a rare complication of cervical surgery. We report a 66-year-old patient who had been referred for the management of a squamous cell carcinoma of the mobile tongue. She was treated by bilateral neck dissection and bilateral resection of the submandibular glands. Bilateral gustatory sweating in the submandibular region developed 10 years later and was successfully treated with bilateral injections of botulinum toxin type A.

Development of a test for evaluation of taste perception after tongue reduction.

When performing a tongue reduction a frequently asked question is how operation will influence taste of the patient. Different kinds of taste tests are designed, most of these being non-specific ways to determine taste sensation in which high concentration of taste solutions are used to detect if a person is able to taste. To be able to judge the influence of tongue reduction on taste we wanted to develop a validated test that could be used in early childhood. No specific tasting areas were found. This test can be used to evaluate tongue reduction procedures.

Medial sural artery perforator flap for intraoral reconstruction following cancer ablation.

In oral cavity reconstruction, the fasciocutaneous flaps of the distal extremities have always been preferred to any other kind of flap because of their thinness and pliability, which makes them adaptable to different areas in the oral cavity. The radial forearm flap is frequently considered the first choice for intraoral reconstruction, but the disadvantages of donor site morbidity include sacrificing a major artery to the hand and leaving a conspicuous donor site scar. The search for another primarily thinned skin flap as an alternative has led to the application of the medial sural artery p

Functional outcomes after surgical reconstruction of the base of tongue using the radial forearm free flap in patients with oropharyngeal carcinoma.

BACKGROUND: Historically, the prevention of functional morbidity after multimodal treatment for squamous cell carcinoma in patients with large base of tongue lesions has been challenging. The purpose of the present research is to report prospectively collected speech and swallowing outcomes for patients with disease that encompassed half or more of the base of tongue. METHODS: Speech and swallowing data were gathered at 4 evaluation times that spanned the first year of treatment for 32 patients treated with surgery and adjuvant radiation therapy. RESULTS: Speech intelligibility remained abov

Swallow recovery in an oral cancer patient following surgery, radiotherapy, and hyperthermia.

BACKGROUND: No study has examined the nature and extent of swallowing impairment in oral cancer patients following treatment with combined hyperthermia and interstitial radiotherapy. Few studies have examined the effects of voluntary swallow maneuvers (supersupraglottic and Mendelsohn) on pharyngeal phase swallowing in the oral cancer patient treated with surgery or radiotherapy. This study examined the effects of combined radiotherapeutic salvage treatments of hyperthermia and interstitial implantation and swallow recovery using swallow maneuvers in a surgically treated and irradiated oral ca

Chemotherapy in Combination With Methionine Restriction Induced A Complete Response in a Patient With Recurrent Metastatic Tongue Squamous-Cell Carcinoma.

BACKGROUND/AIM: Recurrent head and neck squamous-cell carcinoma (HNSCC) is associated with poor prognosis and limited treatment options, particularly after failure of standard therapies including surgery, radiotherapy, chemotherapy, and immune checkpoint inhibitors. Methionine restriction has emerged as a novel clinical metabolic therapeutic strategy targeting the methionine addiction of cancer cells. The present patient report describes a case of recurrent tongue HNSCC that achieved complete remission with chemotherapy combined with methionine restriction. CASE REPORT: A 35-year-old man was
