Medicaid and non-Medicaid case mix differences in Colorado nursing homes.

Profiles of case mix and related variables were compared for 1,064 Medicaid and 459 non-Medicaid residents of 65 freestanding nursing homes in Colorado in 1980. The results point to substantial case-mix differences, with Medicaid residents typically characterized by fewer and less intense long-term care problems as well as greater independence in functioning (ADLs). Policy impacts of these findings in Colorado have included a legislatively mandated home and community-based care program as well as a more stringent preadmission certification program for Medicaid clients. The method of comparing

Crying patterns of Korean infants in institutions.

This study investigates whether the crying pattern of infants reared in institutions is different from those reared at home in the same cultural backgrounds. Ninety-five institutional infants and 143 home infants, aged 2-18 weeks were studied through diaries, recorded by children's care-givers or mothers. Results showed that the crying duration of the infants in institutions was almost double that of infants at home (86 vs 45 min/day) and contact period with the care-givers was half that of home infants (136 vs 279 min/day). In addition, the duration of periods when the babies were alone was m

Home modifications to prevent home fall injuries in houses with Māori occupants (MHIPI): a randomised controlled trial.

BACKGROUND: As with many Indigenous populations internationally, Māori in New Zealand suffer health inequity. We aimed to assess the rate of fall injuries at home with and without home modifications in houses with Māori occupants. METHODS: We did a single-blind randomised controlled trial in the Wellington and Taranaki regions of New Zealand and enrolled owner-occupied households with at least one Māori occupant. Only households who stated they intended to live at that address for the subsequent 3 years were eligible for participation. We randomly assigned (1:1) households to either the inter

Relationship of perinatal outcomes to the competence and quantity of contact with community health workers.

BACKGROUND: The effectiveness of perinatal home visits by community health workers (CHWs) often diminishes when large regional or national programmes are implemented. To address this gap, we aimed to identify which CHW behaviours influence maternal and child outcomes. METHODS: We randomised all government-funded CHWs at eight deeply rural clinics (n = 43) by clinic to usual care, which consisted of home visits (control group; four clinics, 23 CHWs, 392 mothers), or to home visiting, which included improved monitoring and supervision (intervention group; four clinics, 20 CHWs, 423 mothers). Combined home and clinic-based therapy versus home-based exercise programme after distal radial fracture: a randomized controlled study.

We investigated outcomes of clinic-based hand therapy combined with a home-based exercise programme after anterior plating for distal radial fractures. A total of 102 patients were randomly assigned to one of three groups: a home-based exercise programme alone; a home-based exercise programme combined with four hand therapy sessions in the clinic; and a home-based exercise programme with seven sessions in the clinic. Mean Patient-Rated Wrist Evaluation scores at 6 weeks were significantly better for the group of patients with seven sessions in the clinic than in those with only home exercises

Patient perspectives and barriers to effective home-based care in lymphatic filariasis: A mixed methods study from Puducherry, India.

Morbidity Management and Disability Prevention (MMDP) remain an under-addressed component of lymphatic filariasis elimination. Despite the simplicity of the hygiene-based regimen, adherence to these practices remains low due to multiple barriers, exacerbating chronic suffering. As the primary role remains vested in the patients, this study explores their perspectives and barriers to home-based care to inform strategies to improve outcomes. This study employed a mixed-methods approach using an explanatory sequential design. The quantitative phase involved a cross-sectional survey of 321 adult p

'I was trying to get there, but I couldn't': social norms, vulnerability and lived experiences of home delivery in Mashonaland Central Province, Zimbabwe.

Increasing facility-based delivery rates is pivotal to reach Sustainable Development Goals to improve skilled attendance at birth and reduce maternal and neonatal mortality in low- and middle-income countries (LMICs). The translation of global health initiatives into national policy and programmes has increased facility-based deliveries in LMICs, but little is known about the impact of such policies on social norms from the perspective of women who continue to deliver at home. This qualitative study explores the reasons for and experiences of home delivery among women living in rural Zimbabwe.

Barriers to Institutional Delivery Services in Districts With the Lowest Institutional Delivery Coverage in Bhutan.

Bhutan aims to achieve 100% institutional delivery coverage. While evidence indicates improved institutional delivery coverage over the years, coverage in some of the districts is only 49%. This study was aimed at exploring barriers to institutional delivery in three low-coverage districts. In-depth qualitative interviews and six focus group discussions were conducted in December 2015. The analysis was done as per the Braun and Clarke's 6-phase guide to doing thematic analysis. This study sheds light on 15 barriers for institutional deliveries, which include hesitancy to seek health care when

Sense of Community among Female Residents of a Trauma-Informed Sober Living Home.

BACKGROUND: Substance use disorders (SUDs) and domestic and/or sexual violence (DSV) co-occur at high rates. Although some studies have examined sober living homes (SLHs), no research to date has evaluated SLHs specifically designed for women with histories of SUDS and DSV. OBJECTIVE: The purpose of this article is to present data from an exploratory analysis of the association between sense of community (SOC) and mental health symptoms within a unique trauma-informed and gender-responsive SLH (i.e., Support, Education, Empowerment, and Directions [SEEDs]). METHOD: Participants included curr

Engagement with Cognitively-Based Compassion Training is associated with reduced salivary C-reactive protein from before to after training in foster care program adolescents.

BACKGROUND: Children exposed to early life adversity (ELA) have been shown to have elevated circulating concentrations of inflammatory markers that persist into adulthood. Increased inflammation in individuals with ELA is believed to drive the elevated risk for medical and psychiatric illness in the same individuals. This study sought to determine whether Cognitively Based Compassion Training (CBCT) reduced C-reactive protein (CRP) in adolescents in foster care with high rates of ELA, and to evaluate the relationship between CBCT engagement and changes in CRP given prior evidence from our grou

Adverse events and development of tuberculosis after 4 months of rifampicin prophylaxis in a tuberculosis outbreak.

We screened tuberculosis (TB) contacts as an outbreak investigation with tuberculin skin test (TST) and interferon-gamma release assay (IGRA). We evaluated adverse events and TB incidence in all persons screened after rifampicin (RFP) prophylaxis, and specifically assessed the new TB cases in relation to initial TST and IGRA results. The 180 contacts were divided into four groups: TST+/IGRA+ (n = 101), TST+/IGRA- (n = 22), TST-/IGRA+ (n = 16), and TST-/IGRA- (n = 41). RFP treatment (4 months) was prescribed only to the TST+/IGRA+ group. Of 87 contacts who initiated prophylaxis, adverse events

"Couch surfing" of Latino foster care alumni: reliance on peers as social capital.

Youth exiting foster care often experience difficulties transitioning into adulthood. This paper focuses on Latino foster care youth in a major southwestern U.S. city and addresses the importance of peer networks as a crucial form of social capital as youth leave foster care. Case studies illustrate experiences of foster care alumni ranging in age from 18 to 26. Findings suggest that lack of housing forces youth into residential mobility or "couch surfing" and episodes of homelessness. Familial connections continue to be important to Latino youth. When Latino youth are unsuccessful in re-estab

Usage of support services in primary lateral sclerosis.

Patients with amyotrophic lateral sclerosis (ALS) rely on a variety of support services during the course of their illness. Patients with primary lateral sclerosis (PLS) have a slower progression of disease and different clinical spectrum. Their needs for allied health services and social support have not been well characterized. To investigate these needs, 25 patients with PLS and caregivers were surveyed on the use of assistive devices and support services. Needs for assistance changed as the disease progressed. Their greatest need was for gait-assistive devices and home help for activities

Two genealogies of supported housing and their implications for outcome assessment.

Drawing on ongoing fieldwork in New York City, the authors distinguish two "genealogies," or developmental traditions, of supported housing. "Housing as housing" originated in the mental health field to champion normalized, less-structured alternatives to clinically managed residential programs. "Integrated housing development" traces its origins to the movement to combat homelessness by preserving and creating affordable housing. The authors detail the distinctive premises, guiding concerns, and developmental logic of each lineage, contrasting the consumer advocate focus of the first genealog

A questionnaire survey of sleep and night-time behaviour in a community-based sample of adults with intellectual disability.

Two hundred and five people with intellectual disability, aged 18 years or over, and living in health- or social-services-managed community housing, completed sleep and behaviour questionnaires. An 85.7% return rate was achieved. Settling problems were present in 26.8% and night waking in 55.6% of the subjects. Parasomnias were present in 14% of subjects and 15% of the sample showed some features associated with sleep-related breathing problems. Factors associated with sleep-related problems included gender, aetiology of intellectual disability, epilepsy, treatment with antiepileptic medicatio

Structural problems in institutional care for youth.

In examining the relationship of emotional responsiveness and authoritarianism to the success of child welfare institutions as measured by running away behavior structural factors relating to both are discussed. The emotional deficit of the care giving situation seems to be a factor related to running away. Authoritarianism was not a related factor. Though both runaways and nonrunaways rated the institutional experience to be less emotionally responsive than their families, workers and runaways rated runaways experience as the least emotionally responsive. This is thought to relate to both run

Effectiveness of a community-based integrated service model for older adults living alone: A nonrandomized prospective study.

OBJECTIVE: Older adults living alone face physical, emotional, and social health problems, and prefer to age in place (AIP) in their homes. A community-based integrated model for AIP is needed and few studies have identified its impact on older adults living alone. METHODS: This was a non-randomized prospective study. Participants were 877 community-dwelling older adults living alone, aged above 65 years, in S* city in South Korea. The intervention group (n = 331) received a community-based integrated service (CBIS) model based on AIP for six months from October 2019 to April 2020. RESULTS:

Multimorbidity Among Patients Receiving Home Care in Japan: Prevalence, Levels, and Association With Emergency Home Visits.

BACKGROUND: Despite its clinical importance in primary care, the prevalence and levels of multimorbidity among patients receiving home-based medical care remain poorly characterized, and its potential impact on emergency home visits is unclear. This study described the prevalence and levels of multimorbidity among patients receiving physician-led home care in Japan and examined the association between multimorbidity level and emergency home visits. METHODS: A multicenter cross-sectional study was conducted in 2024 using physician-completed questionnaires derived from medical records. Multimor
