The diagnostic approach to tall stature in children is based on collecting birth data (macrosomia), sizes and family puberty, a family history of constitutional or pathological tall stature, search for a delay of development, dysmorphia, disproportion, analysis of the growth velocity (normal or accelerated), general examination and assessment of puberty, and bone age. When there is a history of psychomotor retardation, a family history of pathological tall stature, or a disproportion in the clinical examination, the genetic causes of tall stature will be mentioned. The most frequent causes are

When other causes of retarded growth have been ruled out, investigation for classic growth hormone deficiency is indicated in children with reduced velocity of growth and retarded bone development. In cases of classic growth hormone deficiency there is insufficient increase in growth hormone levels after two stimulation tests. However, some short children whose stimulation tests are normal but whose spontaneous growth hormone secretion is reduced or pathological may possibly benefit from growth hormone treatment. Therefore measurements of spontaneous growth hormone secretion and insulin-like g

The treatment of systemic diseases with glucocorticoids is often associated with decreased height velocity (HV), and can result in shorter final height. Interactions between adrenal and GH-IGF axis have been described and can occur at hypothalamic-pituitary level or at the regulation of IGF system, including the IGF1R signaling. The clinical state of these patients may be considered as an absolute and/or functional IGF-1 deficiency. Interventions aiming to restore the normal function of GH-IGF axis might reduce the glucocorticoids-induced growth suppression in these children. It has been shown

INTRODUCTION: A significant increase in growth velocity is observed during recombinant human growth hormone (rhGH) therapy in patients with growth hormone deficiency (GHD), especially just after the beginning of treatment. This phenomenon is referred to as catch-up growth". After some time, the growth velocity decreases to the physiological value, i.e. the value that is observed in healthy children. The treatment is continued until the time of the growth process is completed. The continuity of the therapy makes it impossible to assess whether the catch-up phenomenon occurs only at the beginnin

INTRODUCTION: Short stature in Klinefelter's syndrome was reported sporadically and was connected with the karyotype 49,XXXXY.
AIM OF THE STUDY: The presentation of growth in a boy with Klinefelter's syndrome and somatotropine deficiency.
CASE REPORT: A 10-year-old boy was examined in the Department of Endocrinology because of short stature and mild mental retardation. On admission, a height 117 cm (<3 percentile) and a body weight 20 kg (<3 percentile) were reported. The boy was in the prepubertal stage of life. The bone age was delayed by 5 years in comparison to the calendar age. Serum le

Two weekly (biw) injections of pituitary human growth hormone (hGH) were compared with three weekly (tiw) injections of hGH (0.24 IU hGH/kg body weight/wk for both groups) for effect on growth rate in children with idiopathic and organic hypopituitarism. Both dosage schedules resulted in increased growth rate during the first 12 months of therapy; the tiw dosage was more effective in the first 6 months in patients with organic hypopituitarism, and in the second 6 months in patients with idiopathic hypopituitarism. There was no advantage to tiw therapy during the second year of therapy in eithe

OBJECTIVES: The aim of the study was to evaluate the dental and bone age delay and occlusal traits of children with growth hormone deficiency (GHD) and idiopathic short stature (ISS).
MATERIAL AND METHODS: The study group included 46 patients aged 5 to 14 years: 15 with ISS, 17 with GHD before growth hormone treatment, and 14 with GHD during substitution therapy. The control group consisted of 46 age and sex-matched subjects of normal height. A calibrated dentist assessed all subjects in terms of dental age and occlusal characteristics. Bone age was evaluated only in GHD and ISS children as a

Growth hormone treatment was introduced in the 1950s to address growth disturbances and metabolic abnormalities. Hundreds of thousands of children have been treated, with gradual expansion of treatment indications. From initially being offered only to patients with severe growth hormone deficiency, today many children are treated for conditions in which the associated short stature is not primarily thought to be due to deficient endogenous growth hormone secretion. This review discusses the history, physiology and safety of growth hormone treatment, with focus on the long-term risks of mortali

PURPOSE OF REVIEW: Metabolic and genetic bone disorders affect not only bone mass but often also the bone material, including degree of mineralization, matrix organization, and lacunar porosity. The quality of juvenile bone is moreover highly influenced by skeletal growth. This review aims to provide a compact summary of the present knowledge on the complex interplay between bone modeling and remodeling during skeletal growth and to alert the reader to the complexity of bone tissue characteristics in children with bone disorders.
RECENT FINDINGS: We describe cellular events together with the

Considerable progress has been made in the delineation of the genetic skeletal dysplasias, a heterogeneous group of disorders, that consist of over 80 distinct conditions. Morphologic studies have added a further dimension to the delineation of these conditions, their diagnosis, and the investigation of their pathogenetic mechanisms. In certain diseases, the morphologic alterations are characteristic and pathognomonic. In others only nonspecific alterations are observed, whereas in still other disorders growth-plate structure is essentially normal. Histologic, histochemical, and electronmicros

BACKGROUND: Hypertensive disorders of pregnancy and fetal growth restriction share common etiopathological origins and could be caused by maternal hemodynamic maladaptation to pregnancy.
OBJECTIVE: The aim of our study is to evaluate if there is a correlation between maternal hemodynamic detected by UltraSonic Cardiac Output Monitor (USCOM®) during the first trimester and the pregnancy outcome.
STUDY DESIGN: We recruited a nonconsecutive series of women in the first trimester of pregnancy with no previous history of hypertensive disorders. We measured the pulsatility index uterine arteries a

The clinical availability of recombinant hematopoietic growth factors was initially thought to be breakthrough in the treatment of bone marrow failure syndromes. However, in most disorders of hematopoeisis, the clinical use was rather disappointing. Only in congenital neutropenias (CNs) has the long-term administration of granulocyte colony-stimulating factor (G-CSF) led to a maintained increase in absolute neutrophil count (ANC) and a reduction of severe bacterial infections. In other disorders of hematopoiesis, the use of lineage-specific growth factors is either not possible due to mutation

The endothelial cells play a crucial role in the progression of angiogenesis, which causes cell re-modulation, proliferation, adhesion, migration, invasion and survival. Angiogenic factors like cytokines, cell adhesion molecules, growth factors, vasoactive peptides, proteolytic enzymes (metalloproteinases) and plasminogen activators bind to their receptors on endothelial cells and activate the signal transduction pathways like epidermal growth factor receptor (EGFR phosphatidylinositol 3-kinase and (PI3K)/AKT/mammalian target of rapamycin (mTOR) which initiate the process of angiogenesis.
