Structural determinants in quality of care for older adults: a nursing perspective on medical services - qualitative findings from focus group discussions in Germany.

BACKGROUND: Aging populations, multimorbidity, and rapid medical progress have expanded treatment options in old age, but also intensified ethical and organizational challenges in everyday care. Little is known about how nurses, as key frontline professionals, experience these structural developments specifically in the context of medical and nursing care for older adults in Germany. Therefore, this study aimed to examine how nursing staff perceive structural conditions in the German healthcare system and their effects on medical and nursing care for older patients. METHODS: This study employ

Enhancing tuberculosis care in the private sector: Role of innovative private sector engagement model under programmatic settings in India.

India's private sector plays a crucial role in the country's tuberculosis (TB) healthcare landscape, with >50% of patients seeking initial care from private providers. Recognizing this critical role, the National Tuberculosis Elimination Program implemented and scaled up an innovative private sector engagement model, Patient Provider Support Agency (PPSA), in over 200 districts of the country in 2023 to improve TB care in the private sector. A cross-sectional study was conducted to assess the role of PPSA in improving TB care services among private-sector patients in 2023. We compared district

Socioeconomic Determinants of Access to Medicines Among Romanian Patients with Chronic Diseases: A Cross-Sectional Study.

Background and Objectives: Access to medicines is a fundamental determinant of health equity and a core pillar of universal health coverage, encompassing the timely availability, affordability, and appropriate use of essential medicines. Socioeconomic disparities may limit actual and timely access to pharmacological treatment, particularly in healthcare systems characterized by mixed public-private financing and significant out-of-pocket expenditures. This study aimed to evaluate socioeconomic determinants of access to medicines among Romanian patients with chronic diseases, focusing on income

Multi-sector determinants of implementation and sustainment for non-specialist treatment of depression and post-traumatic stress disorder in Kenya: a concept mapping study.

BACKGROUND: The global shortage of trained mental health workers disproportionately impacts mental health care access in low- and middle-income countries. In Kenya, effective strategies are needed to scale-up the workforce to meet the demand for depression and post-traumatic stress disorder treatment. Task-shifting - delegating specific tasks to non-specialist workers - is one workforce expansion approach. However, non-specialist workers remain underutilized in Kenya due to a paucity of research on how to scale-up and sustain such service models. METHODS: Purposive sampling was used to recrui

Investment case for primary health care in low- and middle-income countries: A case study of Kenya.

BACKGROUND: Primary healthcare (PHC) systems attain improved health outcomes and fairness and are affordable. However, the proportion of PHC spending to Total Current Health Expenditure in Kenya reduced from 63.4% in 2016/17 to 53.9% in 2020/21 while external funding reduced from 28.3% (Ksh 69.4 billion) to 23.9% (Ksh 68.2 billion) over the same period. This reduction in PHC spending negatively affects PHC performance and the overall health system goals. METHODS: We conducted a cost-benefit analysis and computed costs against the economic benefits of a PHC scale-up. Activity-Based Costing (AB

Developing a length of stay prediction model for newborns, achieving better accuracy with greater usability.

BACKGROUND: One in ten newborn children is born prematurely. The elongated length of stay (LOS) of these children in the Neonatal Intensive Care Unit (NICU) has important implications on hospital occupancy figures, healthcare and management costs, as well as the psychology of parents. In order to allow accurate planning and resource allocation, this study aims to create a generalizable and robust model to predict the NICU LOS of preterm newborns. METHODS: Data were collected from a large tertiary center NICU between 2011 and 2018 and relates to 5,362 newborns. The selected model was externall

Perceptions and drivers of healthcare provider and drug dispenser practices for the treatment of malaria in pregnancy in the context of multiple first-line therapies in western Kenya: a qualitative study.

BACKGROUND: Emergence of Plasmodium falciparum resistance to artemether-lumefantrine in Africa prompted the pilot introduction of multiple first-line therapies (MFT) against malaria in Kenya, potentially exposing women-of-childbearing-age (WOCBAs) to anti-malarials with unknown safety profiles in the first trimester. This qualitative study explored knowledge and perceptions among healthcare providers providing malaria treatment to WOCBAs and pregnant women. METHODS: In-depth interviews were conducted with purposively selected public and private health facility (HF) and drug outlet (DO) provid

Who pays and how much? A cross-sectional study of out-of-pocket payment for modern contraception in Kenya.

OBJECTIVES: Out-of-pocket (OOP) payment for modern contraception is an understudied component of healthcare financing in countries like Kenya, where wealth gradients in met need have prompted efforts to expand access to free contraception. This study aims to examine whether, among public sector providers, the poor are more likely to receive free contraception and to compare how OOP payment for injectables and implants-two popular methods-differs by public/private provider type and user's sociodemographic characteristics. DESIGN, SETTING AND PARTICIPANTS: Secondary analyses of nationally repre…

Performance-Based Financing Empowers Health Workers Delivering Prevention of Vertical Transmission of HIV Services and Decreases Desire to Leave in Mozambique.

BACKGROUND: Despite increased access to treatment and reduced incidence, vertical transmission of HIV continues to pose a risk to maternal and child health in sub-Saharan Africa. Performance-based financing (PBF) directed at healthcare providers has shown potential to improve quantity and quality of maternal and child health services. However, the ways in which these PBF initiatives lead to improved service delivery are still under investigation. METHODS: Therefore, we implemented a longitudinal-controlled proof-of-concept PBF intervention at health facilities and with community-based associa

[Enhancing awareness of the modalities in the management of chronic inflammatory diseases].

From an epidemiologic point of view, the incidence of rheumatoid arthritis in the French regions we studied appears relatively stable and about the same as the prevalence of rheumatoid spondylosis. The incidence of Crohn's disease, among the chronic inflammatory bowel diseases, is currently increasing. Treatment for rheumatoid arthritis, other than symptomatic and maintenance treatments, now includes various specific forms of "biotherapy". Their development was made possible through the decoding of the pathophysiological mechanisms of inflammation and joint destruction, basically involving pro

"Who am I going to stay with? Who will accept me?": family-level domains influencing HIV care engagement among disengaged adolescents in Kenya.

INTRODUCTION: Adolescents living with HIV (ALHIV, ages 10-19) have developmentally specific needs in care, and have lower retention compared to other age groups. Family-level contexts may be critical to adolescent HIV outcomes, but have often been overlooked. We investigated family-level factors underlying disengagement and supporting re-engagement among adolescents disengaged from HIV care. METHODS: Semi-structured interviews were performed with 42 disengaged ALHIV, 32 of their caregivers and 28 healthcare workers (HCW) in the Academic Model Providing Access to Healthcare (AMPATH) program in

Lyme snap: A feasibility study of on-line declarations of erythema migrans in a rural area of France.

The incidence of Lyme borreliosis remains a matter of debate, but it can be estimated using the incidence of erythema migrans (EM), which is pathognomonic of the first phase. The aim of this prospective pilot study was to assess the feasibility of the on-line declaration of EM in rural areas where the incidence of Lyme borreliosis was previously estimated at 85 per 100,000 inhabitants per year. The study was limited to a rural area (Les Combrailles, Auvergne) of approximately 52,800 inhabitants and was preceded by an information campaign for the inhabitants and the healthcare professionals. Clinical Usage and Economic Effectiveness of a Recently Developed Epidermal Autograft Harvesting System in 13 Chronic Wound Patients in a University-Based Wound Center.

INTRODUCTION: Chronic wounds are a significant healthcare problem in the United States. Their costs approach 25 billion dollars in the United States. Current wound-care treatments of local wound care, moist dressings, and source control, while necessary for wound healing, are frequently not enough to ensure complete wound closure. The current surgical technique of split-thickness skin grafting is an operative procedure, painful, time-consuming, and leaves significant donor site wounds. A recently developed and marketed epidermal autograft harvester was tested at our university hospital wound c

Health in South Africa: changes and challenges since 2009.

Since the 2009 Lancet Health in South Africa Series, important changes have occurred in the country, resulting in an increase in life expectancy to 60 years. Historical injustices together with the disastrous health policies of the previous administration are being transformed. The change in leadership of the Ministry of Health has been key, but new momentum is inhibited by stasis within the health management bureaucracy. Specific policy and programme changes are evident for all four of the so-called colliding epidemics: HIV and tuberculosis; chronic illness and mental health; injury and viole

Socioeconomic inequalities in unmet dental care needs in Spain.

OBJECTIVE: Socioeconomic inequalities in access to dental care remain a critical public health challenge in Spain, where adult coverage is limited and largely financed through out-of-pocket payments. This study aims to estimate the magnitude of socioeconomic inequality in unmet dental care needs among the Spanish adult population in 2024 and to decompose the contribution of its determinants. METHODS: A cross-sectional analysis was conducted using data from the 2024 Spanish Living Conditions Survey (ECV). The analytic sample included 32,425 adults who reported a perceived need for dental care

Investigating the impact of chronic graft‑versus‑host disease on patient health‑related quality of life: perspectives from patients and carers.

BACKGROUND: Chronic graft-versus-host disease (cGvHD) is a common complication that can occur following hematopoietic stem cell transplantation. This complex, multisystemic condition often requires multiple lines of therapy to treat both the symptoms of cGvHD and the side effects associated with these treatments. Comprehensive data on the impact of cGvHD on health-related quality of life (HRQoL) of patients who have undergone multiple lines of therapy, remain limited. Greater awareness of these challenges could lead to more personalized treatment plans and inform future research. METHODS: We
