Sonographic dynamic assessment of lung injury in a child with hypoplastic left heart syndrome undergoing extracorporeal membrane oxygenation.

INTRODUCTION: Performed for many years in clinical settings, pleural and lung ultrasound (PLUS) has emerged to be an invaluable tool to diagnose underlying conditions of respiratory failure, to monitor disease progression and to ensure appropriate therapeutic intervention. PLUS basically relies on the analysis of two prevalent ultrasound artefacts: A-lines and B-lines. A-lines are hyperechoic reverberation artefacts of the pleural line. A-lines combined with lung sliding show that lungs are well aerated. B-lines are vertical hyperechoic reverberation artefacts arising from pleural line extendi

Mitochondrial permeability transition in the diabetic heart: contributions of thiol redox state and mitochondrial calcium to augmented reperfusion injury.

Mitochondria from diabetic hearts are sensitized to mitochondrial permeability transition pore (PTP) opening, which may be responsible for the increased propensity for cardiac injury in diabetic hearts. The purpose of this study was to determine if redox-dependent PTP opening contributes to augmented injury in diabetic hearts, and if compounds targeted at mitochondrial PTP, ROS, and calcium influx protected diabetic hearts from injury. Hearts from control or streptozotocin-induced diabetic rats were excised for either whole-heart or isolated mitochondria experiments. Myocardial glutathione con

Heart rate and pulse pressure variability are associated with intractable intracranial hypertension after severe traumatic brain injury.

Calculation of integer heart rate variability (HRVi) permits monitoring over extended periods. We asked whether continuous monitoring of HRVi or integer pulse pressure (PP) variability (PPVi) could predict intracranial hypertension, defined as ICP >20 mm Hg, cerebral hypoperfusion, defined as CPP<60 mm Hg, mortality or functional outcome after severe traumatic brain injury. Dense integer data collected during continuous intensive care unit monitoring for periods of 1 to 11 days on 25 patients admitted to our Level I trauma center with Glasgow Coma Scale <9 provided 1,715,000 data points over a

Simvastatin alleviates myocardial contractile dysfunction and lethal ischemic injury in rat heart independent of cholesterol-lowering effects.

Up to 5% of the population suffer from systemic, 19% from local allergic hypersensitivity reactions to stinging insects. Even though specific immunotherapy is very effective in treating allergy to insect venom, new concepts of treatment strategies with only the disease eliciting allergen in recombinant form, along with antigen application via a less invasive route might be suggested for enhanced treatment efficacy and compliance. In the present study we aimed (i) to establish a mouse model of wasp venom allergy, mimicking the natural mode of sensitization, and (ii) to develop a prophylactic tr

[Resuscitation using percutaneous cardiopulmonary support and beating heart surgery with balloon technique for a patient with penetrating cardiac injury].

We used percutaneous cardiopulmonary support (PCPS) and beating heart hemostasis with a balloon technique to resuscitate an 80-year-old woman who had been stabbed in the chest with pruning shears. She developed pulseless tamponade while waiting for surgery. Resuscitative thoracotomy was impossible in the emergency room due to the lack of appropriate equipment. We therefore decided to immediately resuscitate her using PCPS. Life-support circulation was established within 13 minutes and provided a stable period before surgery. The stab wound in the right ventricle was temporarily controlled with

Junctional adhesion molecule-A-deficient polymorphonuclear cells show reduced diapedesis in peritonitis and heart ischemia-reperfusion injury.

Junctional Adhesion Molecule-A (JAM-A) is a transmembrane adhesive protein expressed at endothelial junctions and in leukocytes. Here we report that JAM-A is required for the correct infiltration of polymorphonuclear leukocytes (PMN) into an inflamed peritoneum or in the heart upon ischemia-reperfusion injury. The defect was not observed in mice with an endothelium-restricted deficiency of the protein but was still detectable in mice transplanted with bone marrow from JAM-A(-/-) donors. Microscopic examination of mesenteric and heart microvasculature of JAM-A(-/-) mice showed high numbers of P

Future research directions in acute lung injury: summary of a National Heart, Lung, and Blood Institute working group.

Acute lung injury (ALI) and its more severe form, the acute respiratory distress syndrome (ARDS), are syndromes of acute respiratory failure that result from acute pulmonary edema and inflammation. The development of ALI/ARDS is associated with several clinical disorders including direct pulmonary injury from pneumonia and aspiration as well as indirect pulmonary injury from trauma, sepsis, and other disorders such as acute pancreatitis and drug overdose. Although mortality from ALI/ARDS has decreased in the last decade, it remains high. Despite two major advances in treatment, low VT ventilat

Injury to rat hearts produced by an exogenous free radical generating system. Study into the role of arachidonic acid and eicosanoids.

This study was designed to evaluate the effect of an exogenous free radical generating system consisting of purine plus xanthine oxidase on the isolated rat heart and in particular to assess the possible contribution of arachidonic acid or its metabolites to toxicity produced by this drug combination. Purine plus xanthine oxidase produced a time-dependent depression in cardiac contractility which was associated with stimulated release of lactate dehydrogenase (LDH). Electron microscopic analysis revealed a distinct separation of the glycocalyx from the sarcolemmal membrane with no apparent int

A new beating heart model of cardiopulmonary bypass-induced acute kidney injury demonstrates heme protein toxicity and ferroptosis.

BACKGROUND: Cardiac surgery-associated acute kidney injury (CSA-AKI) occurs in 20-30% of patients after heart surgery with cardiopulmonary bypass (CPB). AKI rates are even higher in VA ECMO treated patients. The pathogenesis of CSA-AKI is incompletely understood, and preventative therapies are lacking. We hypothesize that CSA-AKI occurs due to pump related RBC hemolysis and accumulation of cell-free hemoglobin (CFH) and hemopexin in the kidneys. METHODS: A new beating heart model of CPB was developed. Rats were used for CPB experiments; blood pressure (BP) was measured by Millar invasive BP c

Therapeutic Strategies Targeting the Kidney-Liver-Immune-Heart Network: Circadian and Mechanosensory Pathways in CKD-Associated Cardiac Injury.

The present review discusses vitamin A/retinoid metabolism as a cross-organ axis in which hepatic clock-dependent retinoid handling may affect immune clock gene expression through the stimulation of retinoic acid 6-Janus kinase 2-signal transducer and activator of transcription 5 signaling, potentially promoting pro-inflammatory monocyte states. We further highlight mechanosensory signaling as a second convergent layer that integrates hemodynamic forces with tissue microenvironmental cues. Among these pathways, G protein-coupled receptor 68, a proton- and flow-sensitive G protein-coupled recep

Enhancing early detection of acute brain injury using heart rate variability in patients on venoarterial extracorporeal membrane oxygenation.

IntroductionEarly detection of acute brain injury (ABI) in patients on venoarterial extracorporeal membrane oxygenation (VA ECMO) is challenging. We aimed to investigate if heart rate variability (HRV) in the first 24 h of ECMO cannulation correlated with ABI in VA ECMO patients.MethodsWe retrospectively analyzed adults (≥18 years) on VA ECMO support in our center (June 2016-April 2022). ABI included ischemic stroke, intracranial hemorrhage (ICH), and hypoxic ischemic brain injury (HIBI) as diagnosed on neuroimaging performed within 48 to 72 h of ECMO cannulation. The primary outcome was ABI,

Exogenous MOTS-c mitigates myocardial ischemia-reperfusion injury: experimental and in silico evidence from rat heart models.

The mitochondrial-derived peptide MOTS-c regulates metabolic and cellular stress responses, but its dose-response profile and direct cardioprotective mechanisms in myocardial ischemia-reperfusion injury (MIRI) remain undefined. This proof-of-concept study aimed to identify the optimal cardioprotective dose of exogenous MOTS-c and delineate its multi-pathway mechanisms using an ex vivo rat heart IR model with in silico support. Isolated Langendorff-perfused rat hearts underwent 30-min global ischemia and 60-min reperfusion with or without MOTS-c (0.25-0.7 mg/kg) delivered via Krebs-Henseleit bu

Subendocardial burn injury causes ST segment depression in bullfrog heart mimicking electrocardiographic change in subendocardial myocardial infarction.

Non-ST segment elevation myocardial infarction (NSTEMI) is caused by partial occlusion of a coronary artery due to atherosclerotic plaque disruption and superimposed thrombus formation. Common electrocardiographic findings in NSTEMI include permanent ST segment depression, indicative of subendocardial myocardial infarction. In our earlier research, we simulated the ST segment elevation observed in human ischemic heart disease by inducing subepicardial burn injuries in the bullfrog heart ventricle. In the present study, we successfully replicated ST segment depression, similar to that observed

C-Reactive Protein as a Biomarker of Inflammation and Graft Injury After Kidney and Heart Allo- and Xenotransplantation Into Non-Human Primates.

INTRODUCTION: C-reactive protein (CRP) is a widely used inflammatory biomarker. We evaluated CRP alongside tissue injury markers in non-human primate (NHP) models of kidney and heart allo- and xeno-transplantation to assess CRP's sensitivity in detecting graft-specific inflammatory responses (acute rejection), and explored CRP elevations in the absence of clinically evident graft injury. A secondary objective compared CRP dynamics between NHP species. METHODS: Forty-two baboon or cynomolgus monkey NHPs received kidney (n = 12) or heart (n = 7) xenografts, or kidney (n = 12) or heart (n = 11)

Renal macro- and microcirculatory perturbations in acute kidney injury and chronic kidney disease associated with heart failure and cardiac surgery.

Chronic kidney disease (CKD) affects 50% of patients with heart failure. The pathophysiology of CKD in heart failure is proposed to be driven by macrocirculatory hemodynamic changes, including reduced cardiac output and elevated central venous pressure. However, our understanding of renal microcirculation in heart failure and CKD remains limited. This is largely due to the lack of noninvasive techniques to assess renal microcirculation in patients. Moreover, there is a lack of clinically relevant animal models of heart failure and CKD to advance our understanding of the timing and magnitude of

Dobutamine-Induced Myoclonus in a Patient With Acute Decompensated Heart Failure and Acute Kidney Injury: A Case Report and Literature Review.

Dobutamine is a potent β1 and a weak β2 adrenergic agonist used in the treatment of patients with acute decompensated heart failure or cardiogenic shock. Its positive inotropic effects enhance myocardial contractibility, leading to increased cardiac output. Myoclonus in patients receiving dobutamine is a rare adverse event that is not completely understood but seems to occur more commonly in patients with renal insufficiency. We present the unique case of a 61-year-old female receiving treatment for acute decompensated heart failure with acute kidney injury (AKI) who developed cortical myoclon

Acute kidney injury recovery status predicts mortality and cardiorenal outcomes in patients admitted with acute decompensated heart failure.

BACKGROUND: Acute kidney injury (AKI) in the context of acute decompensated heart failure (ADHF) encompasses a broad spectrum of phenotypes with associated disparate outcomes. We evaluate the impact of 'ongoing AKI' on prognosis and cardiorenal outcomes and describe predictors of 'ongoing AKI'. METHODS: A prospective multicentre observational study of patients admitted with ADHF requiring intravenous furosemide was completed, with urinary angiotensinogen (uAGT) measured at baseline. AKI was defined using Kidney Disease Improving Global Outcomes (KDIGO) AKI criteria. AKI recovery status was de
