BACKGROUND: Patients with small aortic annulus undergoing isolated aortic valve replacement face an often underestimated surgical risk. We describe initial clinical results and long-term follow-up of this particular high-risk group.
METHODS: Between January 1998 and December 2004, 148 consecutive patients with small aortic annulus underwent isolated aortic valve replacement by implantation of a Mitroflow Aortic Pericardial Heart Valve (Sorin S.p.A., Milano, Italy) 19 or 21 mm bioprostheses. Mean age was 75.4 ± 6.2 years. Female gender, obesity, and multiple comorbidities were predominant. Mea

After aortic valve stenosis, mitral regurgitation (MR) is the second most common valvular disease, particulary affecting older patients. Optimal medical treatment within the context of heart failure therapy is the favored first-line therapy for secondary MR. If symptoms persist despite optimal medical therapy, surgical or transcatheter mitral valve repair is indicated (recommendation class IIb). In contrast, surgical treatment is essential for patients with symptomatic primary MR and left-ventricular ejection fraction (LVEF) >30% and justifiable perioperative risk (repair preferred over replac

BACKGROUND: In patients with refractory heart failure, aortic valve (AV) insufficiency or the presence of a prosthetic AV has been considered a relative contraindication to left ventricular assist device (LVAD) therapy. Nevertheless, we have successfully implanted LVADs in 5 patients with native AV insufficiency or an AV prosthesis by closing the left ventricular outflow tract (LVOT).
METHODS: The method of LVOT closure was tailored to the patients' differing circumstances. Patient 1 had a regurgitant tri-leaflet native AV. Patient 2 had undergone previous aortic root reconstruction with a po

The aim of this review of 20 cases was to identify the surgical indications in heart failure complicating the acute phase of left heart valve endocarditis, to determine the optimal timing of surgery and to evaluate the early and medium term postoperative prognosis. Between January 1985 and May 1990, 20 patients (14 men and 6 women with an average age of 29 +/- 7 years) with native left heart valve endocarditis underwent surgery in the acute phase complicated by heart failure. The were 9 aortic regurgitations and 2 mixed mitral and tricuspid regurgitations. The haemodynamic status of the patien

Propionibacterium acnes, a common human skin organism [Perry A.L., Lambert P.A., Under the microscope Propionibacterium acnes, Lett App Microbiol 2006; 42:185-186], mostly considered a contaminant, has rarely been associated with cases of infectious endocarditis [Vanagt W.Y., Daenen W.J., Delhaas T., Propionibacterium acnes endocarditis on an annuloplasty ring in an adolescent boy, Heart 2004; 90:56]. We report on a 48-year old man with a history of mitral valve replacement who acutely developed dyspnoea, tachypnoea and forward failure. Transesophageal echocardiography showed a dehiscence of t

2013 was the year of transcatheter heart valve interventions. Not least because of the 2012 European guidelines on the management of valvular heart disease (VHD), the multidisciplinary heart team approach became an established concept. Decision-making, when a patient is too 'sick' for surgery and too 'Работает' for catheter-based interventions, is complex, since VHD is often seen at an older age and, as a consequence, there is a higher frequency of co-morbidity and frailty. However, before TAVI and other transcatheter heart valve interventions can be expanded to intermediate-risk patients, evid

The authors used an original method to study the general and regional function of the left ventricle of the heart in patients in the late-term periods after complicated reconstructive operations and mitral valve prosthetics. In patients of the second group increased volumes of the heart were maintained, diminished left ventricular ejection fraction and marked disorders of function in the basal segments were revealed which the authors believe to be caused by excision of the chordo-papillary++ apparatus of the mitral valve and firm fixation of the fibrous ring by the cuff of the prosthesis. At present, the majority of surgical heart valves (SHVs) implanted are bioprosthetic valves. Over time however, these are prone to structural deterioration, which may manifest as valvular stenosis, regurgitation or a combination of the two. Re-operation is the current standard of care for these patients but this itself carries a significant risk of mortality and morbidity. As a natural extension of transcatheter aortic valve implantation (TAVI), now an evidence based solution for severe aortic stenosis in high-risk patients, valve-in-valve (VIV) therapy is evolving into an alternative option i

Patients with aortic incompetence frequently present with anatomical and pathological challenges such as elliptical dilated annulus, dilated aortic root, dilated ascending aorta, and with no calcification in the aortic cusps or annulus. Patients are commonly in graver clinical condition as a result of a long silent clinical course before overt congestive heart failure. All of the above make transcatheter therapies for native aortic valve regurgitation more challenging with poorer outcomes, escalating the risk of insufficient anchoring, prosthesis migration, and residual paravalvular leak compa

Infective endocarditis can be associated with complex clinical presentations, sometimes with a difficult multi-disciplinary management. Actinobacillus actinomycetemcomitans belongs to the Haemophilus species, Actinomycetemcomitans, Cardiobacterium hominis, Eikenella corrodens and Kingella species group, responsible for 5% to 10% of infective endocarditis in native heart valves. These organisms have slow fastidious growth pattern, often associated with negative cultures, and cause systemic embolism with abscess formation. The authors present the case of a 59-year-old man, admitted due to fever

Calcific aortic stenosis is a disorder that impacts the physiology of heart valves. Fibrocalcific events progress in conjunction with thickening of the valve leaflets. Over the years, these events promote stenosis and obstruction of blood flow. Known and common risk factors are congenital defects, aging and metabolic syndromes linked to high plasma levels of lipoproteins. Inflammation and oxidative stress are the main molecular mediators of the evolution of aortic stenosis in patients and these mediators regulate both the degradation and remodeling processes. Mitochondrial dysfunction and dysr

Heart lesions in patients with rheumatoid arthritis (RA) are well documented in literature; however, in the majority of cases these are incidental findings at postmortem. Most patients do not require cardiac surgical intervention unless they develop complications such as significant valvular regurgitation. Patients with RA often require orthopedic operations and therefore a bioprosthetic valve replacement is normally advocated to avoid problems related to anticoagulation. We report a case of a 64-year-old woman with seropositive RA who had undergone bioprosthetic aortic valve replacement three

OBJECTIVE: Partial left ventriculectomy (PLV) (also known as Batista's Procedure) is a surgical procedure for treatment of dilated cardiomyopathy when cardiac transplant is contraindicated. Mitral valve replacement is needed because of mitral regurgitation as a consequence of annulus enlargement and papillary muscle resection. Bleeding and arrythmias are the main complications.
METHODS: We considered for this operation a 60-year-old male patient. He suffered from valvular dilating cardiomyopathy as a consequence of mitral and aortic valve regurgitation. Furthermore, a severe peripheral vascul

OBJECTIVE: Surgical treatment of rheumatic valvular disease still constitutes a significant number of cardiac operations in developing countries. Despite improvements in myocardial protection and cardiopulmonary bypass techniques, triple valve operations (aortic, mitral and tricuspid valves) are still challenging because of longer duration of cardiopulmonary bypass and higher degree of myocardial decompensation. This study was instituted in order to assess results of triple valve surgery.
METHODS: Between 1977 and 2002, 34 patients underwent triple valve surgery in our clinic by the same surg

OBJECTIVE: To evaluate the hemodynamic performance (i.e., gradients and paravalvular leakage [PVL]) of the new and experimental Braile Inovare® Proseal. Additionally, we aimed to assess pre and postoperatively the aortic annulus and the transcatheter prosthesis using multislice computed tomography (MSCT).
METHODS: Patients were selected by a multidisciplinary heart team and referred for transcatheter aortic valve replacement (TAVR). MSCT was performed before and after surgery. Measurements of the aortic valve and prosthesis were conducted and correlated with the valve gradient and residual PV

Intraaortic balloon pump (IABP) is being used in cardiac surgery in an increased ratio. IABP therapy involves considerable risk, mainly vascular complications, postoperative bleeding and infection can represent danger. Between 1999 and 2004 out of 4443 open heart surgery operations we have performed intraaortic balloon pump treatment in case of 75 patients. The mean age was 64 years, 23 patients had diabetes mellitus, 47 patients had hypertension, 20 patients had peripheral vascular disease as well. We performed IABP therapy most frequently during isolated coronary bypass operations (42 cases)

Surgery for radiation-associated valve disease is technically demanding due to challenging exposure, extensive calcification, and risk of paravalvular leak or atrioventricular groove disruption. An oblique aortotomy revealed heavy calcification extending to the aortomitral continuity, where only a 23 mm sizer could barely pass. The aortotomy was extended into the non-coronary sinus adjacent to the left-non commissure. An extended transseptal approach was employed and connected to the aortotomy. The anterior mitral leaflet was excised. From trigone to trigone, non-everting buttressed stitches w

AIMS: The aim of this nationwide descriptive study is to assess short-, mid-, and long-term mortality risk following aortic valve replacement (AVR), mitral valve replacement (MVR) and mitral valve plasty (MVP) in patients with native valve endocarditis (NVE) and prosthetic valve endocarditis (PVE). Mortality outcomes were also evaluated for biological and mechanical valve prostheses.
METHODS AND RESULTS: Patients with infective endocarditis who underwent AVR, MVR, or MVP between 2013 and 2022 were included from the Netherlands Heart Registration. Patients were excluded in case of combined val
