Left ventricular assist devices: current status and future applications.

Despite the advances in the medical management of heart failure over the last 20 years it remains a major cause of morbidity and mortality. While cardiac transplantation has evolved into an established mode of therapy, the number of patients with severe heart failure who could benefit from cardiac transplantation far exceeds the supply of donor organs. The development of an implantable left ventricular assist device (LVAD) began in the early 1970s in centres such as the Texas Heart Institute and was funded by the National Heart, Lung, and Blood Institute. Clinical trials of these devices began

Embolized Hydrophilic Coating Polymers Found in Left Ventricular Assist Device Apical Core Specimen.

Left ventricular assist devices are increasingly used in patients with advanced heart failure. Gross and histologic evaluation of myocardial apical core specimens, extracted during device placement, can provide important insights. Herein, we describe a case of hydrophilic polymer embolization with associated foreign-body giant cell reaction discovered during apical core evaluation. (Level of Difficulty: Advanced.).

Management of cardiac implantable electronic devices in the presence of left ventricular assist devices.

Left ventricular assist devices (LVAD) are increasingly used in the management of patients with advanced heart failure. Many of these patients have or will be considered for a cardiac implantable electronic device (CIED) such as an implantable cardioverter-defibrillator or a cardiac resynchronization therapy device. Frequent interplay is often encountered due to the complexity of these devices and the underlying disease states. Proactive management strategies and an awareness of interactions may help reduce adverse events. Here we review the current literature, present management recommendatio

In vitro hemodynamic characterization of HeartMate II at 6000 rpm: Implications for weaning and recovery.

OBJECTIVES: Pump-off testing to assess left ventricular recovery is not an option for continuous-flow left ventricular assist devices unless measures are taken to prevent pump regurgitation. The purpose of this bench study was to characterize the hemodynamics and pump flow of the HeartMate II (Thoratec Corp, Pleasanton, Calif) left ventricular assist device at 6000 rpm, the speed commonly used clinically to determine left ventricular recovery. METHODS: The HeartMate II device was operated in a mock loop at 3 speeds (6000, 8000, and 10,000 rpm). We acquired pressure-flow curves in each steady

Percutaneous mechanical circulatory support for acute right heart failure: A practical approach.

Acute right heart failure (RHF) represents a critical entity with significant morbidity and mortality. This review examines the role of percutaneous right ventricular assist devices (pRVADs) as a cornerstone of therapy in cases refractory to conventional management. Devices such as the Impella RP and dual-lumen cannulas provide targeted haemodynamic support, with indications in various clinical scenarios, including acute myocardial infarction, post-cardiac surgery, myocarditis, and after left ventricular assist device (LVAD) implantation. Successful implementation requires meticulous haemodyna

Cardiac CT Angiography in Congestive Heart Failure.

Cardiac CT angiography has become an important tool for the diagnosis and treatment of congestive heart failure. Differentiation of ischemic from nonischemic cardiomyopathy; evaluation of myocardial perfusion; characterization of hypertrophic cardiomyopathy, left ventricular noncompaction, and arrhythmogenic right ventricular dysplasia; and delineation of congenital heart defects and valvular abnormalities are the primary diagnostic applications. Therapeutic use includes visualization of the coronary venous anatomy for optimal implementation of cardiac resynchronization therapy and evaluation

ECMO cannulation strategy for pulmonary hypertension: an alternative approach to traditional Veno-Arterial ECMO.

Pulmonary arterial hypertension (PAH) is a severe disease characterized by a progressive increase of pulmonary pressure and resistance, leading to right heart failure. PAH is commonly diagnosed at a late stage of the disease and is associated with progressive clinical deterioration and premature death. The treatment of PAH continues to be complicated, especially when considering treatment with extracorporeal membrane oxygenation (ECMO). A case is described that involves a heart failure patient placed on a ventricular assist device as a bridge to heart transplantation with recent onset of PAH.

Development and assessment of novel assist device for cardiac cannulation.

PURPOSE: Congenital heart defects are the most common birth defects in the USA and in 25% of cases need to be treated with cardiovascular interventions. One of such interventions is the postoperative use of an extracorporeal membrane oxygenation (ECMO) machine for the treatment of cardiorespiratory failure. The process of placing the patient on the ECMO is extremely time-critical and requires the use of cardiac cannulation. For the first time, our team developed and evaluated a new quick-connect cannulation system that allows for rapid, easy, and safe ECMO cannulation in the pediatric populati

A ventricular assist device as a bridge to recovery, decision making, or transplantation in patients with advanced cardiac failure.

Despite many advances in the management of patients with heart failure, acute cardiogenic shock and progressive congestive heart failure remain serious problems with dismal prognoses. Both temporary and permanent mechanical support has been gaining wide clinical application in this patient population. Although mechanical circulatory support technology is rapidly evolving, this approach is associated with multiple issues such as the optimal duration of temporary support, ideal timing to bridge these patients to a long-term device, and selection of the right device for the right patient. The cur

Right Heart Failure Management: Focus on Mechanical Support Options.

Millions of American adults suffer from right heart failure (RHF), a condition associated with high rates of hospitalization, organ failure, and death. There is a multitude of etiologies and mechanisms that lead to RHF, often in a feedforward spiral of decline. The management of advanced cases of RHF can be particularly difficult. For patients who are refractory to the medical optimization of volume status, hemodynamic and pharmacologic support, and rhythm control, mechanical therapies may be warranted. Currently available mechanical assist devices for RHF include venoarterial extracorporeal o

Giant cell myocarditis masquerading as orbital myositis with a rapid, fulminant course necessitating mechanical support and heart transplantation.

Giant cell myocarditis (GCM), a rapidly progressive inflammation of the myocardium, is associated with fulminant heart failure, refractory ventricular arrhythmias, and conduction system abnormalities. Few case reports have noted orbital myositis as the initial clinical presentation. Our case demonstrates a unique presentation of GCM with only ocular symptoms, which unlike prior studies, rapidly progressed to heart failure, tachyarrhythmias, and conduction disease. Our case necessitated quick recognition and treatment with mechanical support making this the first known case of GCM with successf

A Decade of Pediatric Ventricular Assist Device Support in Colombia, a Middle-Income Country.

BACKGROUND: Heart transplantation is the main treatment for refractory end-stage heart failure in patients of all ages, but its use is limited by the availability of organs, especially in pediatric populations. The International Hospital of Colombia has been a pioneer and the single center with the capability in the country for providing ventricular assist device (VAD) support using the CentriMag/PediMag-Levitronix paracorporeal VAD for pediatric patients awaiting transplant, with over a decade of experience albeit with many challenges. METHODS: This retrospective study examines outcomes from

Fabrication of an Antibacterial/Anticoagulant Dual-Functional Surface for Left Ventricular Assist Devices via Mussel-Inspired Polydopamine Chemistry.

Infections and thrombosis remain unsolved problems for implanted cardiovascular devices, such as left ventricular assist devices. Hence, the development of surfaces with improved blood compatibility and antimicrobial properties is imperative to reduce complications after artificial heart implantation. In this work, we report a novel approach to fabricate multifunctional surfaces for left ventricular transplanted ventricular assist devices (LVADs) by immobilizing nitric oxide (NO) generation catalysts and heparin and reducing silver nanoparticles in situ. The general view, structure, and chemic

Study of the energy efficiency of various drive systems of an innovative implantable left ventricular assist device.

This paper presents the results of studies of the energy efficiency of an innovative implantable left ventricular assist device. One of the advantages of the proposed LVAD is in cases where end-stage left ventricular failure is accompanied by mitral valve regurgitation and pulmonary hypertension. Another advantage is minimally invasive implantation with minimal thoracic trauma, without disrupting the breathing process, which shortens the length of hospital stay. The best drive system for converting the rotary motion of the servo motor into reciprocating motion of the piston of the suction-disc

Physiologic and hematologic concerns of rotary blood pumps: what needs to be improved?

Over the past few decades, advances in ventricular assist device (VAD) technology have provided a promising therapeutic strategy to treat heart failure patients. Despite the improved performance and encouraging clinical outcomes of the new generation of VADs based on rotary blood pumps (RBPs), their physiologic and hematologic effects are controversial. Currently, clinically available RBPs run at constant speed, which results in limited control over cardiac workload and introduces blood flow with reduced pulsatility into the circulation. In this review, we first provide an update on the new ch

Histopathologic correlates of myocardial improvement in patients supported by a left ventricular assist device.

BACKGROUND: Left ventricular assist devices unload the failing heart and improve hemodynamic function and tissue architecture. In some patients improvement allows for left ventricular assist device removal. We retrospectively compared histologic features in patients who were weaned off left ventricular assist device support with those who remained on support without evidence of clinical remission. METHODS: We graded left ventricular core samples taken at implantation on a scale we designed for evaluating severity and extent of fibrosis and hypertrophy. We correlated the grades with a computer

A heart failure specialist's perspective on cardiac surgery for heart failure.

Aside from cardiac transplantation, ventricular assist devices, and the total artificial heart, cardiac surgery now also plays a major role in the overall management of the heart failure patient. For patients with heart failure, cardiac surgery has steadily moved from being a predominant rescue procedure (eg, aneursymectomy, rupture repair, transplantation) to surgical interventions that can prevent or delay the progression of cardiac dysfunction and failure; these operations now include coronary artery bypass surgery, ventricular restoration, and valvular repair/replacement.
