Spontaneous rupture of the ascending aorta is a life-threatening condition requiring immediate intervention. The rupture usually leads to sudden death as a result of hemopericardium or hemothorax. The underlying histopathological condition in the cases described so far was mostly an atheromatous plaque. Some other rare underlying conditions were also described. We report a case of cystic medial necrosis Erdheim Gsell as a reason for fatal spontaneous rupture of the ascending aorta.

Pleural sarcoidosis is no longer considered rare, as it occurs in about 3% of all cases of sarcoidosis. However, newer methods of detection may boost this level higher. High-resolution computed tomographic scanning has increased our detection of pleural nodules and pleural thickening. Other manifestations include pneumothorax, chylothorax, and hemothorax. Further studies of large numbers of patients are needed to determine the true prevalence of pleural involvement in sarcoidosis.

A 54-year-old man suffered a self-inflicted penetrating chest wound from an automated crossbow used for hunting large animals. The arrow was in the plane of the diaphragm and penetrated the left ventricle, although no hemopericardium, hemothorax, or pneumothorax were identified. Intraoperative transesophageal echocardiography showed no significant mitral regurgitation despite the proximity of the arrow to the base of a papillary muscle. This case details his subsequent successful treatment with the assistance of cardiopulmonary bypass.

Primary pleural angiosarcoma is an extremely rare tumor. We report the case of a patient who presented with recurrent massive bilateral hemothoraxes. Although thoracoscopy was performed, biopsy samples of the pleura were inconclusive. The delayed onset of skin metastases led to the diagnosis of angiosarcoma, however the patient died from pleuropulmonary progression before treatment could be started. We review the literature of primary pleuropulmonary angiosarcoma and discuss its treatment modalities.

We describe an unusual case of congenital diaphragmatic hernia in a 6-week-old boy who presented with severe respiratory distress due to torsion of the spleen which had passed up into the chest through a Bochdalek hernia and caused a left hemothorax. Scintigraphy with heat-damaged red cells was diagnostic by showing a damaged but functioning spleen above the diaphragm. Surgical repair and return of the spleen to the abdomen resulted in complete recovery.

Complications of pectus excavatum surgery include pneumothorax, pleuritis, hemothorax, pericardial effusion, displacement of bar, pericarditis and cardiac injury, etc. This is the case of a 15-year-old boy with cardiac tamponade caused by pericarditis who had taken the operation for a pectus excavatum repair one year previously. The cause was a sternal wire which was used for attachment of the bar to sternum that had fractured and migrated through the pericardium causing a pericardial injury and a pericarditis.

Penetrating wounds of the anterior chest wall are often associated with cardiac tamponade. Eighty to ninety percent of stab wounds to the heart result in acute tamponade, whereas delayed cardiac tamponade or hemothorax are rare, but hurtful for the patient. Of the cases reviewed, fifty percent of the patients who underwent sternotomy required repair of cardiac injuries. The presented case report adds further justification for early sternotomy in case of precordial penetrating chest injury in the danger zone.

Autotransfusion as a technique in managing hypovolemia has been enjoying renewed popularity. Autotransfusion from hemothorax is the most common application of the procedure; this consists of collection, anticoagulation, and retransfusion. The advantages include compatibility, no time delay required in laboratory type and crossmatch, and no transmission of disease. The use of autologous blood in managing hypovolemia in chest trauma has proven to be a practical and beneficial adjunct to traditional fluid and homologous blood therapy and should be considered by every emergency department.

Immediate postoperative thrombosis of left superior vena cava-left pulmonary artery anastomosis in a modified Fontan procedure for single ventricle and pulmonary artery stenosis is described. Before thrombolytic therapy with streptokinase is initiated, confirmation of thrombosis by venography is mandatory to lessen the risk of cardiac tamponade or hemothorax. By this technique major surgical intervention is avoided, but close attention to the dose of streptokinase and the coagulation profile is essential. Measures to be taken if bleeding occurs with streptokinase therapy are described.

We have described a case of malignant schwannoma (MS) originating from the posterior mediastinum with subsequent pathologic rupture of the thoracic aorta. The patient presented with acute retrosternal pain radiating to the back, and she died on the day of admission. Necropsy revealed a large hemothorax and a tumor mass involving the thoracic aorta. The histologic features of the lesion fulfilled the criteria for delineated MS. We found no similar cases of thoracic MS with consequent aorta rupture.

PURPOSE: There is still debate over the safest route for the placement of long-term central venous access devices. The aim of this study was to review a large, single-institution experience to determine the impact of access location on peri-operative complications.
METHODS: The records of patients undergoing subcutaneous port (SQP) and tunneled catheter insertion over a seven-year period were reviewed. Vein cannulated (subclavian (SCV) versus internal jugular (IJ) vein), and 30-day complications were assessed. Surgical complications included pneumothorax, hemothorax, infections, arrhythmia or…

BACKGROUND: In both urban and military settings, penetrating thoracic injuries remain a significant source of trauma-related mortality, and many patients require resuscitative thoracotomy. Existing literature emphasizes relief of pericardial tamponade and aortic clamp application as the key therapeutic maneuvers. The purpose of this report is to revisit pulmonary hilar clamping and highlight its application for hemorrhage control, air embolism prevention, and other benefits in the setting of massive hemothorax.
METHODS: Records from an urban, American College of Surgeons verified level I trau…

Central venous catheterization during resuscitation of trauma patients remains controversial. Such catheterizations performed at the UMDNJ-Robert Wood Johnson Medical School at Camden Cooper Hospital/University Medical Center (Camden, NJ) trauma center for the period January 1, 1988 to December 31, 1989 were retrospectively reviewed. Patients with underlying hemothorax, pneumothorax, or resuscitative thoracotomy were excluded. There were 269 catheters inserted using the Seldinger technique. Catheters were placed via the internal jugular or subclavian veins by attending traumatologists or emerg…

Postoperative haemothorax following thoracic surgery is a rare complication, but associated with a high mortality. In the literature, this complication occurs in 0.6 to 4.6% of patients and with a mortality of 5.1 to 17.8%. In minor cases, chest tube placement, application of fresh frozen plasma (FFP) or transfusion can be sufficient to control the situation. In severe cases, re-thoracotomy is mandatory. The most frequent origin of bleeding is a bronchial or intercostal vessel (21 and 16%) or the area of pleural adhesions that were separated (10.5%). The pulmonal artery or vein were the origin

Spontaneous haemothorax is a rare event in the general population. However some diseases, including hereditary multiple exostosis, have an increased incidence of this complication. We report the case of a 22 year old patient with hereditary multiple exostosis who presented with a right sided haemothorax due to an exostosis of the seventh rib. The pathophysiology of spontaneous haemothorax in the setting of hereditary multiple exostosis is discussed together with surgical treatment and long term follow up.
