Developmental Dysplasia of the Hip.

Pediatricians are often the first to identify developmental dysplasia of the hip (DDH) and direct subsequent appropriate treatment. The general treatment principle of DDH is to obtain and maintain a concentric reduction of the femoral head in the acetabulum. Achieving this goal can range from less-invasive bracing treatments to more-invasive surgical treatment depending on the age and complexity of the dysplasia. In this review, we summarize the current trends and treatment principles in the diagnosis and treatment of DDH.

Osteonecrosis of the Hip: A Primer.

In this report, we deliver a concise and up-to-date review of osteonecrosis, a pathologic, painful, and often disabling condition that is believed to result from the temporary or permanent disruption of blood supply to an affected area of bone. We will discuss the epidemiology (disease distribution), pathogenesis (mechanism of development), etiology (associated risk factors, causes, and disorders), clinical manifestations (reported symptoms and physical findings), diagnosis and classification, and treatment options for hip osteonecrosis.

Adhesive capsulitis of the hip.

PURPOSE: Adhesive capsulitis of the hip (ACH) is not a well-defined disease entity. We evaluated the change in the clinical features of patients whose chief complaint was inability to sit cross-legged on the floor. All patients had painful limitation in hip joint motion. Based on the diagnosis of ACH, active stretching exercise (ASE) was recommended. METHODS: In February 2013 and June 2014, a total of 44 patients (50 hips), who visited outpatient clinic in 2011, were evaluated by telephone interviews. Recovery of hip motion was analyzed according to compliance with ASE and abnormalities found…

[A man with a dislocated hip].

A 95-year-old male presented with a dislocated hip hemiarthroplasty after falling off his chair. After closed reposition, symptoms of dislocation remained. A control X-ray showed a disassociation of the prosthesis at the head-neck interface. Therefore, open reposition was performed. X-ray control is important to encounter complications, which can occur after reposition.

Ultrasound of the postoperative hip.

Imaging of the postoperative hip with ultrasound can be difficult and challenging. The primary role of ultrasound is to help determine whether or not there is infection. It is difficult at times to differentiate between complex fluid and synovium. Whether fluid is infected or not cannot be determined by ultrasound criteria and aspiration, and laboratory analysis is required. Particle disease and infection cannot be reliably differentiated by ultrasound criteria. In the setting of possible infection, any fluid collection/s around the hip should be treated with suspicion and, if possible, aspira…

Developmental dysplasia of the hip.

The close interaction between community practitioners and young families provides an invaluable opportunity for prevention, education, support and even diagnosis in cases of developmental hip dysplasia. This article provides a brief overview of the condition, including clinical findings, screening protocols, 'hip safe' practices, and common treatments that may be encountered in the community.

[Developmental dysplasia of the hip].

At one month, the acetabular depth from a standard lateral coronal section is 4.6 ± 1.0mm. The correlation between experienced and junior radiologists is high (kappa 0.795). A treshhold of 6mm with a Δ (difference between right and left) less than 1.5mm provides a sensitivity of 100% for the detection of DDH at one month. Universal US screening at one month of all high-risk infants in 2009 resulted in a reduction of delayed diagnosis of DDH (zero girls, two boys) in Ille-et-Vilaine due to continuous medical education of general radiologists.

Developmental dysplasia of the hip.

In its severest form, developmental dysplasia of the hip is one of the most common congenital malformations. The pathophysiology and natural history of the range of morphological and clinical disorders that constitute developmental dysplasia of the hip are poorly understood. Neonatal screening programmes, based on clinical screening examinations, have been established for more than 40 years but their effectiveness remains controversial. Whereas systematic sonographic imaging of newborn and young infants has afforded insights into normal and abnormal hip development in early life, we do not cle…

Hip disorders in the adolescent.

This article deals with common hip problems in the adolescent age group. Some of these problems, such as slipped capital femoral epiphysis, require urgent surgical treatment. Early detection is essential. Other problems, such as many of the athletic injuries, are less urgent but important to patients who desire rapid return to full athletic capacity. The emphasis here is on understanding the conditions and diagnosis. Surgical options are mentioned but not detailed. Office management, where appropriate, is discussed against the background of the natural history of the conditions. The intended a…

Idiopathic chondrolysis of the hip.

Idiopathic chondrolysis of the hip is a rare disease in which gradual necrosis of the hyaline cartilage covering the femoral head and acetabulum leads to joint space narrowing and motion restriction. Pain, motion range limitation, and radiographic joint space narrowing are the main manifestations. We report 2 cases in 15-year-old girls, one black and the other Caucasian. Unexplained pain and motion restriction in the left hip were the presenting manifestations. Narrowing of the hip joint space was noted on standard radiographs. Magnetic resonance imaging showed a joint effusion in 1 patient an…

Developmental dysplasia of the hip.

Developmental dysplasia of the hip refers to a continuum of abnormalities in the immature hip that can range from subtle dysplasia to dislocation. The identification of risk factors, including breech presentation and family history, should heighten a physician's suspicion of developmental dysplasia of the hip. Diagnosis is made by physical examination. Palpable hip instability, unequal leg lengths, and asymmetric thigh skinfolds may be present in newborns with a hip dislocation, whereas gait abnormalities and limited hip abduction are more common in older children. The role of ultrasonography …

Fractures of the hip in children.

Fractures of the hip are uncommon in children, and their importance is related not to the frequency of the injury but to the frequency of complications. Many of these complications can be minimized or avoided by anatomic reduction and internal fixation. Open reduction frequently is necessary to obtain a stable, anatomic reduction. Regardless of the age of the child, stable fixation of the fracture must be given priority over preservation of the proximal femoral physis. The development of osteonecrosis, however, is most likely related to the severity of the initial injury and is largely unaffec…

Hamstring injuries of the hip.

Hamstring injuries can be classified with regard to the site of involvement. Traumatic disorders at the proximal bone-tendon origin are best defined as avulsion injuries, such as ischial tuberosity fractures and hamstring tendon tears. Musculotendinous lesions include muscle strains and muscle contusions. Most hamstring injuries occur after in-direct trauma from excessive stretching or forceful contraction, leading to avulsion injuries or muscle strains and tears. Insufficient warm-up, lack of flexibility, inadequate muscle strength and endurance, or abnormal contraction and running may predis

[Osteonecrosis of the hip in adults].

Osteonecrosis of the hip (ON) in the adult is still an unsolved therapeutic problem. New pathophysiological findings have increased our knowledge of the disease course. Technical progress in imaging modalities and a better understanding of pathomorphology have improved the diagnostic procedure. The most significant progress for pathophysiology, early diagnosis and therapeutic decision making has been evolved by the routine use of MR-imaging. Using the international ARCO staging system, the pathomorphological and imaging changes during the different ON stages are described in this review. A dia…

[Arthroscopy of the hip joint].

PURPOSE OF THE STUDY: Arthroscopic examination of joints has recently gained wide application. Due to hip joint shape and a difficult approach to it, hip arthroscopy has long remained outside the attention and abilities of arthroscopists. The authors present their first experience with operative hip arthroscopy that offers new options for the treatment of intra-articular pathology of the hip joint. MATERIAL: In the years 2001-2003, 24 hip arthroscopies were performed. The following pathological conditions were diagnosed and treated: loose bodies, chondral lesions of the femoral head and aceta…

[Bone diseases of the hip].

Hip pain is likely to be due to a local or regional bone disease when the hip joint is painless and of normal mobility by physical examination. Targeting the right diagnosis is easy in a patient affected by bone disease (osteoporosis, Paget's disease) or when the background of the patient is suggestive (malignancy, pregnancy). However it could be more tricky in most cases: transient osteoporosis of the hip, bone fissures, Paget's coxopathy, and tumoral cell osteolysis of bones surrounding the hip are the main causes, in addition to femoral head osteonecrosis, which is detailed in another chapt…
