Ureteral polyps: an etiological factor of hydronephrosis in children that should not be ignored.

To better define the demographics, urothelial distribution and typical gross anatomic and radiologic appearances of fibroepithelial polyps of the ureter in children. We reviewed 15 cases of fibroepithelial polyps of the ureter with hydronephrosis from the archives of our department. Data were collected from radiographic studies, gross anatomic pathology and pathology and radiology reports and categorized by age, sex, clinical presentation, lesion size and location. The mean patient age was 9.1 years, and 80% were male. All of them presented with hematuria and/or flank pain. The polyps were loc…

Hydronephrosis as a prognostic marker in bladder cancer in a cystectomy-only series.

OBJECTIVES: Hydronephrosis in patients with bladder cancer is caused by tumour at the ureteral orifice, secondary ureteral tumours, intramural or extravesical tumour infiltration, or compression of the ureter. This study investigated the prognostic impact of hydronephrosis in bladder cancer. METHODS: A series of 788 patients were treated with radical cystectomy with curative intent for transitional cell carcinoma of the bladder without neoadjuvant/adjuvant radiotherapy/chemotherapy between January 1986 and September 2003. All patients had a complete follow-up until death or until the study's …

[Prenatal diagnostic of congenital unilateral hydronephrosis with megaureter--a case presentation].

Unilateral mild hydronephrosis with ureteral dilatation is a condition that can be detected by careful prenatal ultrasound examination. We present such a case, in a 26 year pregnant woman with hereditary antecedents (mother with Ombredan disease). The fetal ultrasound at 34 weeks indicated a left anechogenic sinuous image, identified as dilated ureter, and connected to a grade 2 hydronephrotic kidney. Normal data regarding the other kidney and the bladder made us recommend a close follow-up, considering this case as with a good prognosis. After birth, reassessment was favourable, and at 1 year…

Transcriptional response in the unaffected kidney after contralateral hydronephrosis or nephrectomy.

BACKGROUND: Unilateral loss of kidney function is followed by compensatory contralateral growth. The early, genome-wide transcriptional response of the untouched kidney to unilateral ureteral obstruction (UUO) or unilateral nephrectomy is unknown. METHODS: Twelve adult male Sprague-Dawley rats were subjected to UUO and twelve rats to unilateral nephrectomy. At time points 12, 24, and 72 hours after insult four rats each were sacrificed and the contralateral kidney harvested for genome-wide gene expression analysis, transcription factor analysis, and histomorphology. RESULTS: Microarray studi

Outcome of isolated antenatal hydronephrosis: a systematic review and meta-analysis.

Idiopathic antenatal hydronephrosis (IAHN), defined as antenatal hydronephrosis not associated with other morphologic renal tract abnormalities, is the most common abnormality detected by antenatal ultrasound. We performed a systematic review and meta-analysis to determine the outcome of IAHN. We screened three databases and bibliographies to identify English-language original peer-reviewed papers that reported serial postnatal renal ultrasonography in children with IAHN. Patients who stabilized and/or improved were extracted and pooled according to the individual grading systems used by each …

Single umbilical artery and associated hydronephrosis: a report of 2 cases.

BACKGROUND: A single umbilical artery (SUA) in a fetus may be associated with hydronephrosis. CASES: We describe 2 cases of SUA, one of which had severe hydronephrosis with ascites and was stillborn. The other case was born alive with a unilateral hydroureter and is being managed with close monitoring. CONCLUSION: Detection of SUA on antenatal ultrasound should prompt a careful search for urinary tract abnormalities, as associated severe hydronephrosis may lead to a poor outcome.

[Occurrence, diagnostics and therapeutic management of hydronephrosis in pediatric patients in Germany].

As urinary tract obstruction in children may impair renal function, the early detection and evaluation of the degree of obstruction using adequate diagnostic tools is necessary for the choice of the optimal therapeutic procedure. This study describes diagnostic and therapeutic standards in relation to the quality of management of pediatric hydronephrosis in Germany in the first 6 months of the year 2000. In our study 407 of 711 (57.2%) children with a hydronephrotic condition were detected by routine ultrasound. This, and the fact that 25% of the patients, who were prenatally detected, had a d…

The association of isolated mild fetal hydronephrosis with postnatal vesicoureteral reflux.

OBJECTIVE: We evaluated the incidence of vesicoureteral reflux in fetuses with prenatally detected isolated mild fetal hydronephrosis. METHODS: Fetuses with isolated mild fetal hydronephrosis (defined as a fetal renal pelvis anteroposterior diameter of > or = 4 and < 10 mm before 24 weeks' gestational age) were prospectively evaluated with postnatal renal ultrasound and voiding cystourethrography within the first few weeks after delivery. Infants were evaluated regardless of whether or not renal pelvic dilatation was seen on postnatal ultrasound examination. RESULTS: Forty cases of mild feta…

The natural history of neonatal vesicoureteral reflux associated with antenatal hydronephrosis.

PURPOSE: In an attempt to evaluate the natural history of neonatal vesicoureteral reflux, patients with antenatal history of hydronephrosis and documented reflux in the first 30 days of life were analyzed. MATERIALS AND METHODS: Between 1993 and 1998, 260 patients with a history of antenatal hydronephrosis were referred for evaluation. Of these patients 31 were diagnosed with unilateral or bilateral refluxing renal units (54 renal units). Patients were treated with a prospective plan of observational therapy and prophylactic antibiotics, and resolution rate was calculated in patients with ade…

The value of renal scintigraphy during controlled diuresis in children with hydronephrosis.

Dynamic renal scintigraphy (DRS) during controlled diuresis is the method of choice to diagnose the functional relevance of urinary tract obstruction in children with sonographically demonstrated hydronephrosis. However, there are no commonly accepted scintigraphic criteria for surgical intervention. On the basis of our findings, we propose four stages of washout (WO) of tracer following diuresis: in stage I, WO>50%, neither further diagnosis nor intervention is necessary; in stage II, 50%>/=WO>/=12%, repetition of DRS is advised within 3-4 months; and in stage III, 12%>WO>/=5%, DRS should be …

[Evaluation of the functional state of the urinary tract in children with congenital hydronephrosis].

The study of intrarenal and upper urinary tracts urodynamics in the presence of the ureteropelvic obstruction and of the function of the lower urinary tract shows that congenital hydronephrosis was in 42.8% of cases associated with bladder dysfunction presenting with hyperreflex-type dysfunction. Maladapted hyperreflex bladder was diagnosed in 29.4%, adapted--in 13.5% of cases. The hyporeflex dysfunction occurred in 15.9% of cases. The urinary bladder dysfunction in children with congenital hydronephrosis accounts for 58.7%. These aggravate the same defects of the upper urinary tracts and dete…

Expression of subunits of the metalloendopeptidase meprin in renal cortex in experimental hydronephrosis.

Meprin A is a metalloendopeptidase in the proximal tubular epithelium of rodents that is capable of hydrolyzing a great variety of peptides and proteins. The aim of the present investigation was to investigate effects of ureteral ligation on the expression of meprin subunits. Ureteral ligation resulted in marked decreases in the expression of both alpha- and beta-meprin subunits within 12 h of ureteral obstruction. Even greater downregulation of expression of meprin alpha- and beta-mRNA was noted at 24, 48, and 96 h after ureteral ligation. The greatest decrease in meprin mRNA expression in ob

Giant ovarian leiomyoma as a rare cause of acute abdomen and hydronephrosis.

BACKGROUND: Leiomyoma of the ovary is rare, usually small, and rarely induces serious symptoms. CASE: We report a case of a leiomyoma of the ovary that weighed 11.65 kg and led to acute abdomen and bilateral hydronephrosis. To our knowledge, this complication has not been described previously. Transabdominal sonography and computed tomography scan did not identify tumor type or origin. We performed a total abdominal hysterectomy and bilateral salpingo-oophorectomy. Abdominal symptoms and defects in urinary function disappeared completely after surgery. CONCLUSION: In very few cases, benign l…

Giant hydronephrosis in adults: the great mimic. Early diagnosis with ultrasound.

Giant hydronephrosis in adults is uncommon and often misdiagnosed clinically. We present three cases of giant hydronephrosis in adults secondary to ureteric calculous obstruction, two of whom presented with an acute abdomen and one with an indirect inguinal hernia. All patients were diagnosed promptly with ultrasound.

[Neutral alpha-glucosidase activity in the urine in congenital hydronephrosis in children].

Enzymuria was studied in children who had undergone surgery for congenital hydronephrosis. A total of 109 patients were operated on according to Andersen-Hynes-Kucher with ureteral intubation and pelvic drainage. The course of renal tissue tubular function recovery upon elimination of the obstruction in the pyeloureteral segment was followed up by urine activity of neutral alpha-glucosidase as was the enzyme activity dynamics in dysplastic congenital hydronephrosis. The latter evidences reduced, but satisfactory functional capacity of the operated on organs.

Hydronephrosis in malignant tumors: rationale and efficiency of endo-urological diversions.

The improvement of minimal invasive endo-urological urinary diversions in patients with malignant ureteral obstruction (MUO) provides an alternative to open surgery. Endo-urologic procedures cause less morbidity than conventional surgical techniques. From April 1986 to April 1989, 52 patients suffering from MUO representing 64 reno-ureteral units were treated by endo-urological diversions. Proper drainage was achieved in all cases. Initial retrograde JJ-stenting was successfully performed in 30 instances. Percutaneous nephrostomy tubes were primarily placed in 34 units. Fourteen of these were
