Synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome in childhood: a report of ten cases and review of the literature.

UNLABELLED: Chronic recurrent multifocal osteomyelitis is a rare chronic inflammatory musculoskeletal process observed in children and young adults. Recently, the acronym SAPHO syndrome (for synovitis, acne, pustulosis, hyperostosis, osteitis) was coined to emphasise the association between osteo-articular inflammations and different skin abnormalities which are aseptic and filled with neutrophils. In adults, chronic recurrent multifocal osteomyelitis is now a classical manifestation of SAPHO syndrome. Chronic skin disorders were seen in eight of ten children on follow-up at the University Chi…

[Dysphagia as a symptom of diffuse idiopathic skeletal hyperostosis (Forestier-Rotes disease). A case report and literature review].

Swallowing disorders can be caused by cervical spine diseases, such as diffuse skeletal hyperostosis (DISH). This entity is diagnosed as a cause of dysphagia only after excluding all other possible causes. We report a case in which dysphagia was the initial symptom of DISH. The most important clinical and diagnostic aspects of the disease are discussed.

Case Report: Successful treatment of refractory synovitis, acne, pustulosis, hyperostosis, and osteitis syndrome and palmoplantar pustulosis with ustekinumab.

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a rare chronic inflammatory disease mainly manifested as skin and osteoarticular lesions. We describe a male patient with SAPHO syndrome who exhibited primary palmoplantar pustulosis (PPP). Notably, his condition worsened during treatment with adalimumab and other immunosuppressants. After switching to ustekinumab, the patient obtained significant improvement in both skin lesions and osteoarticular pain. These findings suggest that ustekinumab may represent an appropriate option for patients with refractory SAPHO syndr…

Delayed Paraplegia by Thoracolumbar Fracture in Diffuse Idiopathic Skeletal Hyperostosis Developed After Femur Fracture Surgery: A Case Report.

Diffuse idiopathic skeletal hyperostosis (DISH) increases with aging, as does osteoporotic fracture. DISH spine fractures are sometimes difficult to detect at the initial evaluation and result in neurological compromise. We report a case of unstable DISH spine fracture with neurological deficits developing from an asymptomatic DISH spine fracture. A 98-year-old independent female suffered from a right femoral shaft fracture due to a ground-level fall. After she started early mobilization following fracture surgery, she developed mild back pain and became paraplegic. Imaging studies showed DISH…

Synovitis, acne, pustular lesions, hyperostosis, and osteitis syndrome associated with monoclonal gammopathy of undetermined significance: a case report.

Herein, we present a case of synovitis, acne, pustular lesions, hyperostosis, and osteitis (SAPHO) syndrome in which the patient had monoclonal gammopathy. To identify whether the patient had blood proliferative disorders, we performed bone marrow aspiration and positron emission tomography-computed tomography (PET-CT) examination. Finally, we diagnosed the patient with SAPHO syndrome and monoclonal gammopathy of undetermined significance (MGUS). The patient was followed up for two years with repeat bone marrow and PET-CT examinations, and no apparent disease progression or progression to anot…

Novel genetic mutation associated with hyperphosphatemic familial tumoral calcinosis/hyperostosis-hyperphosphatemia syndrome treated with denosumab: a case report.

In this case report, a novel N-acetylgalactosaminyltransferase 3 homozygous mutation (c.782 G>A; p.R261Q) associated with hyperphosphatemic familial tumoral calcinosis/hyperostosis-hyperphosphatemia syndrome is described. The patient had elbow, pelvis, and lower limb pain and a hard mass in the hip and olecranon regions. Increased levels of inorganic phosphorus (Pi) and C-reactive protein were observed. After treating the patient with conventional drugs, we tested denosumab, which reduced but did not normalize the Pi.

Successful Treatment of Refractory Synovitis, Acne, Pustulosis, Hyperostosis, and Osteitis (SAPHO) Syndrome with Baricitinib, a Janus Kinase Inhibitor.

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome, a rare immune-mediated inflammatory disease, poses diagnostic and therapeutic challenges owing to its multi-system involvement, high heterogeneity, and lack of specific laboratory tests. Additionally, lacking evidence-based treatment recommendations, with the primary approach focusing on symptomatic relief. Herein, we report the case of a 32-year-old Chinese woman who presented with recurrent, generalized multiple osteoarticular pain lasting over one year and skin erythema pustulosis for 11 months. Traditional treatments…

LRP5 high bone mass (Worth-type autosomal dominant endosteal hyperostosis): case report and historical review of the literature.

PURPOSE: LRP5 high bone mass (HBM) is an autosomal dominant endosteal hyperostosis caused by mutations of the low-density lipoprotein receptor-related protein 5 (LRP5) gene. Alternative names included "autosomal dominant osteosclerosis" and "Worth disease." The aim of the paper is to provide an historical overview of a disorder whose literature is complicated and confusing due to the past use of several denominations and lack of reviews. METHODS: We collected case reports of HBM with evidence of autosomal dominant transmission preceding the identification of the LRP5 mutations in 2002 (Worth-…

Usefulness of dynamic stabilisation with mobile percutaneous pedicle screw for thoracic vertebral fractures in diffuse idiopathic skeletal hyperostosis.

We report a case of vertebral fracture with diffuse idiopathic skeletal hyperostosis (DISH) who underwent posterior dynamic stabilisation using mobile percutaneous pedicle screws (PPS) with 1 above-1 below and obtained good bone fusion. A 76-year-old man experienced severe low back pain after he fell backward 1 m off a stepladder during work. A 12th thoracic vertebral fracture with DISH was observed. As the fractured part was unstable due to a three-column injury, and the conservative treatment of resting was not successful, posterior dynamic stabilisation with a mobile PPS between T11-L1 was …

Association between diffuse idiopathic skeletal hyperostosis and thoracic kyphosis in patients with cervical myelopathy: a retrospective observational study.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is a structural abnormality of the thoracic spine that is known to impair posture. However, the relationship between DISH and sagittal balance in the whole spine is unclear. The aims of this study were to investigate the prevalence of DISH in patients with cervical myelopathy caused by cervical ossification of the posterior longitudinal ligament (OPLL) or cervical spondylosis and to compare sagittal alignment of the spine between patients with and without DISH. METHODS: A total of 103 consecutive patients with a diagnosis of cervical…

Atypical case of diffuse idiopathic skeletal hyperostosis mimicking ankylosing spondylitis in a young woman with polycystic ovary syndrome.

Both diffuse idiopathic skeletal hyperostosis and ankylosing spondylitis present with similar clinical manifestations of restricted spinal mobility and postural abnormalities, and radiographic resemblances including axial spine involvement and enthesopathy. Nonetheless, they are two entirely different diseases. We report an unusual case of DISH in a young woman whose diagnosis was established based on radiologic features. This case report aims to highlight the under-recognised radiologic aspects of the differential diagnosis between DISH and AS in order to avoid an inaccurate diagnosis.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) and non small cell lung cancer: case presentation and review of the literature.

Diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease, is a systemic non inflammatory disease of unknown cause. It is characterized by the presence of osteophytes due to calcification and ossification of spinal ligaments and entheses. Moreover, diffuse idiopathic skeletal hyperostosis has been associated with a variety of metabolic disorders. However, to the best of our knowledge no association with non small cell lung cancer (NSCLC) has been reported so far. In the present study we report a case of a patient with NSCLC and DISH.

Spinal Epidural Hematoma Related to Vertebral Fracture in an Atypical Rigid Diffuse Idiopathic Skeletal Hyperostosis: A Case Report.

INTRODUCTION: Spinal epidural hematoma (SEH) is a rare disease that causes cord compression and neurologic deficit. Spontaneous SEH is related to minor trauma, bleeding disorders, and anticoagulant medications. Posttraumatic SEH has been associated with low-energy spine hyperextension injuries in patients with ankylosing spinal disorders such as ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis (DISH). A variant named atypical DISH-like with SEH is reported. OBJECTIVE: To describe the management, diagnosis, and treatment of an unusual SEH case in a patient causing delayed ne…

Gingival pustules and sterile diffuse sclerosing osteomyelitis as a feature of synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome.

Synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome represents the rare co-occurrence of sterile inflammatory osteoarticular disease in association with a variety of cutaneous manifestations. Oral involvement is uncommon. The etiology of SAPHO is complex and is likely the combined result of infectious, genetic, and immunologic factors. Due to diverse clinical presentations, SAPHO is difficult to diagnose. Here, we describe the case of a 74-year-old man, who had a history of SAPHO syndrome and presented with gingival pustules and sterile diffuse sclerosing osteomyelitis of the …

L1 Fracture Subluxation in Patient with Diffuse Idiopathic Skeletal Hyperostosis Diagnosed After Hip Fracture Nail: A Case Report.

CASE: An 86-year-old man underwent closed reduction and internal fixation of a comminuted intertrochanteric hip fracture with a cephalomedullary nail on a Hana table. Postoperatively, a displaced fracture subluxation of L1 was detected, resulting in complete loss of motor and sensory function below L1. Urgent posterior decompression and fusion from T10-L4 was performed. Diffuse idiopathic skeletal hyperostosis (DISH) was only diagnosed after the incident. CONCLUSION: A diagnosis of DISH, with increased risk of occult fracture and/or iatrogenic spine injury during hip fixation, may be unknown …
