BACKGROUND: Nocardia asteroides is an opportunistic infection caused by an aerobic actinomycete, which, in the immunocompromised host, can be associated with severe invasive disease with a predilection for the brain.
METHODS AND RESULTS: The authors describe a 62-year-old man with a malignant thymoma that was clinically responsive to oral prednisone. N. asteroides sepsis subsequently developed, leading to his death.
CONCLUSIONS: Nocardiosis should be considered a potential pathogen in this immunocompromised setting.

OBJECTIVE AND IMPORTANCE: Bartonella henselae infections are among the most common causes of fever and lymphadenopathies, but can lead to severe complications in immunocompromised hosts; early recognition of these infections is of paramount importance in immunocompromised patients.
CLINICAL PRESENTATION: Here we report the case of a renal transplant recipient who presented with fever, lymphadenopathies, and a splenic abscess secondary to Bartonella henselae infection, successfully treated with doxycycline.
DISCUSSION AND CONCLUSIONS: We discuss the various clinical presentations of Bartonell…

PURPOSE OF REVIEW: In the era of highly active antiretroviral therapy, immune reconstitution inflammatory syndrome has become well recognized in the HIV-infected population. However, little is known about its occurrence in non-HIV immunocompromised hosts. The present review aims to propose the pathogenesis of immune reconstitution inflammatory syndrome, summarize its occurrence in immunocompromised patients without HIV infection, and suggest potential treatment options.
RECENT FINDINGS: Immune reconstitution inflammatory syndrome is exuberant and dysregulated inflammatory responses to invadin…

Cryptococcosis is a systemic infection and it may occur in immunocompromised and immunocompetent hosts. In order to better understand the clinical characteristics of patients with PC in different immune status, we retrospectively investigated the clinical, radiological, and treatment profiles of immunocompetent and immunocompromised patients with PC during a 10-year period (2008-2017). As a result, out of 136 patients, 94 (69.1%) were immunocompromised hosts. For the PC patients without CNS involvement, higher percentage of immunocompetent patients (39.5%, 15/38) had asymptomatic presentation …

Pneumocystosis is a common opportunistic infection in immunocompromised patients, especially in AIDS patients. The diagnosis of this pneumonia has presented several difficulties due to the low sensitivity of conventional staining methods and the absence of culture system for Pneumocystis. The molecular biology techniques, especially the PCR, have improved the detection of DNA of this fungus in invasive and noninvasive samples, and in the environment which highlighted human transmission and the existence of environmental source of Pneumocystis. In addition, various molecular biology techniques

Mycetoma is a chronic granulomatous infection that is difficult to treat, notably when due to fungi such as Scedosporium apiospermum. Recent antifungal agents could be an option, but cases are rarely reported, and none with posaconazole. Paradoxical responses, defined as initial clinical worsening despite appropriate treatment, are common in tuberculosis but rare in deep mycoses in non-immunocompromised hosts. Hence, paradoxical responses in context other than mycobacterial infection in an immunocompromised host could provide insights into the pathophysiology and the optimal strategy for treat…

Disseminated fusariosis is a life-threatening, invasive, opportunistic infection in immunocompromised patients, especially those with haematological malignancies. The prognosis is poor because these fungi are resistant to many of the available antifungal agents. We present a case of disseminated fusariosis caused by Fusarium proliferatum in a patient with severe aplastic anaemia complicated by a secondary infection of Aspergillus flavus, with a fatal outcome. We also review the documented Fusarium infections in immunocompromised hosts.

An immunocompromised patient, 6 weeks after bone marrow transplantation, developed extensive skin infection due to Trichophyton rubrum. Because she could not tolerate ketoconazole and was hypersensitive to griseofulvin, she received itraconazole with complete recovery within 30 days. This case demonstrates that itraconazole may be effective treatment for dermatophytosis, even in immunocompromised hosts, and that there is no cross-reactivity between ketoconazole and itraconazole.

Members of Cryptococcus gattii/neoformans species complex are the etiological agents of the potentially fatal human fungal infection cryptococcosis. C. gattii and its sister species cause disease in both immunocompetent and immunocompromised hosts, while the closely related species C. neoformans and C. deneoformans predominantly infect immunocompromised hosts. To date, most studies have focused on similarities in pathogenesis between these two groups, but over recent years, important differences have become apparent. In this review paper, we highlight some of the major phenotypic differences b…

Bartonella henselae infection can cause a wide spectrum of diseases in both the immunocompetent and immunocompromised host with BA a severe form relegated to immunocompromised hosts, including solid organ transplant population. There are established criteria for diagnosis of Bartonella infection based on clinical presentation, serologic testing, imaging studies and, when indicated, tissue sampling for histopathological evaluation, particularly for BA. However, treatment recommendations for BA are inconclusive. Furthermore, there are no studies in the pediatric solid organ transplant population…

BACKGROUND: Invasive pulmonary aspergillosis usually occurs in immunocompromised patients. Mild abnormality of host defence is usually present in the chronic necrotising form of the disease. Acute aspergillus pneumonia usually affects patients who are seriously immunocompromised.
OBJECTIVES: The purpose of the study was to highlight the possibility of occurrence of invasive pulmonary aspergillosis also in patients with mild abnormality of host defence.
METHODS: In a retrospective study 6 patients were analysed. The inclusion criterion was evidence of Aspergillus sp. invasion in lung tissue.

Invasive aspergillosis (IA) is one of the most common fungal infections among immunocompromised individuals. Despite the availability of antifungal drugs, IA can cause >50% mortality in infected immunocompromised patients. It is crucial to determine both host and pathogen factors that contribute to infection susceptibility and low survival rates in infected patients in order to develop novel therapeutics. Innate immune responses play a pivotal role in recognition and clearance of Aspergillus spores, though little is known about the exact cellular and molecular mechanisms. Reliable models are r

AIM: To summarize the evidence on the association of oral candidiasis with solid organ transplantation in case-control and cohort studies.
METHODS: The quality of studies was assessed using the Newcastle-Ottawa scale. Random-effects restricted maximum likelihood model or Fixed-effects Mantel-Haenszel models were used to pool the results appropriately, and sensitivity analysis, subgroup analysis, and publication bias were assessed. The certainty of evidence was assessed using the GRADE approach.
RESULTS: The pooled OR of candidiasis in immunosuppressed individuals receiving solid organ transp

PURPOSE OF REVIEW: This review examines the critical knowledge gaps limiting optimal management of invasive fungal diseases (IFDs) in immunocompromised paediatric patients and proposes a research agenda to address these deficiencies.
RECENT FINDINGS: Novel immunomodulatory therapies, including chimeric antigen receptor T-cell therapy, bispecific antibodies, and immune checkpoint inhibitors, are creating new at-risk paediatric populations whose susceptibility to IFDs remains poorly characterized. IFDs in these populations may present differently and beneficial effects from antifungal prophylax

Transplant medications are an indispensable component of treatment for patients with autoimmune diseases, malignancy, or solid organ transplants. These immunosuppressive agents, although life preserving, create unique challenges when patients require orthopaedic surgery. With increased survival rates, immunosuppressed transplant patients frequently require orthopaedic intervention, with approximately 5% developing osteonecrosis, 15% to 20% experiencing osteoporotic fractures, and many developing degenerative joint disease necessitating arthroplasty or reconstructive procedures. By inhibiting i

INTRODUCTION: Cryptococcal meningoencephalitis (CM) is the leading cause of non-viral meningitis in the United States, reflecting both the rise in immunocompromised populations and the prevention of bacterial meningitis through vaccination. Formerly linked primarily to HIV-associated immunodeficiency, effective antiretroviral therapy has reshaped CM epidemiology: solid-organ transplant recipients and apparently healthy individuals now constitute the main risk groups, with mortality remaining 30-40%.

The proportion of the population with immunocompromising conditions, who are at increased risk for complications from infectious diseases, continues to grow. Concurrently, outbreaks due to known and emerging pathogens are increasing. Vaccines are the foundation of infection prevention; however, attenuated immune responses in people who are immunocompromised necessitates innovation in design and delivery strategies. Passive immunisation, whereby the desired immunity is directly transferred to an individual, albeit transiently, could be valuable for patients who are unable to generate robust imm

We present the case of a 42-year-old woman with a history of renal transplantation who developed severe enteritis while taking enteric-coated low-dose aspirin. Despite signs of localized peritonitis, conservative management-including aspirin discontinuation, fasting, and antibiotics-was effective, and surgical intervention was avoided. Endoscopic and histologic findings were consistent with drug-induced enteritis, and no infectious etiology was identified. This case highlights the potential for serious gastrointestinal injury from low-dose aspirin in immunocompromised patients and underscores

Fungal infections have global distribution, and their prevalence depends upon the fungal species and the geographic location of the host. They may be superficial, requiring only topical treatment, or systemic, requiring hospitalization and treatment with parenteral antifungals, with complications being more common in immunocompromised individuals. Primary care clinicians should be knowledgeable and prepared to appropriately diagnose and treat these infections.
