[Influence of smoking during pregnancy on children's health--overview of epidemiologic studies].

The association between maternal smoking and retarded maternal condition and birth outcome is well know. Smoking during pregnancy increases risk of spontaneous abortion, placenta previa, abruptio placenta, preterm premature rupture of membranes, stillbirth, preterm delivery and sudden infant death syndrome. The recently conducted studies also indicate that prenatal exposure to tobacco smoke is a risk factor for respiratory infections, asthma, allergy, childhood cancer, and it has neurobehavioral consequences regarding children's health. The risk for most of these conditions has been found to i

The transformation of child health in the United States.

Social trends and medical progress have fueled major changes in the epidemiology of child health in the United States. Injuries remain a major contributor to childhood illness and death. However, among noninjury causes, chronic illness now accounts for the majority of children's hospital days and deaths. Although mortality rates for all children have fallen dramatically, social disparities persist. Approximately half of all excess deaths among African American children occur during infancy, primarily from extremely premature births, and the remaining portion, primarily from homicide and seriou

The Australian Nurse-Family Partnership Program for aboriginal mothers and babies: Describing client complexity and implications for program delivery.

CONTEXT: The Australian Nurse-Family Partnership Program is a home visiting program for Aboriginal mothers and infants (pregnancy to child's second birthday) adapted from the US Nurse Family Partnership program. It aims to improve outcomes for Australian Aboriginal mothers and babies, and disrupt intergenerational cycles of poor health and social and economic disadvantage. The aim of this study was to describe the complexity of Program clients in the Central Australian family partnership program, understand how client complexity affects program delivery and the implications for desirable progr

Breastfeeding and diabetes: part 1.

Diabetes is an increasingly common disease (Narayan et al 2003; Cowie et al 2006). The financial impact of managing this condition is rising (Hogan et al 2003). Diabetes is a syndrome with genetic and environmental influences that results in both insulin resistance and insufficient insulin production (Weyer et al 1999). Increasing evidence suggests that exclusive breastfeeding may have a significant impact on the prevention of diabetes in adult life (Young et al 2002). This paper looks at why it is important to motivate mothers with diabetes to exclusively breastfeed their babies as directed b

Child health and mortality.

Bangladesh is currently one of the very few countries in the world, which is on target for achieving the Millennium Development Goal (MDG) 4 relating to child mortality. There have been very rapid reductions in mortality, especially in recent years and among children aged over one month. However, this rate of reduction may be difficult to sustain and may impede the achievement of MDG 4. Neonatal deaths now contribute substantially (57%) to overall mortality of children aged less than five years, and reductions in neonatal mortality are difficult to achieve and have been slow in Bangladesh. The

Hypoplasia of the small pulmonary arteries in total anomalous pulmonary venous connection with obstructed pulmonary venous drainage.

OBJECTIVE: Preoperative pulmonary venous obstruction has been reported to be a risk factor negatively impacting survival in total anomalous pulmonary venous connection. We examined lung tissue from total anomalous pulmonary venous connection patients with pulmonary venous obstruction and demonstrated hypoplasia of small pulmonary arteries to elucidate the mechanism underlying the poor outcome. METHODS: Ten total anomalous pulmonary venous connection patients with preoperative pulmonary venous obstruction between the ages of 2 days and 10 months were studied. As histological parameters, we ass

Myocardial contractility in premature neonates with and without patent ductus arteriosus.

Controversy exists as to whether a hemodynamically significant left-to-right shunt due to a patent ductus arteriosus (PDA) affects ventricular contractility. Load-dependent indices such as ejection fraction and shortening fraction have traditionally been used to assess contractility, but the relationship between the rate-corrected velocity of fiber shortening (MVCFc) and wall stress may be more suitable, as it is a preload-independent, afterload-adjusted method of assessing ventricular contractility. Age-related differences have been established for these variables in normal adults and childre

Promoting children's wellbeing: Professionals' experiences and views on early identification.

AIM: This study aimed to investigate how professionals working with infants and preschoolers perceive the concept of early identification of children in need of support due to developmental, behavioral, or emotional concerns, which children should be identified and offered support or intervention, and why this should be done. Concerns can typically be associated to risk factors such as, parental mental health problems, relational challenges, or child abuse and neglect. The study was performed in municipalities implementing models inspired by the Getting It Right for Every Child (GIRFEC) framew

The Unequal Consequences of Mass Incarceration for Children.

A growing literature has documented the mostly deleterious intergenerational consequences of paternal incarceration, but less research has considered heterogeneity in these relationships. In this article, I use data from the Fragile Families and Child Wellbeing Study (N = 3,065) to estimate the heterogeneous relationship between paternal incarceration and children's problem behaviors (internalizing behaviors, externalizing behaviors, and early juvenile delinquency) and cognitive skills (reading comprehension, math comprehension, and verbal ability) in middle childhood. Taking into account chil

[Interesting cases in clinical practice concerning Kawasaki syndrome and child abuse].

Typical cases and families are reported to inform the reader about two new or newly recognized conditions of infants and young children, Kawasaki syndrome and child abuse. Although such patients have usually to be cared for in hospitals, it is the responsibility of the general practitioner to suspect the diagnosis in a very early phase and to refer the child immediately. Kawasaki syndrome, also called mucocutaneous lymph node syndrome, is an acute disease of infants and young children with yet unknown cause. The main symptoms are high fever, conjunctivitis, red and dry lips, palmar and plantar

Tail Docking of Canine Puppies: Reassessment of the Tail's Role in Communication, the Acute Pain Caused by Docking and Interpretation of Behavioural Responses.

Laws, regulations and professional standards increasingly aim to ban or restrict non-therapeutic tail docking in canine puppies. These constraints have usually been justified by reference to loss of tail participation in communication between dogs, the acute pain presumed to be caused during docking itself, subsequent experiences of chronic pain and heightened pain sensitivity, and the occurrence of other complications. These areas are reconsidered here. First, a scientifically robust examination of the dynamic functional foundations, sensory components and key features of body language that a

Parental Incarceration and Child Overweight: Results From a Sample of Disadvantaged Children in the United States.

OBJECTIVES: Rates of childhood obesity and parental incarceration have been increasing in the United States since the 1970s. We examined whether parental incarceration was associated with child overweight at age 9 and whether that association differed by which parent was incarcerated. METHODS: We ran cross-sectional logistic regression models predicting the likelihood of a child being overweight, conditional on whether the child's mother, father, or both parents had ever been incarcerated. Our sample included non-Hispanic black (n = 1638) and non-Hispanic white (n = 504) children who were sur

Poverty, Homelessness, and Family Break-Up.

This study examines the extent and correlates of family separations in families experiencing homelessness. Of 2,307 parents recruited in family shelters across 12 sites, a tenth were separated from partners and a quarter from one or more children. Additional separations before and after shelter entry and reasons, from parents' perspectives, were documented in qualitative interviews with a subsample of 80 parents. Separations were associated with economic hardship, shelter conditions, and family characteristics.

Children's environmental health: intergenerational equity in action--a civil society perspective.

Since World War II, approximately 80,000 new commercial synthetic chemicals have been released into the environment, with approximately 1500 new chemicals released annually. Most of these have not been adequately tested for their impacts on human health or their particular impacts on children and the developing fetus. Yet, children are exposed to hazardous chemicals through residues in their food, indoor and outdoor air pollution, and through household products and contaminated house dust. Many of these synthetic chemicals are persistent and bio-accumulative, remaining in the human body long a

Clinical significance of placental examination in perinatal medicine.

Complete pathologic evaluation of the placenta provides valuable information for perinatal care for the obstetrician, neonatologist, pediatrician, and family. The histopathology of the placenta can answer specific questions about in utero insults, give insight into management of subsequent pregnancies, and provide an assessment of the newborn risk. Placental pathology has been a key litigious informant in inferring timing of insults. Despite these well known advantages of placental pathologic examination, it remains an under-utilized part of perinatal medicine. This stems from a historically u
