The article deals with 8 cases of iatrogenic obstruction of the vas deferens after herniotomy for inguinal hernia, the consequence of which was sterility. Anastomosis of the vas deferens was formed in 6 patients by means of precision microsurgical manipulations; the spermatological indices improved markedly in 3 of them, which was evidence of restored passage of the semen. To avoid injury to the spermatic cord, the author recommends microsurgical techniques in treatment of the hernial sac in children with congenital inguinal hernia.

The chromosome makeup of a 3-year-old crossbred heifer was examined in an attempt to explore the cytogenetic causes of her infertility. The heifer proved to be a mosaic carrying predominantly XY cells and with approximately 30% of cells lacking a normal Y. We studied the importance of a history of appendectomy for appendicitis in 279 women with laparoscopically or surgically diagnosed tubal infertility and a control group of 957 fertile women. Increasing numbers of patients are seeking treatment for infertility. The primary care physician is often the first professional to face this diagnostic challenge. Recent developments in the diagnosis and treatment of the infertile man have led to increased success in management of the infertile couple. This article reviews these developments.

Fifty men with oligospermia or reduced sperm motility were tested with scrotal cooling by ice packs applied to the scrotum at night and held in place with jockey shorts. Sperm density and motility were compared before and after treatment. At least a 2-fold increase in sperm density and comparable increases in sperm motility were seen in 65 per cent of the patients in this series. The percentage of successful treatment was the same whether or not a varicocele was present. These results further support the detrimental influence of heat on spermatogenesis.

Microsurgery has become an important tool in the management of the infertile male. Although macrosurgical techniques are still required for preservation and protection of cryptorchid testes, microscopic techniques are also useful in the preservation of testicular tissue. We now use the microscope in all vasovasostomy and vasoepididyostomy procedures, but its final role in the treatment of male infertility has yet to be determined.

Of 17 patients with longstanding (3--15 years, mean 7.7 years) amenorrhea and hyperprolactinemia, 8 developed their amenorrhea after the use of oral contraceptives (Group I) and 9 became amenorrhoic spontaneously (Group II). One hundred nineteen Jamaican women who underwent myomectomy from 1964 to 1969 were followed up for 5 to 9 years. Infertility was the most common presenting complaint. Preoperative hysterosalpingograms were performed on 61 patients and were abnormal for the majority of these patients. Surgery revealed submucous fibroids in 41 patients and pelvic inflammatory disease in 51 patients.
