Reclassifying cT4b buccal mucosa/gingivobuccal complex cancers: do we need to change?

T4b carcinomas are termed as very locally advanced carcinomas of the oral cavity and are deemed borderline resectable or unresectable. The role of surgery for these patients is not well defined. We therefore aimed to relook at the role of surgery for cT4b carcinoma of the oral cavity. We evaluated 596 patients with cT4 oral cancers. A total of 218 patients were staged as cT4b based on clinicoradiological findings. These patients underwent bite composite resection either before or after neoadjuvant chemotherapy. Additional compartmental infratemporal fossa (ITF) clearance was done in patients w

Solitary fibrous tumor of the masticator space - Report of a rare case.

The solitary fibrous tumor (SFT) is a potentially malignant spindle cell neoplasm of the mesenchymal origin that was originally described as a thoracic lesion originating from the pleural tissue. Recently, numerous extrapleural sites of origin have been described, also affecting the head and neck region. SFTs are benign in most cases, but 10%-15% of extrapleural SFTs show malignant behavior in the form of recurrent or metastatic disease. We present the case of a 25-year-old female who presented with an asymptomatic left-sided facial swelling of over three years. She had a diffuse swelling in t

Transinfratemporal Fossa Transposition of the Temporalis Muscle Flap for Skull Base Reconstruction after Endoscopic Expanded Nasopharyngectomy: Anatomical Study and Clinical Application.

Objective  Temporalis muscle flap (TMF) is widely used in traditional skull base surgery, but its application in endoscopic skull base surgery remains rarely reported. We aimed to investigate the surgical anatomy and clinical application of TMF for reconstruction of skull base defects after expanded endoscopic nasopharyngectomy. Methods  Nine fresh cadaver heads (18 sides) were used for endoscopic dissection at the University of Pittsburgh School of Medicine in the United States. TMF was harvested using a traditional open approach and then transposed into the maxillary sinus and nasal cavity t

Orbital pseudotumor presenting as a temporomandibular disorder: A case report and review of literature.

BACKGROUND AND OVERVIEW: Orofacial pain has multifactorial causes and is often a diagnostic challenge. Misdiagnosis can result in morbidity or mortality due to misdirected and inappropriate treatment. A delay of necessary treatment, in cases of ominous illnesses, may result in its perpetuation or progression. The authors present a case report that illustrates these possibilities. CASE DESCRIPTION: This case report describes a 36-year-old woman with the chief symptom of painless, limited mandibular movement. She also reported restricted movement of the left eye. She was previously diagnosed an

Imaging of Unilateral Meningo-ophthalmic Artery Anomaly in a Patient with Bilateral Nasopharyngeal Angiofibroma.

A 12-year-old boy with epistaxis presented with a rare midline nasopharyngeal angiofibroma that extended lateral into the pterygoid and infratemporal fossae. Pre-operative angiography revealed bilateral prominent feeder arteries and two major anastomotic connections, and a rare left meningo-ophthalmic artery (M-OA) anomaly that was the sole path of supply to the eye. A literature search using Pubmed and Medline was conducted. For imaging, a six-vessel study (i.e. external and internal carotid and vertebral arteries on both sides) was selected. Embolization of prominent tumor feeder arteries wa

Endoscopic sinus surgery for benign and malignant nasal and sinus neoplasm.

PURPOSE OF REVIEW: Endoscopic sinus surgery has become widely accepted as the standard of treatment for chronic inflammatory diseases of the paranasal sinuses unresponsive to medical treatment. With increased skill with endoscopic surgical technique, advanced technologies such as intraoperative imaging systems, and a better understanding of the complex anatomy of the paranasal sinuses and surrounding vital structures, many otolaryngologists have increasingly applied their expertise in endoscopic sinus surgery to the resection of nasal and sinus neoplasms. The following represents a review of t

Endoscopic Assisted Transoral Approach for Maxillofacial Tumor Resection-A Retrospective Study.

OBJECTIVE: Surgical resection of maxillofacial tumors requires balancing radical resection with functional and esthetic preservation. Endoscopic-assisted and transoral techniques offer minimally invasive alternatives, but each has limitations. This retrospective study evaluates the endoscopic-assisted transoral approach (EATA) for maxillofacial tumor resection, analyzing its technical characteristics, perioperative outcomes, and safety. METHODS: Patients who underwent tumor resection via EATA at West China Hospital of Stomatology, Sichuan University, between April 2024 and September 2025 were

Challenges in Diagnosing the Course of the Lingual Nerve for Clinical Practice and Research.

The accurate identification and protection of the lingual nerve during oral surgery are critical to avoid complications such as a loss of taste or sensation and chronic pain. While numerous studies have described the nerve's anatomy and injury outcomes, no consensus exists on the optimal method to trace its full course. This narrative review systematically examined the literature from 2010 to 2024, using databases like PubMed, MEDLINE, Embase, and Google Scholar. Keywords included "Lingual nerve," "Course," "Anatomy," and "Clinical implications," combined with Boolean operators. Studies were s

The combined compartmental maxillary resection: technique codification and preliminary multicenter series analysis.

PURPOSE: Maxillary cancers extending to the deep craniofacial spaces represent one of the most challenging scenarios in head and neck oncology. Integrating compartmental surgery principles with combined endoscopic-open approaches may optimize deep-margin control, a key determinant of oncologic radicality. METHODS: A retrospective analysis included 34 patients treated between 2019 and 2025 at four referral centers using combined compartmental maxillary resection (CCMR). Procedures were assigned to CCMR types 1-3 according to the compartmental resection performed; preoperative imaging, includin

Head and neck manifestations of 22q11.2 deletion syndromes.

The allelic loss of 22q11.2 results in various developmental failures of pharyngeal pouch derivatives ("22q11.2 deletion syndromes", 22q.11DS), consequently affecting the anatomy and physiology of head and neck (H&N) organs. The objective of this paper was to describe those manifestations. Two 22q11.2DS patients with H&N manifestations were studied along with a comprehensive review of the English literature, from 1975 to 2010 regarding the associated H&N malformations among 22q11.2DS. A 24-year-old mentally disabled 22q11.2DS male presented with right hemithyroid enlargement, causing significa

Spatial correlation of the buccal and inferior alveolar nerves: Morphological evidence from cadaveric dissections.

OBJECTIVE: This study aimed to examine the spatial relationships between the buccal nerve (BN) and the inferior alveolar nerve (IAN) with respect to superficial facial landmarks and to evaluate the clinical relevance of their topographic correlation. A clearer understanding of these relationships may help prevent iatrogenic injury during maxillofacial procedures and regional anesthesia. METHODS: The study was conducted on 15 adult cadavers (30 hemispheres). Each specimen was carefully dissected to expose the BN and IAN within the infratemporal fossa. The tragus (TRA), lateral canthus (LC), an

Minimally invasive ictal EEG source localization including sphenoidal electrodes - a retrospective study and a meta-analysis.

BACKGROUND: In patients with pharmacoresistant temporal lobe epilepsy, minimally invasive sphenoidal electrodes can enhance presurgical video-EEG monitoring. Placed into the infratemporal fossa, these electrodes record EEG activity from the mesial temporal lobe. We hypothesized that inclusion of the signal from sphenoidal electrodes would improve the diagnostic sensitivity of ictal EEG source localization (ESL). METHODS: We retrospectively included patients with pharmacoresistant focal epilepsy, presurgical low-density EEG including bilateral sphenoidal electrodes, subsequent resective surger
