Transforaminal Epidural Steroid Injection for Zoster-Related Pain: The Golden Period for the Best Outcome.

BACKGROUND: Zoster-related pain (ZRP) has many negative effects on a patient's quality of life. The transforaminal steroid injection (TFESI), which reduces neural inflammation and pain, has been advocated by pain physicians. Many reports demonstrated that early administration of TFESI showed better efficacy; however, the golden period during which TFESI is most effective remains unclear. OBJECTIVES: This multicentre retrospective cohort study aimed to identify the golden period by which TFESI yields the best outcome in patients with ZRP. STUDY DESIGN: Multicenter, retrospective cohort study.

Pneumocephalus and Chemical Meningitis after Inadvertent Dural Puncture during Lumbar Epidural Injection.

Inadvertent dural puncture (IDP) is one of the complications of lumbar epidural steroid injections (ESIs). We report a case in which pneumocephalus and chemical meningitis developed at the same time after an IDP during a lumbar interlaminar ESI. A 60-year-old woman presented to the emergency room with thunderclap headache and febrile sensation 3 hours after receiving a lumbar interlaminar ESI. Brain computed tomography (CT) scan showed multiple small foci of air within the subarachnoid space and ventricle. After the admission, the patient was afebrile and reported mild improvement of headache

Needle-Through-Needle Technique in Lumbar Interlaminar Epidural Steroid Injection: A Case Report.

When performing lumbar epidural steroid injection on obese patients, needle placement can be challenging due to the difficulty in estimating the appropriate needle length to utilize. Often times, the standard 3.5-inch Tuohy needle is too short to reach its target. In our case report, a needle-through-needle technique was attempted in a lumbar interlaminar epidural steroid injection procedure after the initial needle fell short of the epidural space. To avoid removing the initial needle and restarting the procedure using a longer needle, a 20-gauge 6-inch Tuohy needle was inserted into the 17-g

Predictability of the effects of epidural injection in lumbar spinal stenosis by assessment of lumbar MRI scans.

BACKGROUND: Numerous classification systems have been proposed to interpret lumbar MRI scans. The clinical impact of the measured parameters remains unclear. To evaluate the clinical significance of imaging results in patients with multisegmental degenerative pathologies, treating specialists can perform image-guided local injections to target defined areas such as the epidural space. OBJECTIVE: The aim of this retrospective study was to evaluate the correlation between lumbar spinal stenosis measurements obtained by MRI and improvement obtained through local epidural injection. METHODS: In

Hypothalamic Pituitary Adrenocortical Axis Suppression following a Single Epidural Injection of Methylprednisolone Acetate.

BACKGROUND: Epidural injections (EIs) are the most commonly performed minimally invasive intervention in managing chronic low back pain (CLBP). There is inconsistency in data to accurately predict the degree of hypothalamic-piuitary-adrenal (HPA) axis suppression in patients receiving exogenous steroid therapy, especially in the form of epidural steroid injections (ESIs). OBJECTIVE: We aim to quantify the degree and duration of HPA axis suppression after a single ESI of 80 mg methyl prednisolone acetate in patients with CLBP. STUDY DESIGN: A single open-label prospective study. SETTING: An

Post Procedural Complication following Cervical Epidural Local Anesthetic Injection: A Case Report.

INTRODUCTION: Cervical radiculopathy is a common condition affecting many people each year. The efficacy of cervical epidural steroid injection for patients that have not responded to conservative treatment has been demonstrated. Even with confirmatory radiocontrast dispersion indicating correct presence in the epidural space, there still may be rostral spread of steroid and local anesthetic resulting in an unusual presentation of symptoms and potentially life threatening complications. CASE PRESENTATION: We present the case of a 52-year-old male presenting for a right sided C6-C7 epidural st

Clinical efficacy of transforaminal epidural injection for management of zoster-associated pain: a retrospective analysis.

OBJECTIVES: Transforaminal epidural injection (TFEI) has superior accessibility to the dorsal root ganglion, which is an essential location of pain signaling in herpes zoster. However, the effectiveness of TFEI for herpes zoster patients has not previously been studied. We retrospectively analyzed the efficacy of TFEI for pain control and prevention of PHN in patients with acute and subacute herpes zoster. METHODS: Medical records of 137 patients who underwent TFEI for zoster-associated pain (ZAP) were reviewed. The participants were divided into two groups: acute TFEI group (TFEI within 30 d

Evaluation of epidurographic contrast patterns with fluoroscopic-guided lumbar interlaminar ventral epidural injection.

BACKGROUND: Targeted ventral epidural steroid injection with a transforaminal approach has been widely used for the treatment of nerve root inflammation and pain. Lumbar interlaminar approach is also commonly used; however, ventral epidural injection may not be obtained properly. Lumbar interlaminar ventral epidural (LIVE) injection can be achieved as the epidural catheter is placed at the ventrolateral side of the nerve root. The purpose of this study is to evaluate the contrast dispersal pattern with LIVE injection. METHODS: Forty patients with lower back and L5 radicular pain were studied.

Uterine bleeding in a postmenopausal women following epidural triamcinolone acetonide injection: A case report.

Epidural steroid therapy for herpes zoster associated pain is known to be effective with rare complications. However, certain amount of epidurally-injected steroid may be absorbed and affect endocrinological system. Specially, triamcinolone acetonide has high affinity for progesterone receptors and may influence mensturation. We report a case of a postmenopausal women treated with 40 mg of epidural triamcinolone acetonide twice with a two week interval and followed a large amount of uterine bleeding.
