Moderate hypothermia in neonatal encephalopathy: safety outcomes.

Hypoxic-ischemic injury may cause multisystem organ damage with significant aberrations in clotting, renal, and cardiac functions. Systemic hypothermia may aggravate these medical conditions, such as bradycardia and increased clotting times, and very little safety data in neonatal hypoxic-ischemic injury is available. This study reports a multicenter, randomized, controlled pilot trial of moderate systemic hypothermia (33 degrees C) vs normothermia (37 degrees C) for 48 hours in infants with neonatal encephalopathy instituted within 6 hours of birth or hypoxic-ischemic event. The best outcome

Neonatal and maternal outcomes of scheduled versus unscheduled ex-utero intrapartum treatment (EXIT) procedures.

OBJECTIVE: Ex-utero intrapartum treatment (EXIT) procedures are employed during delivery to establish a safe airway on placental support for neonates with potential airway obstruction. While EXITs are typically planned with a multidisciplinary team, these procedures may occur emergently due to preterm labor, late diagnosis, or delayed transfer of care. This systematic review compares outcomes of scheduled versus unscheduled/emergent EXIT procedures. DATA SOURCES: We searched PubMed, Scopus, and CINAHL in March 2025 to identify observational studies describing EXIT procedures. METHODS: Studie

Heart team approach for comprehensive management of aortic coarctation in the adult.

BACKGROUND: Aortic coarctation in the adult is usually associated with chronic systemic hypertension, which leads to the sequelae of congestive heart failure, vascular dysfunction and decreased lifespan. Open and endovascular treatment modalities both provide excellent procedural outcomes with minimal mortality and morbidity, but a structured algorithm for workup and periprocedural decision making is not well established. We outline our heart team decision making approach along with our institution's experience treating this condition. METHODS: We retrospectively reviewed twenty-four consecut

Resectoscopic Surgery Part I: Overcoming Obstacles and Mastering the Basics.

The introduction of the continuous flow gynecologic resectoscope (CFGR) in 1989 revolutionized minimally invasive gynecologic surgery (MIGS) by introducing such intrauterine procedures as hysteroscopic myomectomy, polypectomy, and endometrial ablation. However, with the subsequent introduction of global endometrial ablation (GEA) devices and hysteroscopic morcellators (HMs), the CFGR has fallen into relative disuse-a regrettable situation since it remains ideally suited for accomplishing many procedures that are otherwise not achievable with these newer technologies. Procedures which involve g

Improving outcomes following reconstruction of pressure sores in spinal injury patients: A multidisciplinary approach.

BACKGROUND AND AIM: Pressure sore treatment in spinal injury patients is challenging. A multidisciplinary approach with joint management by the plastic surgery and spinal injury teams was initiated at our institution in 2005 to improve patient care and surgical outcomes following reconstruction. This study assessed the surgical outcomes following reconstruction using the team approach and to compare inpatient stay and readmissions for complications before and after the multidisciplinary protocol was introduced. METHODS: A retrospective review of consecutive patients in the multidisciplinary p

Evaluation of laparoscopic transperitoneal adrenalectomy: is it feasible for large masses?

AIM: The aim of this paper was to determine whether laparoscopic adrenalectomy (LA) is a safe and effective treatment for the management of large adrenal tumors. METHODS: We retrospectively evaluated the data of patients who underwent LA at our institution between September 2002 and September 2012. Seventy-six transperitoneal LA were performed by the same surgical team. Patients with invasive tumors to adjacent organs or distant metastasis were excluded from the study. All patients were operated using the 450 oblique position as transperitoneal approach. RESULTS: The mean age of the patients

Emergency Department Guidelines for Orthopedic Consultation in an Academic Trauma Center: Effect on ED Throughput and Consultation Utilization.

OBJECTIVES: Musculoskeletal injuries account for approximately 20% of presentations to emergency departments (ED) in the United States. While many patients can be treated exclusively by an emergency medicine (EM) clinician, others require consultation with an orthopedic surgeon. The objective of this quality improvement initiative was to develop and evaluate the effect of institutional orthopedic consultation guidelines on ED length of stay (LOS), the percentage of patients with fractures who receive an orthopedic consultation in the ED and orthopedic clinic referrals without an ED orthopedics

Providing medical care at mass gathering sporting events: the 2023 Ryder Cup experience.

BACKGROUND: The Ryder Cup is a men's golf competition and one of the most high-profile sporting events worldwide, presenting significant challenges for healthcare providers, especially when hosted in non-urban areas. Medical professionals must address sport-related illness and injuries in elite athletes, exacerbations of pre-existing conditions in spectators, and occupational health issues among staff and volunteers. Proper healthcare planning and infrastructure are crucial to reduce the burden on local hospitals and ensure timely care for everyone involved. Aim of the study is to describe the

Assessment of the socio-economic impacts associated with the arrival of apple snail (Pomacea canaliculata) in Mwea irrigation scheme, Kenya.

BACKGROUND: In Kenya, rice (Oryza sativa L.) is mainly produced under irrigation by small-scale farmers. Mwea irrigation scheme (MIS) in Kirinyaga County accounts for 80-88% of rice production. Here, rice is the main source of livelihood and a source of revenue generation for the county. However, a recently established invasive freshwater snail, Pomacea canaliculata (Lamarck) (family: Ampullariidae), a species of apple snail, presents a serious threat to rice production. RESULTS: Household surveys, focus group discussions and key informant interviews highlight apple snail as a serious problem

Cellular Therapy for Systemic Autoimmune Diseases: New Opportunities for Future Research and Interdisciplinary Collaboration.

Systemic autoimmune diseases (sAIDs) affect 1 in 10 people and represent a leading cause of death in women under the age of 65 years. Despite the emergence of targeted-biologic therapies, mortality rates for sAIDs have not improved. Emerging cellular therapies represent a promising therapeutic strategy for patients living with sAIDs given their potential to induce sustained remission and limit long-term exposure to glucocorticoids. Encouraging outcomes from a case series of patients with systemic lupus erythematosus, idiopathic inflammatory myopathy, and systemic sclerosis (SSc) have ignited c

Comparison of knowledge to antimicrobial stewardship institution policies targeting Staphylococcus aureus bacteremia and candidemia between medical doctors and pharmacists in an academic teaching hospital in Japan.

Staphylococcus aureus bacteremia (SAB) and candidemia have significant impacts on mortality. Both have important implications for antimicrobial stewardship programs (ASPs). However, there are limited data regarding who should be educated and what components should be considered for the ASPs. Hence, we investigated the possibility of the key elements for implications of SAB and candidemia managements for ASPs. We conducted a cross-sectional study on the knowledge of antimicrobial stewardship institution policies targeting SAB and candidemia for all medical doctors (MDs) and pharmacists to using

Coordinated multidisciplinary care for ambulatory Huntington's disease patients. Evaluation of 18 months of implementation.

BACKGROUND: A multidisciplinary outpatient department was set up in the northern part of the Netherlands because of a local lack of adequate treatment and care for Huntington's disease (HD)patients. Outreaching multidisciplinary care is a novel way to optimise functioning and quality of life of HD patients. The vast majority of patients want to stay home as long as possible. Huntington's disease is a devastating neurodegenerative disorder leading to complete disability and long term residence in a specialised institution. In this paper we outline this new type of treatment and give the results
