Term infants with hypoxic ischaemic encephalopathy: poor neurodevelopmental outcome despite standard neonatal intensive care.

Perinatal asphyxia may be severe and affect the neurological system, causing hypoxic ischaemic encephalopathy (HIE). Twenty-three term infants with HIE were studied over 3.5 years. Nine died and of the 14 survivors, eight were normal and the rest had abnormal neurological sequelae. Favourable prognostic markers were a 5 minute Apgar score > 5, first pH > 7.1, and stage I HIE. Despite standard neonatal intensive care management, the mortality and morbidity rates for these infants remained high.

Rehospitalization in the first two weeks after discharge from the neonatal intensive care unit.

BACKGROUND: High-risk newborns are known to have higher than average utilization of services after discharge from the neonatal intensive care unit (NICU). Most studies on this subject report aggregate data over periods ranging from 1 to 3 years postdischarge. Little is known about events that are temporally close to NICU discharge. OBJECTIVES: To characterize rehospitalizations within the first 2 weeks after discharge from six community NICUs. METHODS: We scanned electronic databases and reviewed the charts of rehospitalized infants from six NICUs in the Kaiser Permanente Medical Care Progra

Autopsy in a neonatal intensive care unit: utilization patterns and associations of clinicopathologic discordances.

OBJECTIVES: (1) To develop methods to describe autopsy utilization patterns in a neonatal intensive care unit. (2) To identify classes of patients likely to have clinicopathologic concordance or discordance. METHODS: Five hundred forty-five consecutive neonatal intensive care unit deaths (338 autopsied, rate 62%) in a regional tertiary/quaternary care neonatal intensive care unit for referred infants (65,000 annual births) were classified in six clinical diagnostic groups (anomalies, cardiac anomalies, hypoxic ischemic encephalopathy, prematurity and its complications, infections, and other)

Sepsis at a neonatal intensive care unit: a four-year retrospective study (1989-1992).

During the 4-year period 1989-1992, 18,227 neonates were born at Kaplan Hospital and 614 (3.4%) were admitted to the neonatal intensive care unit. During this period, 120 episodes (6.6/1000 live births) of neonatal sepsis were recorded in 109 neonates (6/1000 live births). The incidence of early-onset sepsis was 19/109 (17%). The main pathogens of early-onset sepsis were S. agalactiae (42%) and E. coli (32%). Seven of the 8 S. agalactiae cases were recorded during 1989-1990. The main pathogens of late-onset sepsis were Klebsiella spp. (31%), coagulase-negative staphylococci (18%) and Candida s

[Rehospitalization of very-low-birth-weight infants after discharge from neonatal intensive care units].

The higher survival rate of very-low-birth-weight (VLBW) infants is associated with an increasing rehospitalization load, especially during the first year after discharge from neonatal intensive care units. This was evaluated in a sample of 86 VLBW infants born during 1989-1990, compared with 120 normal term newborns delivered during 1990. 34 (39.5%) VLBW infants were rehospitalized, compared with 16 (13.3%) term newborns (2.9:1; p < 0.001). The ratio of hospitalizations between the groups was even higher, 4:1. Of the 49 VLBW infants hospitalized, 25 had respiratory complications (in 15 bronch

Pneumonia in the neonatal intensive care unit. Diagnosis by quantitative bacterial tracheal aspirate cultures.

Growth of > or = 10(5) colonies of bacteria per milliliter obtained at bronchoscopy in children and adults correlates with bacterial pneumonia. To determine whether quantitative tracheal aspirate cultures aid in diagnosis of pneumonia in the neonatal intensive care unit setting, tracheal aspirates were obtained from 25 infants who had recently undergone endotracheal intubation; 15 of the infants had suspected pneumonia and 10 control infants had undergone intubation for suspected apnea of prematurity (4 infants) or elective surgery (6 infants). Studies also were performed to detect Mycoplasma,

[Incidence and prognostic significance of positive blood cultures in neonatal and pediatric intensive care].

The incidence and prognostic importance of positive blood cultures were evaluated over a two-year period (1986/87) in 1371 admissions to a multi-disciplinary neonatal and pediatric intensive care unit (ICU). Blood cultures were performed in 439 patients of which 80 cultures were positive. Septicemia was confirmed in 70 cases, the 10 remaining cases being classified as contamination or bacteremia. The incidence of nosocomial, ICU-acquired septicemia was low (7/70). The major causative organisms of sepsis were those commonly encountered in neonatal and pediatric infections, including Haemophilus

Cluster of Malassezia furfur pulmonary infections in infants in a neonatal intensive-care unit.

Between 23 and 27 July 1987, three infants at one hospital developed severe bronchopneumonia associated with respiratory failure, thrombocytopenia, and leukocytosis. Two infants died; at postmortem examination, Malassezia furfur was identified in their lung tissues. M. furfur was isolated from cultures of blood, urine, and stool samples from the infant who survived. All documented M. furfur infections occurred in infants with a birth weight of less than 1,000 g; the attack rate was 42.9% (three of seven infants). A case-control study comparing the three cases and nine infants randomly selected

Visual sensory determinants of EEG activity in neonates receiving intensive care.

INTRODUCTION: Cortical activity is crucial for neural organisation during early life, especially in vulnerable infants who have suffered neurological compromise and/or require intensive care. However, its sensory determinants are poorly understood. In hospital, acutely unwell infants sometimes wear eye shields. We used this natural experiment to test the hypothesis that reduction of external visual inputs will attenuate occipital cortical activity. METHODS: We analysed 9-channels video-EEG from 18 sleeping neonates (median gestational age 39 weeks (IQR 36-40); 14/18 had mild-moderate hypoxic-

The Soundscape of Neonatal Intensive Care: A Mixed-Methods Study of the Parents' Experience.

Parents who have infants hospitalised in neonatal intensive care units (NICUs) experience high levels of stress, including post-traumatic stress disorder (PTSD) symptoms. However, whether sounds contribute to parents' stress remains largely unknown. Critically, researchers lack a comprehensive instrument to investigate the relationship between sounds in NICUs and parental stress. To address this gap, this report presents the "Soundscape of NICU Questionnaire" (SON-Q), which was developed specifically to capture parents' perceptions and beliefs about the impact that sound had on them and their

The clinical safety and efficacy of flexible bronchoscopy in a neonatal intensive care unit.

Flexible bronchoscopy (FB), developed in the 1960s, is widely used in the clinical practice of pediatrics and has demonstrated fundamental value in clinical diagnoses and treatment. However, as an invasive procedure, the use of FB is limited due to concerns regarding the tolerance of the procedure and the possible complications in neonatal units. Thus, the present study aimed to investigate the clinical safety and efficacy of flexible bronchoscopy (FB) in a neonatal intensive care unit (NICU). Neonates (n=54) who received FB in the NICU of Shanghai Children's Hospital between January 2012 and

Challenges in the practice of human milk nutrition in the neonatal intensive care unit.

The use of human milk for preterm infants has increased over the past decade reflecting an improved awareness of the benefits of human milk. Inherent in this paradigm shift is the recognition that human milk is a living tissue; full of immune cells, probiotics and hundreds of compounds that confer bioactivity and immune protective properties. Together these factors deliver a powerful effect in reducing clinical morbidities such as necrotizing enterocolitis and sepsis in the preterm infant. However, as breastfeeding is not possible for the very premature infant, human milk needs to be introduce
