Pelvic fistulas complicating pelvic surgery or diseases: spectrum of imaging findings.

Pelvic fistulas may result from obstetric complications, inflammatory bowel disease, pelvic malignancy, pelvic radiation therapy, pelvic surgery, or other traumatic causes, and their symptoms may be distressing. In our experience, various types of pelvic fistulas are identified after pelvic disease or pelvic surgery. Because of its close proximity, the majority of such fistulas occur in the pelvic cavity and include the vesicovaginal, vesicouterine, vesicoenteric, ureterovaginal, ureteroenteric and enterovaginal type. The purpose of this article is to illustrate the spectrum of imaging feature

Laparoscopic treatment of gallstone ileus.

Gallstone ileus is a rare complication of cholelithiasis that has high morbidity and mortality. An intestinal obstruction can be caused by migration of a large gallstone through a biliary enteric fistula or by impaction within the intestinal tract. In this study, we present the case of an 81-year-old woman with a mechanical bowel obstruction by a gallstone that was treated by laparoscopy.

Textiloma, migration of retained long gauze from abdominal cavity to intestine.

Retaining of gauzes and surgical sponges in the abdomen is one of the most frequent medical errors usually manifesting as abscess or abdominocutaneus fistulas with no definite symptoms during lifetime. Here, we introduce a 35 year old woman with symptoms and signs of partial bowel obstruction and enterocutaneous fistulas caused by migration of retained gauze from abdominal cavity to terminal ileum, 9 months after cesarean section. This is called "Textiloma". There are several reports of gossypiboma worldwide but migration of retained gauze into intestine causing an enterocutaneous fistula is r

Ileourethral fistula following cystectomy: a rare complication.

Fistulae between the urinary and intestinal tracts of adult male patients usually result from trauma, inflammatory diseases, necrosis and infiltration of a neoplasm of either the bladder, prostate or colon, or radiation therapy. We report 2 patients who developed an ileourethral fistula following cystectomy which had been performed for carcinoma of the bladder in 1 patient and leukoplakia of the bladder in the other. The diagnosis of an ileourethral fistula was established by retrograde urethrography and a barium follow-through study. Computed tomography performed in 1 patient showed a tumor m

Ascorbic acid absorption in Crohn's disease. Studies using L-[carboxyl-14C]ascorbic acid.

Total body pool and intestinal absorption of ascorbic acid were studied in 12 patients undergoing operation for Crohn's disease (six with fistulae and six without) and in six control patients undergoing operation for reasons other than Crohn's disease. L-[carboxyl-14C]Ascorbic acid, 0.19-0.40 megabecquerels (MBq), was given orally. After a period of equilibration, the labeled ascorbic acid was flushed out of the patient's body tissues using large doses of unlabeled ascorbic acid. Intestinal absorption of ascorbic acid, assessed from the total cumulative urinary 14C recovery, was found to be si

N-butyl hyoscine exerts local spasmolytic effect in the small and large bowel of the conscious dog.

The spasmolytic activity of N-Butyl Hyoscine (NBH) (1) has been investigated in conscious dogs provided with ileal or colonic Thiry fistulas in which motility was stimulated by intraluminal distension. In ileal motility experiments, phasic motility index (PMI), intestinal tonus, contraction frequency and heart rate were monitored. Intravenous administration of NBH (10 to 100 micrograms/kg) depressed PMI (ED50 35.1 micrograms/kg) and the other motility parameters. Heart rate increases were observed at 100 micrograms/kg. Application of NBH directly into the fistula (300 to 3000 micrograms/kg) ca

Benign gastrocolic fistula associated with peritonitis.

Gastrocolic fistula is most often related to malignancy or previous gastric surgery. It is an uncommon complication of benign gastric ulceration in patients who have not had a previous operation. Benign gastrocolic fistula associated with peritonitis is extremely rare--this case is only the fourth ever reported. The patient presented with an acute abdomen, and subsequent investigations demonstrated a gastrocolic fistula of benign aetiology.

Arterioenteric fistulas.

Any patient with an abdominal aortic aneurysm, aortic graft or previously resected infected graft who presents with gastrointestinal bleeding or sepsis must be assumed to have an arterioenteric fistula. Warning bleeds are common. Oesophagogastroduodenoscopy may be diagnostic and should exclude other causes of upper gastrointestinal haemorrhage. Urgent investigation and surgery are essential for a successful outcome.
