Lactobacillus paracasei from koumiss mediates intestinal stem cells to promote intestinal mucosal recovery through intestinal microbiota-metabolites.

BACKGROUND: Infectious diarrhea caused by Salmonella typhimurium (S. typhimurium) leads to intestinal barrier dysfunction, dysbiosis, and impaired epithelial regeneration. Current antibiotic therapies are prone to adverse effects such as drug resistance and drug residues, whereas probiotic interventions have shown beneficial effects. In this context, the study focused on a traditional fermented product called koumiss, from which a strain of Lactobacillus paracasei (L. paracasei) was isolated. Previous studies have demonstrated that this strain is capable of repairing the damaged intestinal muc

Remote ischemic conditioning avoids the development of intestinal damage after ischemia reperfusion by reducing intestinal inflammation and increasing intestinal regeneration.

AIM OF THE STUDY: Midgut volvulus is a potentially life-threatening condition which is based on intestinal ischemia and reperfusion (I/R) injury. Remote ischemia conditioning (RIC) applied to a limb can protect distant organs such as heart and kidney. The aims of this study were to investigate the effect of RIC on a model of midgut volvulus and to explore its underlying mechanism of action. METHODS: Six-weeks old C57BL/6 mice were studied: (a) sham (n = 4): laparotomy alone. (b) Intestinal I/R injury (n = 5): occlusion of the superior mesenteric artery (SMA) for 45 min followed by reperfusion

Intestinal Surgery in Rabbits.

Advances in rabbit intestinal physiology and diagnostic imaging have clarified surgical indications and improved safety with modern anesthesia and critical care. This review focuses on mechanical obstructions, appendiceal disorders, and intestinal biopsies. Intestinal obstruction remains a common rabbit emergency; most trichobezoars pass spontaneously, but some require intervention. Proximal duodenal obstructions typically need surgery, while jejunal or ileal blockages may be managed medically with aggressive fluids, intravenous lidocaine, and analgesia. When surgery is necessary, decompress,

The intestine and cardiovascular disease.

PURPOSE OF REVIEW: A review of recent publications demonstrating the important role of the intestine in the pathogenesis of cardiovascular disease. RECENT FINDINGS: At baseline (≥ 6 months since myocardial infarction), the pattern of fecal microbiota and blood LPS levels after a meal predicted risk for new major adverse cardiovascular events over the next 7 years. In intestine, tryptophan is metabolized primarily by the kynurenine pathway, which is regulated by the enzyme indoleamine-2,3-dioxygenase 1 (IDO1). Tryptophan flow into the kynurenine pathway limits its availability for the formatio

Intestinal Ketogenesis and Permeability.

Consumption of a high-fat diet (HFD) has been suggested as a contributing factor behind increased intestinal permeability in obesity, leading to increased plasma levels of microbial endotoxins and, thereby, increased systemic inflammation. We and others have shown that HFD can induce jejunal expression of the ketogenic rate-limiting enzyme mitochondrial 3-hydroxy-3-methylglutaryl-CoA synthase (HMGCS). HMGCS is activated via the free fatty acid binding nuclear receptor PPAR-α, and it is a key enzyme in ketone body synthesis that was earlier believed to be expressed exclusively in the liver. The

Intestinal Re-Transplantation.

The history of intestinal transplantation can be traced back to the turn of the twentieth century. Although advancements have been made, the intestine still presents a greater challenge to transplantation than does that of other solid organs, experiencing higher rates of graft rejection and lower long-term survival. Increasingly, intestinal re-transplantation (re-ITx) is seen as a viable option and is now the fourth most common indication for ITx. Changes to immunosuppression protocols, technical modifications, and infectious disease monitoring have contributed to improved outcomes. The author

[Immunopathology of the small intestine].

The gastrointestinal tract is the site of exciting immunological interactions between the epithelium and the mucosa-associated lymphoid tissue, leading to the immune response to food and microbial antigens in the digestive lumen. The objective of this review is to present the main dysimmune pathologies of the digestive tract leading to an enteropathy. As examples, we describe celiac and non-celiac enteropathies to clarify a florid diagnostic framework, by identifying a spectrum of elementary lesions, which must be confronted with the clinico biological context of the patient to orient the diag

Pediatric intestinal transplantation.

Advancements in donor management, organ preservation and operative techniques, as well as immunosuppressive therapies, have provided children with intestinal failure and its complications a chance not only for enteral autonomy but also long-term survival through intestinal transplantation (ITx). First described in the 1960's, experience has grown in managing these complex patients both pre- and post-transplant. The goals of this review are to provide a brief history of intestinal transplantation and intestinal rehabilitation in pediatric patients, followed by focused discussions of the indicat

Primary intestinal melanoma?

The vast majority of malignant melanomas in the small intestine are metastasis of cutaneous tumors. Few cases have been published on primary melanomas in this location, some authors consider that they are always metastatic and that the primary tumor has regressed. In this letter, we present the case of a 77-year-old woman with a history of cutaneous melanoma excision 38 years ago who was diagnosed with ileal melanoma in the absence of other lesions during the study of iron deficiency anemia, and we discuss about the origin of this type of neoplasms.

[Chronic intestinal failure].

The term intestinal failure (IF) is understood as the transient or irreversible loss of the resorptive capacity of the bowels. This includes a multitude of diseases, some of which have anatomical causes and others functional causes. The functional capacity (absorption and motility) of the remaining digestive tract and the bacterial overgrowth and false colonization of the small bowel are of prognostic importance. After exclusion of pathological intestinal findings, such as stenosis and dilatation, initially conservative treatment is employed with the aim of intestinal adaptation. Before failur

Atlas of the human intestine.

In this issue of JEM, Wang et al. (https://doi.org/10.1084/jem.20191130), using a single-cell RNA-seq approach, establish an atlas of human colon, rectum, and ileum epithelial cells. Their study reveals that different regions have specialized nutrient absorption preferences, microbe defenses, and endocrine function. They also identify new markers for a variety of cell types.

Small intestinal diverticula.

The prevalence of small intestinal diverticula (SID) in the population is 0.5-2.3%, and in most cases they are asymptomatic. In the presence of small intestinal bacterial overgrowth this results in chronic diarrhea and malabsorption. When it is complicated by diverticulitis it causes pain and other symptoms of inflammatory bowel disease. Inflammatory process progression may be accompanied by bleeding, invagination, intestinal obstruction, diverticulum abscess and perforation with peritonitis development. SID include separate nosological forms such as paraphateral diverticulum and Meckel's dive

Intestinal Failure in Children.

Children with intestinal failure have had improved survival, particularly those with extreme short bowel syndrome, over the last 10-15 y. This has been attributed to better understanding of the pathophysiology of intestinal failure, improvement in line care, recognition of the importance of a team approach as well as the progress of intestinal transplant as a viable option. Parenteral nutrition remains the cornerstone for the continual survival of these patients. This review will cover contemporary approaches to intestinal failure including post surgical approaches, non-transplant surgery, die

[Adult intestinal no rotation].

90% of cases of intestinal obstruction caused by intestinal malrotation occur in children under 1 year of age, being very uncommon in adults. A female patient of 27 years of age consults with abdominal pain, vomiting, abdominal distention, dehydration and signs of severe malnutrition. After EGDS, CT scan, and 48 hours of clinical expectation, surgical intervention was decided with diagnosis of high intestinal obstruction. Intraoperative finding was no intestinal rotation at all. Ladd´s technique was performed together with prophylactic appendectomy. Postoperative evolution was favorable. The r

Mechanisms of intestinal adaptation.

Following loss of functional small bowel surface area due to surgical resection for therapy of Crohn's disease, ischemia, trauma or other disorders, the remnant gut undergoes a morphometric and functional compensatory adaptive response which has been best characterized in preclinical models. Increased crypt cell proliferation results in increased villus height, crypt depth and villus hyperplasia, accompanied by increased nutrient, fluid and electrolyte absorption. Clinical observations suggest that functional adaptation occurs in humans. In the immediate postoperative period, patients with sub
