[Corneal crosslinking as treatment for progressive keratoconus].

Corneal crosslinking (CXL) is a relatively new medical treatment for progressive cases of keratoconus. This disease is characterized by corneal deformation and thinning, leading to corneal surface distortion and visual disability, mainly occurring in young patients. CXL was first applied in humans in 1999 and is internationally considered the standard treatment for cases of rapidly progressing keratoconus. The treatment protocol consists of corneal epithelial removal, the administration of riboflavin eye drops and irradiation with ultraviolet-A light. The primary goal of CXL is to mechanically…

[Evaluation of dynamic contour tonometry in keratoconus].

PURPOSE: The gold standard for measuring intraocular pressure (IOP) until now has been Goldmann applanation tonometry (GAT), which depends on the central corneal thickness (CCT) and curvature. In patients with keratoconus who have an abnormal corneal geometry and thickness, measurement of pressure with GAT is often difficult and not very reproducible. We compared the impact of the central corneal thickness (CCT) on the IOP measured with dynamic contour tonometry (DCT), a digital method which is adapted to the corneal geometry, and GAT in patients with keratoconus. METHODS: IOP was measured in…

Keratoconus associated with the Pierre Robin sequence.

A 30-year-old female with known Pierre Robin sequence presented to the emergency department of the Royal Victorian Eye and Ear Hospital, Melbourne, with acute unilateral hydrops as her first presentation of bilateral keratoconus. Initial management consisted of eye patching, hypertonic saline and cycloplegia. However, the long-term prognosis for visual rehabilitation remained poor in this patient because of the presence of a central corneal opacity and inability to comply with contact lenses. She was not an appropriate candidate for keratoplasty due to severe mental retardation. This report hi…

Histopathological study of 49 cases of keratoconus.

AIMS: Keratoconus is a bilateral and asymmetrical corneal ectasia. The pathophysiology of this disorder has yet to be fully elucidated. The purpose of our study was to document the prevalence of the most common morphological features of keratoconic corneas. METHODS: A retrospective analysis of 49 cases diagnosed as keratoconus between 2001 and 2006 was undertaken. Histopathological reports were reviewed to obtain data such as age and gender. Specimens were fixed in 10% buffered paraformaldehyde solution for 24 h, bisected through the centre of the button, and embedded in paraffin. Sections we…

Modern treatment options for the therapy of keratoconus.

The following case report describes the implantation of intrastromal corneal ring segments (ICRS) and the postoperative contact lens treatment in a 42-year-old patient with bilateral advanced keratoconus. The patient had a preoperative contact lens intolerance. After femtosecond laser assisted surgery and the implantation of ICRS new custom mini-scleral contact lenses were fitted. The postoperative subjective and objective contact lens tolerance was excellent.

Ultrasound biometry in the diagnosis of amblyopia in keratoconus.

Here, we describe a case in which A-scan ultrasound biometry was used to differentially diagnose the cause of reduced vision in a patient with keratoconus. A patient with keratoconus presented with reduced visual acuity in one eye, not improvable above 6/18 with specialist keratoconus contact lens fitting. This monocular amblyopia was attributed to previous anisometropic ametropia by the use of ultrasound biometry. The report demonstrates the usefulness of ultrasound biometry in the detection of amblyopia in circumstances in which a previous assessment of amblyopia is not possible. This inform…

No VSX1 gene mutations associated with keratoconus.

PURPOSE: To determine whether mutations of the VSX1 gene play a pathogenetic role in the development of keratoconus (KTCN). METHODS: DNA extraction, PCR amplification, and direct sequencing of the VSX1 gene were performed in 100 unrelated patients with diagnoses of clinical and topographic features of KTCN. RESULTS: Of the four previously identified presumed pathogenic mutations in the VSX1 gene (Leu17Pro, Asp144Glu, Leu159Met, and Arg166Trp), only Asp144Glu was identified in a single affected patient. Two novel single nucleotide polymorphisms (SNPs), both resulting in synonymous substitutio…

Morphological changes in keratoconus: pathology or pathogenesis.

Keratoconus was first discriminated from other corneal ectatic diseases in 1854. Since that time the morphological characteristics of keratoconic progression have been invaluable in the diagnosis of the condition. The key clinical features used to identify keratoconus have remained essentially the same since the introduction of the slit-lamp biomicroscope. Only relatively recently has the development of computerized corneal topography revolutionized the diagnosis of early keratoconus. Analysis of peer-reviewed literature databases revealed a steady chronological increase in pathological resear…

Involvement of corneal nerves in the progression of keratoconus.

Keratoconus is a debilitating corneal thinning disease that principally develops in the second and third decades of life. Our group previously developed a novel approach to studying keratoconus, based on the observation that there is a gradient of damage across the keratoconic cone. We identified a number of cellular characteristics of keratoconus such as discrete incursions of fine cellular processes from the anterior keratocytes in association with localised indentation of the basal epithelium, and increased levels of the lysosomal enzymes Cathepsin B and G in aberrant keratocytes, located b…
