INTRODUCTION: In adults hospitalized with community-acquired pneumonia (CAP) after >48 h of outpatient beta-lactam monotherapy, coverage of atypical pathogens is recommended based on expert opinion.
METHODS: In a post-hoc analysis of a large study of CAP treatment we included patients who received beta-lactam monotherapy for >48 h before hospitalization. Length of hospital stay (LOS), 30-day mortality, and number of treatment escalations were compared for those that continued beta-lactam monotherapy and those that received atypical coverage at admission.

BACKGROUND: β-lactams are among the most frequently prescribed antibiotics for the treatment of neonatal sepsis. Survival of extremely preterm neonates necessitates an improved understanding of how β-lactams should be used in this vulnerable population. Appropriate dosing regimens for neonates remain unclear. We reviewed available data on the pharmacokinetics (PK) of β -lactam drugs in neonates. Our aim was to identify long-term β-lactam resistance trends in local Klebsiella pneumoniae isolates, which are a common cause of sepsis in Western Australia. Infection caused by Streptococcus pneumoniae has become increasingly complicated and costly to treat due to the increase in β-lactam resistance. A total of 80 pneumococcal isolates with stepwise increase in penicillin and ceftriaxone resistance were analyzed for the sequential changes in their penicillin-binding proteins (PBPs) 1a, 2b, and 2x. The result provides an insight into the evolution of PBPs associated with β-lactam resistance in a natural population of S. pneumoniae.

BACKGROUND AND PURPOSE: The objective of this structured review was to analyze critically the findings of pharmacokinetic studies of beta-lactam antibiotics in patients with intra-abdominal disease; that is, intra-abdominal infection (IAI) or previous abdominal surgery and determine the requirements for dosage modification in this population.
