Superficial lamellar keratectomy using an automated microkeratome to excise corneal scarring caused by photorefractive keratectomy.

Corneal haze is a recognized complication of excimer laser photorefractive keratectomy. When severe, it can result in the formation of a dense corneal scar, with a reduction in best corrected visual acuity. In extreme cases, medical treatment and repeated phototherapeutic excimer ablations may fail to restore corneal transparency. In these patients, penetrating keratoplasty (PKP) has been used to restore visual function. We describe a technique for excising a superficial corneal scar using an automated microkeratome. The resultant corneal surface is quite smooth, and good visual acuity may be

Laser eye surgery for refractive errors.

Several laser and non-laser refractive surgical procedures have been used to modify the shape of the cornea and correct myopia, hyperopia, astigmatism, and presbyopia. Introduction of the excimer laser to reshape the cornea has resulted in remarkable developments in the correction of these refractive errors. Combined with other advanced ophthalmological instruments, laser refractive eye surgery has resulted in a substantial rise in the safety, efficacy, and predictability of surgical outcomes. Despite these advances, certain limitations and complications persist. In this review, we describe th

Is refractive surgery justified?

Refractive surgery may be defined as any kind of eye surgery in which attempt is made to produce a level of spectacle error that is accepted to the patient. Cataract surgery with lens implantation is the most often done refractive surgery on the eye. The following kinds of refractive surgery are being practised: With normal crystalline lens, phacic intra-ocular lens, surgery with cornea, using excimer laser, other newer techniques. Radial keratotomy is one procedure of refractive surgery with cornea. In this operation, radial cuts are made on the cornea starting near the edge of the pupil and

Mycobacterium chelonae keratitis following laser in situ keratomileusis.

Corneal infection after laser in situ keratomileusis (LASIK) is rare. However, surgical trauma or breakdown of epithelium increases the risk of surface infection. We present the case of a 45 year old woman who developed keratitis due to Mycobacterium chelonae 1 month after LASIK with the Mini-Laser Sight 2000 excimer laser. After an initial improvement following antibiotic therapy the infection progressed until it was necessary to perform penetrating keratoplasty, with a successful result.

Laser therapies for glaucoma: new frontiers.

Glaucoma is a long-term ocular neuropathy defined by optic disc or retinal nerve fiber structural abnormalities and visual-field defects. Treatment for glaucoma consists of reducing intraocular pressure (IOP) to an acceptable target range to prevent further optic-nerve damage. Currently available treatments include topical drug (single then multidrug combinations) followed, for those patients on maximal tolerated medical therapy who still need additional IOP reduction, by laser treatments. These included laser iridotomy, laser trabeculoplasty, laser iridoplasty, laser cyclophotocoagulation. Al

[Intrastromal femtosecond laser-based presbyopia correction].

The surgical correction of presbyopia is gaining more and more popularity in the field of refractive surgery. Besides intraocular treatment with multifocal or accommodative intraocular lenses more and more corneal laser approaches are being established. These are performed either with the Excimer laser as laser in situ keratomileusis (LASIK) or the femtosecond laser. The femtosecond laser treatment is done purely intrastromally without dissection of the epithelium and is characterized by an extremely low risk of infection and inflammation. The procedure leads to a central corneal steepening wi

Enhanced Ablation of Cornea Doped with a Fluorescein Dye Using a Visible Wavelength Laser.

Myopia (nearsightedness) and presbyopia (aging eye) are the most common refractive errors of the human eye. Technology has advanced toward correcting aberration using various surgical procedures, including laser surgery, as well as prescribing corrective lenses. Hence, the effect of various laser wavelengths on the eye has been extensively studied over the last few decades. Usually, excimer lasers are used for this purpose, which increases the cost of the procedure because they are unique and difficult to manufacture and require regular maintenance. Due to the absorption properties, visible wa

Wavefront analysis and Zernike polynomial decomposition for evaluation of corneal optical quality.

Wavefront-guided excimer laser refractive surgery and new customized intraocular lens and contact lens designs are major clinical applications of corneal wavefront analysis. Other therapeutic applications include corneal disorders, conventional excimer laser refractive surgery, incisional techniques and cataract surgery, corneal transplantation, intrastromal corneal ring segment implantation, and crosslinking therapy. Basic data regarding corneal wavefront aberrations, such as distribution in the population and changes with aging, are essential for understanding the nature of each aberration a
