Computed Tomography for Structural Heart Disease and Interventions.

Transcatheter cardiac interventions are a fast evolving field. The past decade has seen the development of transcatheter aortic valve replacement, transcatheter mitral valve repair and replacement, septal defect closure devices and left atrial appendage closure devices for thromboprophylaxis. More than ever, medical imaging is taking a central role in the care of patients with structural heart disease. In this review article we outline the use of MSCT as a tool for diagnosis of structural heart interventions, as well as patient selection, pre-procedural planning, device sizing and post-procedu…

Atrial fibrillation in coronary artery bypass patients: what should be offered.

PURPOSE OF REVIEW: Of patients undergoing coronary artery bypass grafting (CABG), 10-20% have preexisting atrial fibrillation (AF), which remains undertreated at the time of surgery. With rapidly evolving data, interest and enthusiasm for concomitant surgical management of AF have grown concurrently. This review summarizes the contemporary evidence, techniques, and approaches for surgical management of AF in patients undergoing CABG and proposes tailored operative strategies for these patients at the time of surgery. RECENT FINDINGS: Concomitant surgical management of AF at the time of CABG i

Virtual left atrial appendage occlusion in paroxysmal atrial fibrillation during sinus rhythm predicts variable reductions in blood stasis.

Left atrial appendage occlusion (LAAO) is an emerging treatment option for cardioembolic stroke risk reduction in patients with sustained or paroxysmal atrial fibrillation (AF). How LAAO affects the atrial blood flow field during sinus rhythm has not yet been defined. Here, we simulated virtual LAAO in 21 paroxysmal AF patients and eight controls in normal sinus rhythm using computational fluid dynamics (CFD). We found that in AF patients, LAAO reduces the amount of blood that remains in the LA for more than three cardiac cycles to levels similar to the control group with intact LAA. However,

Multicenter Experience of Percutaneous Left Atrial Appendage Occlusion in Current Indications and Different Anesthetic Approaches.

BACKGROUND: Stroke remains a concern in patients with atrial fibrillation despite the efficacy of oral anticoagulants. Left atrial appendage closure has emerged as a treatment option for patients with suboptimal pharmacological therapy. OBJECTIVES: This retrospective multicenter study aimed to evaluate the feasibility, safety, and outcomes of left atrial appendage occlusion procedures for two different indications. Short-term outcomes between two different methods of general anesthesia during the procedures were also compared. METHODS: The study included patients who underwent appendage clos…

New Cerebral Microbleeds After Catheter-Based Structural Heart Interventions: An Exploratory Analysis.

Background Cerebral microbleeds (CMBs) are increasingly recognized as "covert" brain lesions indicating increased risk of future neurological events. However, data on CMBs in patients undergoing catheter-based structural heart interventions are scarce. Therefore, we assessed occurrence and predictors of new CMBs in patients undergoing catheter-based left atrial appendage closure and percutaneous mitral valve repair using the MitraClip System. Methods and Results We conducted an exploratory analysis using data derived from 2 prospective, observational studies. Eligible patients underwent cerebr…

Permanent Atrial Fibrillation is Restored And Sinus Rhythm Maintained Without Catheter Ablation After Atrial Septal Defect Closure: A Case Report.

Giant atrial septal defect (ASD) often is associated with atrial arrhythmia, such as atrial fibrillation (AF). The recovery rate of AF is very low. Moreover, it is difficult for the intervention of a giant atrial septal defect, and it also is more difficult to perform atrial septal puncture and left atrial appendage (LAA) closure after ASD occlusion. Here, we report a case of a giant ASD and permanent AF. We find that the AF is significantly improved after atrial septal defect (ASD) occlusion and left atrial appendage (LAA) occlusion, which is manifested by spontaneous restoration and maintena

Left atrial appendage edema and spontaneous echo contrast following pulsed field ablation for pulmonary vein isolation: A case report.

Pulsed Field Ablation (PFA) is a novel non-thermal ablation technique with several advantages and fewer complications. A 70-year-old male with persistent atrial fibrillation, heart failure with preserved ejection fraction, and prior ischemic stroke who underwent pulmonary vein isolation using PFA with planned left atrial appendage closure (LAAC). Post-ablation transesophageal echocardiography revealed significant dense spontaneous echo contrast in the left atrial appendage (LAA) and left atrial ridge edema, leading to LAAC postponement and anticoagulation initiation. While PFA has advantages o…

An integrated fluid-dynamic and structural pipeline to assess atrial hemodynamics before and after left atrial appendage occluder deployment in a patient-specific anatomy.

Atrial fibrillation is associated with a high risk of thromboembolic events, often mitigated by left atrial appendage occlusion (LAAO) when anticoagulation is contraindicated. This study presents a patient-informed computational workflow integrating pre-operative computational fluid dynamics (CFD), finite element (FE) modeling of device deployment, and post-operative CFD informed by the FE-deformed configuration. A proof-of-concept case was analyzed considering three atrial wall stiffness values to explore biomechanical variability. Pre-implant CFD simulations identified regions of blood stasi

History of Percutaneous Left Atrial Appendage Occlusion with AMPLATZER Devices.

AMPLATZER devices preceded WATCHMAN occluder in 2002 for catheter-based left atrial appendage occlusion. The AMPLATZER technique facilitates simultaneous closure of atrial shunts using two devices through one gear. Randomized WATCHMAN follow-up data showed a mortality benefit over warfarin. AMPLATZER data make this likely valid for the strategy. Particularly young people with atrial fibrillation should be offered left atrial appendage occlusion because the risk is confined to the intervention and early postintervention period. Guidelines should be adapted to make this progress in prevention of

Transcatheter left atrial appendage occlusion with Amplatzer devices to obviate anticoagulation in patients with atrial fibrillation.

It is assumed that over 90% of clinically apparent embolisms in atrial fibrillation originate from the left atrial appendage. Recently, a pe … trials how the simpler Amplatzer technique compares with the complex PLAATO technique, and whether left atrial appendage closure is competitive with oral anticoagulation with warfarin or the novel ximelagatran to prevent thromboembolism in atrial fibrillation.

Left atrial appendage occlusion guided by intracardiac echocardiography in a patient with a 34 mm atrial septal defect occluder: a case report.

BACKGROUND: Intracardiac echocardiography (ICE)-guided left atrial appendage occlusion (LAAO) is increasingly common. Patients with previous atrial septal defect closure constitute a significant challenge for transseptal access. CASE SUMMARY: A 49-year-old man with persistent atrial fibrillation, hypertension, and reduced left ventricular function was considered for LAAO after a life-threatening intrathoracic bleeding while on oral anticoagulation. Percutaneous atrial septal defect closure was performed 15 years before with a 34 mm Amplatzer Septal Occluder. Preprocedural cardiac computed tom

Left atrial appendage occlusion device causing coronary obstruction: A word of caution.

Closure of the left atrial appendage is a common procedure for patients with atrial fibrillation undergoing cardiac surgery. The technique of left atrial appendage occlusion (LAAO) by an extracardiac clip (AtriClip) is established as a reliable method. Acute coronary obstruction of the circumflex artery has already been described after minimally invasive LAAO. Here, we report a case of delayed circumflex artery obstruction after open-heart surgery. A patient who had mitral and tricuspid valve surgery in combination with AtriClip implantation suffered from myocardial infarction 24 h after clip

The role of biomarkers and neuroimaging in ischemic/hemorrhagic risk assessment for cardiovascular/cerebrovascular disease prevention.

Stroke prevention in patients with atrial fibrillation is arguably one of the fastest developing areas in preventive medicine. The increasing use of direct oral anticoagulants and nonpharmacologic methods such as left atrial appendage closure for stroke prevention in these patients has increased clinicians' options for optimal care. Platelet antiaggregants are also commonly used in other ischemic cardiovascular and or cerebrovascular conditions. Long term use of oral anticoagulants for atrial fibrillation is associated with elevated risks of major bleeds including especially brain hemorrhages,…

Multimodality Imaging for Left Atrial Appendage Occlusion Devices.

PURPOSE OF REVIEW: This review summarizes the current and evolving landscape of multimodality imaging for percutaneous endocardial left atrial appendage occlusion (LAAO) across three phases: preprocedural planning, periprocedural guidance, and postprocedural follow-up. RECENT FINDINGS: Transesophageal echocardiography (TEE) remains the primary imaging modality to assess the left atrial appendage (LAA) in all three phases of clinical care surrounding LAAO, though many innovations have led to the rise of cardiac computed tomography (CCT) and intracardiac echocardiography (ICE) as powerful adjunc

Recurrent Gastrointestinal Bleeding in Patients With Atrial Fibrillation Treated With Left Atrial Appendage Occlusion.

BACKGROUND: Concomitant atrial fibrillation (AF) and gastrointestinal (GI) bleeding present a clinical challenge due to recurrent bleeding risk associated with anticoagulation for AF-associated stroke prevention. Left atrial appendage occlusion (LAAO) offers an alternative stroke prevention strategy, but its impact on recurrent GI bleeding remains unknown. METHODS: This retrospective, multicenter cohort study used the TriNetX database to identify adults with AF on oral anticoagulation and a history of GI bleeding. Patients were stratified by treatment with or without LAAO. 1:1 propensity scor

Concomitant LAA Closure during Cardiac Surgery-Update 2025.

Atrial fibrillation is associated with an increased risk of embolic strokes and is present in about one-fourth of all patients undergoing cardiac surgery. Closure of the left atrial appendage (LAA) can effectively reduce the risk of neurological events and is now a class IB recommendation in the most recent ESC/EACTS AF guidelines. The working group "Heart Rhythm Disorders" of the German Society for Thoracic and Cardiovascular Surgery (DGTHG) evaluates the current state of clinical research and recommends concomitant LAA closure in patients with preoperative atrial fibrillation as a routine pa

[Giant Left Atrium Thrombus after Left Atrial Appendectomy:Report of a Case].

A 78-years-old woman was admitted to our hospital due to recurrence of cerebral infarction. She had history of chronic atrial fibrillation, so we performed left atrium appendectomy for prevention of cardiogenic brain embolism. But a giant left atrium thrombus appeared in spite of left atrial appendage closure and anticoagulant therapy. Echo cardiogram showed severe spontaneous echo contrast in the large left atrium. The criteria of mitral valve stenosis were not met, but restriction of the mitral valve opening was suspected. So, we performed removal of left atrium thrombus and mitral valve rep…

Cardioembolic Stroke Three Days Post-Video-Assisted Thoracoscopic AtriClip in a Patient With Paroxysmal Atrial Fibrillation.

Atrial fibrillation is a global epidemic due to aging and chronic diseases. Treatment options are expanding to prevent thromboembolism in anticoagulant-ineligible patients. The left atrial appendage, implicated in 90% of embolic strokes, is increasingly managed with occlusion devices like the AtriClip. A 62-year-old woman with prior stroke, severe gastrointestinal bleeding on anticoagulation, and paroxysmal atrial fibrillation experienced sudden left-sided weakness and altered mental status three days post-AtriClip procedure. Brain MRI revealed acute infarcts in the right cerebellum and scatte
