We report herein a rare case of leiomyosarcoma arising from the greater omentum with concomitant liver metastases. A 63-year-old Japanese woman was admitted to our hospital with an elastic-hard mass in the abdomen, but no other symptoms. The findings of several examinations led us to suspect an omental leiomyosarcoma. A laparotomy revealed an elastic hard and foliaceous mass (12.5 x 9.0 x 8.0 cm) arising from the omentum, and six masses in the liver. A leiomyosarcoma of the uterus in a 54-year-old woman exhibited striking involvement of large vessels of the myometrium and broad ligament on both gross and microscopic examination. The pattern of vascular involvement resembled that seen in intravenous leiomyomatosis. Imaging studies showed recurrent tumor within the inferior vena cava 3 months after hysterectomy. To our knowledge, this is the first reported such case in the literature, for which we propose the designation intravenous leiomyosarcomatosis of the uterus.

Leiomyosarcomas of the spermatic cord are very rare tumours, which, in contrast to the rhabdomyosarcoms of childhood, occur almost exclusively in adults. We report the case of a 51-year-old male patient with a metastasizing, pleomorphic leiomyosarcoma of the spermatic cord. The Authors describe a case of double jejunal diverticulum, characterized by the presence of a leiomyosarcoma in one diverticulum and ectopic pancreatic mucosa in the other. The rarity of the occurrence of a leiomyosarcoma in the diverticular area is outlined; only few cases have been reported in Literature, excluding those originating, from Meckel's diverticulum. Barium X-rays of the small intestine allowed the neoplasia to be discovered, while an Echo-Color-Doppler examination suggested the histological type given the high vascularization typical of leiomyosarcomas.

The case of a 33-year-old man with a primary leiomyosarcoma arising in a mature teratoma in the pineal area is presented. The tumor extended into the posterior part of the third ventricle and caused hydrocephalus. Its smooth muscle derivation was confirmed by immunohistochemistry and electron microscopy. The patient has been followed for more than 2 years after surgery and postoperative radiotherapy. He has full working capacity and there are no signs of tumor recurrence. To our knowledge this is the first presentation of a leiomyosarcoma derived from a teratoma in the pineal area.

A 76 year old man presented with an asymptomatic, very big left scrotal mass that was found to be a leiomyosarcoma of the spermatic cord. It was treated by radical orchiectomy with ligation of the spermatic cord at the internal inguinal ring. Convalescence was uneventful. 6 months postoperatively there was no evidence of local recurrence of the neoplasm or distant metastasis.

Malignant tumors of the gastro-intestinal tract showing both carcinomatous and sarcomatous components are rare. Very few cases were described in the stomach and only one case in the colon. We describe a second case of a tumor of the sigmoid colon showing adenocarcinoma intermingled with leiomyosarcoma elements.

A 57 year-old man was admitted to the hospital because of bloody stools. A barium enema and coloscopy disclosed a tumor of the rectum. The resected tumor was diagnosed as a leiomyosarcoma, histologically. Many epithelioid granulomas with Langhans type giant cells were found in the tumor, and there was no evidence suggestive of generalized sarcoidosis, tuberculosis or mycosis. The case of a 64 year old female who was known to have gallstones is presented. She was admitted to the Hospital following an attack of acute cholecystitis. Ten days after vigorous conservative treatment cholecystectomy was performed. The histological examination showed the presence of the gallbladder leiomyosarcoma. Primary sarcomas of the gallbladder are rare, leiomyosarcoma being the most infrequent type, their preoperative diagnosis almost impossible and their prognosis poor.

Ten children (5 males, 5 females) with leiomyosarcomas were treated at the Children's Hospital, Boston, during the 20-year period from 1963 through 1983. The patients ranged in age from 2 1/2 to 15 years, the average age at diagnosis being 10 1/2 years. Leiomyosarcoma of vascular origin are rare tumors arising most frequently from the inferior vena cava (IVC). We report on three patients one of whom underwent definitive resection. These tumors most commonly involve the upper segment of the IVC, and appear with manifestations of the Budd-Chiari syndrome. Lesions at this level are not amenable to surgical therapy. Tumors of the middle and lower segments of the IVC usually cause right-sided pain. Diagnosis is difficult, but is best approached preoperatively by angiography and vena cavography.
