Neonatal Kawasaki syndrome presenting as out of hospital cardiac arrest rescued by extracorporeal cardiopulmonary resuscitation.

A 7-week-old girl presented in severe shock to a local emergency department. During transfer to the quaternary pediatric hospital, the child had a cardiac arrest and cardiopulmonary resuscitation was commenced en route. Upon arrival to the pediatric intensive care unit, extracorporeal life support was initiated via trans-sternal cannulation. Chest CT performed after extracorporeal life support cannulation, demonstrated widespread aneurysms and a diagnosis of Kawasaki disease was made. Immunomodulatory therapy with immunoglobulin and glucocorticoid medication was commenced and the child was sep

The role of the pediatric home healthcare nurse.

The acuity and number of children with life-threatening, life-limiting, and chronic conditions has increased dramatically over the past decade. The pediatric home care nurse needs a special body of knowledge and repertoire of tools to accurately assess, intervene, manage, evaluate, and provide resources to this most vulnerable population. Inherent in caring for these children is the need to support the family, nurturing the parents who care for chronically ill children at home.

Life skills programmes for chronic mental illnesses.

BACKGROUND: Most people with schizophrenia have a cyclical pattern of illness characterised by remission and relapses. The illness can reduce the ability of self-care and functioning and can lead to the illness becoming disabling. Life skills programmes, emphasising the needs associated with independent functioning, are often a part of the rehabilitation process. These programmes have been developed to enhance independent living and quality of life for people with schizophrenia. OBJECTIVES: To review the effects of life skills programmes compared with standard care or other comparable therapi

Case series of therapeutic hypothermia for neonatal hypoxic-ischemic encephalopathy during extracorporeal life support.

Therapeutic hypothermia initiated within 6 hours of birth is currently the standard of care for the management of neonates with hypoxic-ischemic encephalopathy. Neonates undergoing therapeutic hypothermia for hypoxic-ischemic encephalopathy are also at risk for severe respiratory failure and need for extracorporeal life support. The risks and benefits of therapeutic hypothermia for hypoxic-ischemic encephalopathy during extracorporeal life support are still not well defined. We report our experience of a case series of six neonates who underwent therapeutic hypothermia for hypoxic-ischemic enc

[Basics of emergency ultrasound].

Focused ultrasound is a key methodology of critical care medicine. By referencing few ultrasound differential diagnosis, it is possible to identifying in real-time the reason of the critical state of a patient. Therefore typical focused ultrasound protocols were developed. The well known Focused Assessment with Sonography for trauma (FAST) was incorporated into the Advanced Trauma Life Support (ATLS) for shock room. Focused echocardiographic evaluation in life support (FEEL) has been designed to be conformed with the universal Advanced Life Support (ALS) algorithm and to identify treatable con

Extracorporeal membrane oxygenation (ECMO) in pregnancy and peripartum: a focused review.

As the medical complexity of pregnant patients increases, the rate of maternal morbidity has risen. Maternal cardiovascular disease is a leading cause of maternal morbidity and mortality followed closely by sepsis and infection, both of which may be associated with respiratory failure. There has been an expansion in the application of extracorporeal life support in pregnant and peripartum patients which requires obstetric anesthesiologists to understand the indications, obstetric and medical considerations, relative advantages and potential complications of this invasive technology in this pop

Neonatal Intensive Care Unit Nurses' End-of-Life Care Experience: Becoming a More Empathetic Professional Caregiver.

BACKGROUND: Few studies have examined how empathy develops in neonatal intensive care units (NICUs) as nurses experience negative outcomes in end-of-life care delivery (eg, burnout, moral conflict, and psychological distress). Various characteristics of neonatal care contribute to frequent infant deaths and nurse emotional strain, rendering knowledge of how empathy evolves essential for improving end-of-life care and NICU nurse well-being nationwide. PURPOSE: To explore the experiences of NICU nurses providing end-of-life care for infants in South Korea, focusing on the impact of recurrent ex

Supporting Parental Decision-Making After Life-Limiting Fetal Diagnoses: The Role of Perinatal Hospice and the NOVA-L Decision Support System.

Background: Prenatal diagnosis of life-limiting fetal conditions often leads to counseling focused primarily on therapeutic abortion. Perinatal hospice has emerged as an alternative model of care for families who choose to continue the pregnancy. This paper has two primary aims. First, it discusses structured perinatal hospice programs and their role in supporting parental decision-making after such diagnoses, with attention to ethical and emotional complexities. Second, the paper introduces NOVA-L (Navigating Options & Vital Assistance for Life-limiting conditions), a conceptual Decision Supp

"We need them, and they need us"-Registered nurses' experiences of leading home care workers caring for dying individuals in their last days of life: A content analysis study.

BACKGROUND: A mayority of individuals suffering from life-threatening diseases prefer a home death. Registered nurses (RNs) in home healthcare (HHC) play a crucial role in providing this care, as they are responsible for the caregiving process by leading home care workers (HCWs) who provide bedside care, constituting a complex collaboration. However, the research available on this topic, with a focus on nurse leadership, is rather limited. OBJECTIVE: This study aimed to explore RNs' experiences of leading HCWs who were caring for dying individuals in the last days of life. DESIGN: A qualitat

A Patient-Centered Conceptual Model of AYA Cancer Survivorship Care Informed by a Qualitative Interview Study.

Purpose: Conceptual models provide frameworks to illustrate relationships among patient-, provider-, system-, and community-level factors that inform care delivery and research. Existing models of cancer survivorship care focus largely on pediatric or adult populations whose needs differ from adolescents and young adults (AYAs). We developed a patient-centered conceptual model of AYA survivorship care. Methods: We conducted a narrative literature review of current conceptual and theoretical models of care. We engaged AYA cancer survivors (n = 25) in semi-structured one-hour telephone interview

The experiences of caring for someone with dementia and a learning disability: A qualitative systematic review.

BACKGROUND: The life expectancy of people with a learning disability is increasing and with this comes a greater risk of developing dementia. Dementia poses new challenges for both family and formal learning disability carers as they try to support dementia's progressive nature and quality of life for their care recipient. This qualitative systematic review explores the evidence base of family and formal carers' experiences and needs of caring for someone with both a learning disability and dementia. METHODS: Six electronic databases (PubMed, PsycINFO, Cochrane Library, Prospero, Scopus, CINA

Life expectancy at birth in Duchenne muscular dystrophy: a systematic review and meta-analysis.

Several studies indicate that prognosis for survival in Duchenne muscular dystrophy (DMD) has improved in recent decades. However, published evidence is inconclusive and some estimates may be obsolete due to improvements in standards of care, in particular the routine use of mechanical ventilatory support in advanced stages of the disease. In this systematic review and meta-analysis (PROSPERO identifier: CRD42019121800), we searched MEDLINE (through PubMed), CINAHL, Embase, PsycINFO, and Web of Science for studies published from inception up until December 31, 2018, reporting results of life e

Exploring primary care doctors' perceptions on their roles in palliative care in Singapore: a qualitative study.

BACKGROUND: Singapore faces a rapidly aging population with increasing multimorbidity, creating an urgent need for expanded palliative care services. Despite primary care physicians (PCPs) being ideally positioned to provide palliative care throughout patients’ disease trajectories, their role remains underdeveloped in Singapore’s healthcare system. This study aimed to explore PCPs’ perceptions of their roles in providing palliative care to patients with life-limiting illnesses in Singapore’s primary care setting. METHODS: We conducted a qualitative study at two SingHealth Polyclinics with dis

Digital Respiratory Technologies Across the Lifespan: An Overview of Opportunities and Challenges From Children to Older Adults.

TOPIC IMPORTANCE: Digital health technologies (DHTs), such as mobile health technologies, wearables, telehealth, and telemonitoring, are used increasingly in health care. This is particularly true for respiratory conditions such as asthma, cystic fibrosis, TB, interstitial lung disease, and COPD because DHTs can support diagnosis, self-management, and ongoing care. However, respiratory conditions change across an individual's lifespan in both their presentation and management priorities for the clinician and patient. This creates new challenges and opportunities for using DHTs. Adopting an all

Deactivation of Cardiac Devices at the End of Life: Clinical and Ethical Challenges.

PURPOSE OF REVIEW: Device deactivation in patients with heart failure is an increasingly relevant clinical and ethical challenge as the use of cardiovascular implantable electronic devices expands. RECENT FINDINGS: Current guidelines affirm the ethical and legal permissibility of deactivating life-sustaining devices, including implantable cardioverter-defibrillators, pacemakers, and mechanical circulatory support systems, when requested by patients or surrogates with decision-making capacity. Importantly, guidelines from professional societies recommend that discussions about device deactivat

Palliative Phase of Cancer Rehabilitation.

Palliative cancer rehabilitation plays a critical role in optimizing function, alleviating symptom burden, and enhancing quality of life for individuals with serious or terminal illness. Despite its benefits, rehabilitation and other supportive care services remain underutilized, often due to the lack of ongoing need assessments throughout the survivorship continuum. Physiatrists are uniquely positioned to lead palliative rehabilitation across diverse care settings, using a multidisciplinary approach to address physical decline, psychosocial distress, and caregiver burden. These interventions
