Ligament Injury, Reconstruction and Osteoarthritis.

PURPOSE OF REVIEW: The recent literature on the factors that initiate and accelerate the progression of osteoarthritis following ligament injuries and their treatment is reviewed. RECENT FINDINGS: The ligament-injured joint is at high risk for osteoarthritis. Current conservative (e.g. rehabilitation) and surgical (e.g. reconstruction) treatment options appear not to reduce osteoarthritis following ligament injury. The extent of osteoarthritis does not appear dependent on which joint is affected, or the presence of damage to other tissues within the joint. Mechanical instability is the likely

Posterior cruciate ligament recession.

Three cases of posterior cruciate ligament (PCL) laxity without posterolateral rotatory instability had magnetic resonance imaging scans that documented the structural continuity of the PCL. Tibial PCL recession was effective in eliminating symptomatic laxity in 1 case and lacked efficacy in the other 2 cases.

Traumatic cataract and Wieger's ligament.

The objective of this study was to determine the relationship between posterior subcapsular contusion cataract and Wieger's ligament. Two clinical cases of traumatic cataract, occurring in young patients, were analyzed. In neither case was there evidence of penetrating or perforating injuries to the globe. During cataract extraction a smooth oval defect in the posterior capsule was observed; however, there was no evidence of intraoperative capsular damage. The outline of the defect was consistent with the border of Wieger's ligament. It appears that blunt trauma avulsed this region of lens cap

The inframammary ligament: myth or reality?

After histological investigation, we concluded that a transversely oriented inframammary ligament extending from the sternum to the lateral margin of the pectoralis major muscle is invariably present in female transsexuals. The literature on this subject is reviewed.

Accidental injuries of the hepatoduodenal ligament.

OBJECTIVE: To report our experience in the management of accidental injuries to the structures of the hepatoduodenal ligament. DESIGN: Retrospective study of medical records. SETTING: University department of surgery. SUBJECTS: 8 patients admitted after accidents (road traffic accidents, n = 4; falls, n = 2; and gunshot and stab wounds, n = 1 each) who were found at laparotomy to have injuries of the structures of the hepatoduodenal ligament. INTERVENTIONS: Laparotomy in all cases. MAIN OUTCOME MEASURES: Morbidity and mortality. RESULTS: One patient died (13%) of multiple organ failure a

Ligamentous injuries of the wrist in athletes.

The athlete's wrist is particularly prone to injury. Although many wrist injuries are dismissed as sprains or strains, a number of these injuries involve significant ligamentous damage that can lead to chronic carpal instability. A complete knowledge of the carpal ligamentous anatomy and classification of wrist instability patterns is necessary for the adequate diagnosis and treatment of these injuries.

Muscle, tendon, and ligament evaluation.

Magnetic resonance imaging is the technique of choice for soft tissue evaluation. Muscle injuries occurring during physical activities are v … aluated using ultrasonography. Nevertheless, MR imaging is superior for deeply situated tendons and ligaments of the knee and ankle.

Giant broad ligament leiomyoma.

Giant fibroids are known to arise from the uterus, but occasionally from the broad ligament also. A case of giant broad ligament fibroid is reported for its rarity, and the diagnostic difficulties it posed are discussed.

Asperger's syndrome and ligamentous laxity.

Two autistic girls and one autistic man in whom Asperger's syndrome coexists with lifelong ligamentous laxity and muscular incoordination are described. Two had cranial circumferences at or above the 90th percentile as children, two have complex partial epilepsy, one has a colloid cyst of the third ventricle, and one had evidence of Sotos syndrome. Echocardiography was performed in the two girls and both had evidence of increased aortic compliance. It is suggested that all three suffer from a Marfan-like disorder of connective tissue and, more speculatively, that this has led to anomalous deve

Fractures and ligamentous injuries of the wrist.

Rapid diagnosis, appropriate management, and sport-specific rehabilitation are important in the management of any wrist injury in the athlete. The addition of technetium scans, CT, arthroscopy, and MRI in the evaluation of the athlete's wrist has provided valuable and powerful diagnostic tools. The versatility and relatively low invasiveness of arthroscopic intervention and the subsequent early return to activity make this technique especially attractive in the high-performance athlete. Suspicion of potential injuries, complete knowledge of wrist mechanics and anatomy, and a careful physical e

Tibial collateral ligament bursitis.

In 91 patients evaluated between 1982 and 1985, tibial collateral ligament bursitis was diagnosed. This entity has not been described since the work of Brantigan and Voshell in 1943. The diagnosis was based on the findings of tenderness over the tibial collateral ligament at the joint line, without a history of mechanical symptoms. With a nonsurgical program, 62% of the patients improved and subsequently required no further treatment. Fifteen percent did not improve; of this group, one-third had a negative arthrogram or arthroscopy. Another 23% did not improve and ultimately underwent an arthr

Defects of the broad ligament of the uterus.

A case of intestinal obstruction resulting from passage of the entire small bowel and cecum and its mesentery through a fenestration in the broad ligament of the uterus is presented. A review of the literature reveals that broad ligament defects may occur in multiple locations, which may be responsible for intestinal obstruction. A simple classification of broad ligament defects is proposed. Type 1 defects occur caudal to the round ligament of the uterus. Type 2 defects occur above the round ligament. Type 3 defects occur between the round ligament and the remainder of the broad ligament, thro

The regenerative potential of the periodontal ligament.

The current decline in the incidence of dental caries indicates that patients will retain most of their dentition. Reconstruction or regeneration of the entire attachment apparatus, including the periodontal ligament, cementum, and bone, is an attainable therapeutic goal. Clinical treatments designed to reattach connective tissue to exposed root surfaces commonly result in the formation of a long junctional epithelial attachment. This attachment is probably not an effective barrier to bacterial toxins and may allow recurrent pocket formation. To enhance reattachment of connective tissue, root-
