Trans-mastoid facial nerve localisation for malignant neoplasms confined to the parotid gland.

Up to 32% of parotid neoplasms are malignant, and treatment for resectable parotid carcinomas remains primarily surgical. Resection is centred round the identification and preservation (where possible) of the main trunk of the facial nerve. Limited dissection of the temporal bone and identification of the facial nerve proximally in the mastoid portion can facilitate resection for all parotid carcinomas that occur in the preauricular portion of the gland. Dissection in the narrow space between the ear or base of the skull and the tumour reduces the risk of tumour spillage, and the margin for re…

Volume enhancement of upper lip in secondary cleft deformity using composite mastoid tissue.

BACKGROUND: Secondary cleft lip deformity correction needs thorough assessment and planning to optimize treatment and achieve good results. Deficient volume of the upper lip, which is one of the most common secondary cleft lip deformities, has been addressed in many ways using tissues from local or distant areas. Consideration must be given to safety, easy availability of the tissues, minimum morbidity, and longevity of the results when selecting a procedure. METHODS: This retrospective study included ten patients with a minimum follow-up of 1 year. Composite mastoid fascia was harvested thro…

Impact of endolymphatic mastoid shunt surgery on saccule and lateral semicircular canal function.

Endolymphatic mastoid shunt surgery (EMSS) is widely performed in patients with medically intractable Meniere's disease. Although many patients report an improvement of symptoms after surgery, the mechanisms which are responsible for the relief of complaints are not known. To date, only few studies exist which studied the influence of EMSS on vestibular function. The present study examines the effect of EMSS on saccule function by measuring vestibular evoked myogenic potentials and the effect on lateral semicircular canal function by sinusoidal harmonic acceleration (SHA) testing. No changes i…

Observations on mastoid versus ear canal recorded cochlear microphonic in newborns and adults.

The cochlear microphonic (CM) may play an important role in the diagnosis of auditory neuropathy (AN) in newborns. However, since the CM ten … in humans by investigating the following parameters: (1) secondary minus electrode recording site (mastoid versus ear canal [EC]), (2) stimulus parameters, and (3) grounding and shielding conditions. Normative data were collected in full-term newborns (n = 7) and adults (n = 4) with no known risk factors for cochlear or retrocochlear pathology. Results s …

Salvage of cartilage framework exposure in microtia reconstruction using a mastoid fascial flap.

One of the common complications of microtia reconstruction with cartilage grafting is exposure of the framework. Recently, we used a mastoid fascial flap for salvage operations in two microtia patients, who had partial skin necrosis with resultant exposure of the cartilage framework. Following the excision of the necrotic skin, the anteriorly based mastoid fascial flap was elevated and transferred as a hinge flap on to the portion of the exposed framework. Split-thickness skin grafts were applied over the mastoid fascial flap. Each skin graft took well and the relief of the reconstructed ear w…

Morphometric examination of the paranasal sinuses and mastoid air cells using computed tomography.

BACKGROUND: The paranasal sinuses and mastoid air cells vary considerably in size and shape from person to person. The main structures are pneumatic. In this study, we investigated the relationship between right and left sides and evaluated the volume changes according to age and sex. METHODS: Of all patients attending the radiology department, 91 cases without paranasal sinuses and mastoid air cells pathology (i.e., inflammation, operation or trauma) were selected for evaluation. Axial computed tomography (CT) scans were obtained for both paranasal sinuses and temporal bones. In all scans, t…

Investigation of the mastoid electrode contribution to the brain stem auditory evoked response.

The auditory nerve and bran stem responses recorded at the vertex and mastoids were closely examined in normal subjects using a larynx electrode as a neutral reference point. With monaural stimulation, significant activity was found at the vertex and both mastoids, and the wave-forms at the ipsi- and contralateral mastoids were consistently different. With binaural stimulation the small amplitude mastoid response were largely in phase with the vertex response. The implications of the findings are discussed.

Size of the mastoid air cell system in children with middle ear effusion.

The mastoid air cell system probably functions as an air reservoir. Among adults with chronic otitis media a small mastoid air cell system is prevalent In cases of chronic nonpurulent middle ear effusion among children, the reports have been contradictory concerning the size of the mastoid air cell system. In the present investigation, white children with long-standing problems of middle ear effusion have been examined by means of X-rays of the mastoid process in the lateral projection and the size of the air cell system measured by planimetry. The size of the air cell system was found to be s…

The volumetric relationship of mastoid cavity and paranasal sinuses in unilateral chronic otitis media.

AIMS: A number of studies link ear and paranasal sinus disorders even though correlation between the development of the paranasal sinuses and the mastoid has not been demonstrated clearly. In this study, we examined temporal bone computed tomography (CT) scans to calculate mastoid and paranasal sinus volumes using three-dimensional (3D) reconstruction in patients with unilateral nonsuppurative chronic otitis media (COM) and investigated whether there was a correlation between mastoid and paranasal sinus volumes. METHODS: Of 85 patients with unilateral nonsuppurative COM, 51 patients who under…

Otoscleroma of the Middle Ear and Mastoid Cavity with Facial Palsy: A Case Report.

INTRODUCTION: Scleroma is a chronic, specific granulomatous disease that affects the head and neck mucosa. Its common sites are the nose and larynx; however, it might affect other areas. One of the rare sites to be affected is the middle ear and mastoid cavity, for which the term otoscleroma was coined. We present such a rare case in this report. CASE REPORT: A 47-year-old patient with a history of both old laryngoscleroma and recent Rhinoscleroma. He presented with symptoms of ear discharge and facial palsy. Examination revealed complete facial nerve palsy and an external auditory canal poly…

Pneumatization Pattern and Status of the Mastoid Antrum in Chronic Otitis Media: A Review.

Chronic otitis media is generally more prevalent in individuals with a weakly pneumatized temporal bone, whereas acute otitis media has a propensity for pneumatized temporal bone. Antimicrobial drugs are thought to have influenced the incidence and progression of middle ear infections. The mastoid air cell system, a part of the middle ear cleft, has recently been recognized as a crucial factor in the genesis, behaviour, course, and outcome of middle ear inflammatory conditions. Epithelium infiltrates the growing bone and produces epithelium-lined air cell chambers, a process known as pneumatiz…

Characteristics and Management of Dilated Mastoid Emissary Veins: A Case Report and Literature Review.

INTRODUCTION: The mastoid emissary vein (MEV) describes a transosseous connection between the sigmoid dural venous sinus and the suboccipital venous plexus. In cases of outflow stenosis or malformation, the MEV may become dilated and a source of pulsatile tinnitus (PT) amenable to treatment. We describe a case of PT secondary to MEV treated successfully via endovascular coil embolization and conduct a systematic review of the literature. METHODS: We performed a systematic review without meta-analysis of studies involving management of dilated MEV on January 14, 2022, and describe a case of PT…
