Suboptimal care and maternal mortality among foreign-born women in Sweden: maternal death audit with application of the 'migration three delays' model.

BACKGROUND: Several European countries report differences in risk of maternal mortality between immigrants from low- and middle-income countries and host country women. The present study identified suboptimal factors related to care-seeking, accessibility, and quality of care for maternal deaths that occurred in Sweden from 1988-2010. METHODS: A subset of maternal death records (n = 75) among foreign-born women from low- and middle-income countries and Swedish-born women were audited using structured implicit review. One case of foreign-born maternal death was matched with two native born Swe…

Evaluating maternal mortality, risk factors, and emergency c-section timing in polytrauma pregnant patients.

BACKGROUND: Trauma is a leading non-obstetric cause of maternal morbidity and mortality, yet standardized guidelines for the management of pregnant polytrauma patients remain limited. Physiologic changes of pregnancy may alter trauma response and complication risk, but data guiding risk stratification and time-sensitive interventions are sparse. OBJECTIVE: To evaluate maternal mortality and complications among pregnant polytrauma patients, with particular focus on maternal age, early blood transfusion, and timing of emergency cesarean section. METHODS: This retrospective cohort study analyze…

Maternal mortality from obstetric hemorrhage in Brazil: a 10-year analysis (2015 to 2024).

BACKGROUND: Obstetric hemorrhage is a leading preventable cause of maternal mortality (MM) worldwide. This study described MM due to obstetric hemorrhage in Brazil and examined temporal trends and associations with maternal age, skin color, educational level, and geographic region. METHOD: We conducted a nationwide population-based cross-sectional study using data from the Mortality Information System (SIM) and Live Birth Information System (SINASC), including 2,985 hemorrhage-related maternal deaths recorded between 2015 and 2024. Causes were classified as first-half pregnancy hemorrhage, pl…

Maternal pre-pregnancy diabetes and risk of all-cause and cause-specific infant mortality.

BACKGROUND: Maternal pre-pregnancy diabetes is associated with a higher risk of adverse pregnancy outcomes. Few large, cohort studies have assessed associations with a wide large range of causes of infant death. METHODS: This retrospective cohort study included all singleton live births to women aged 18-49 years in the US National Vital Statistics System from 2011 to 2020. Multivariable Poisson regression models were used to estimate adjusted relative risks (RRs) with 95% confidence intervals (CIs) for all-cause and cause-specific infant mortality. RESULTS: Of 34  918  803 pregnant women wit

Maternal mortality in Bangladesh: Who, when, why, and where? A national survey-based analysis.

BACKGROUND: Despite a notable decline in recent decades, maternal mortality in Bangladesh remains high. A thorough understanding of causes of maternal deaths is essential for effective policy and programme planning. Here we report the current level and major causes of maternal deaths in Bangladesh, focusing on care-seeking practices, timing, and place of deaths. METHODS: We analysed data from the 2016 Bangladesh Maternal Mortality and Health Care Survey (BMMS), conducted with nationally representative sample of 298 284 households. We adapted the World Health Organization's 2014 verbal autopsy…

Maternal mortality and its determining factors among hospitalised mothers in Tehran, Iran, 2013-2020.

The maternal mortality ratio (MMR) is a significant indicator of the quality of a health care system. Despite considerable progress in reducing MMR in Iran in the past few years, we still face a long road ahead in eliminating preventable maternal deaths. In the present study, we evaluated all 80 cases of maternal deaths in 20 hospitals under the supervision of Tehran University of Medical Sciences between March 20 2013 and March 19 2020. During this time, these hospitals recorded 335,216 live births, with an MMR of 24 per 100,000 live births. The average age of deceased mothers was 31.9 ± 6.9 …

Risk Factors for Maternal Mortality in Rural Tigray, Northern Ethiopia: A Case-Control Study.

BACKGROUND: Maternal mortality continues to have devastating impacts in many societies, where it constitutes a leading cause of death, and thus remains a core issue in international development. Nevertheless, individual determinants of maternal mortality are often unclear and subject to local variation. This study aims to characterise individual risk factors for maternal mortality in Tigray, Ethiopia. METHODS: A community-based case-control study was conducted, with 62 cases and 248 controls from six randomly-selected rural districts. All maternal deaths between May 2012 and September 2013 we…

Systematic review and meta-analysis: association between water and sanitation environment and maternal mortality.

OBJECTIVE: To assess whether the lack of water or the lack of sanitation facilities in either the home or in health facilities is associated with an increased risk of maternal mortality and to quantify the effect sizes. METHODS: Systematic review of published literature in Medline, Embase, Popline and Africa Wide EBSCO 1980. RESULTS: Fourteen articles were found. Four of five ecological studies that considered sanitation found that poor sanitation was associated with higher maternal mortality. Meta-analysis of adjusted estimates in individual-level studies indicated that women in households …

Maternal mortality in health institutions with emergency obstetric care facilities in Enugu State, Nigeria.

In order to assess the current level of maternal mortality in health institutions with comprehensive emergency obstetric care in Enugu State, South Eastern Nigeria, a retrospective analysis of maternal deaths for the years 1999-2003 was carried out to establish the maternal mortality ratios in the eligible health institutions. Each maternal death was studied in detail to establish the socio-demographic characteristics of the women who died; their referral sources, type of delay (if any), medical causes of death and their preventability. In-depth interviews of the service providers were carried…

Adverse pregnancy outcomes in successive pregnancies and maternal cardiovascular mortality: a Norwegian nationwide study.

BACKGROUND AND AIMS: Binary classification of adverse pregnancy outcomes (APOs) may obscure differences in women's long-term cardiovascular disease (CVD) mortality. CVD mortality was therefore evaluated considering APO sequence and lifetime parity. METHODS: A total of 1 001 409 women with complete pregnancy histories were identified from the Medical Birth Registry of Norway (1967-2020) and linked to the Norwegian Cause of Death Registry for premature CVD mortality (≤70 years). Pregnancy histories were categorized by lifetime parity (1 vs ≥2) and APO sequence (first vs later) for composite APO

Neonatal and infant mortality after maternal influenza and pertussis vaccination: Probabilistically linked cohort study.

Maternal influenza and pertussis vaccination is an important strategy to reduce morbidity and mortality in infants. Previous vaccine safety studies have mostly focused on the association between maternal vaccination and fetal death. We conducted a probabilistically linked cohort study of mother-infant pairs in three Australian jurisdictions: Queensland, Northern Territory, and Western Australia, to evaluate the risk of all-cause neonatal and infant mortality associated with seasonal influenza and pertussis maternal vaccination. Vaccination records were obtained from regional immunization datab
