Giant cell granuloma is an uncommon bony benign lesion that generally involves the mandible and maxilla. It may be locally aggressive and result in extensive tissue destruction in advanced cases. Surgery is the traditional and still the most accepted treatment for giant cell granuloma. We report a pediatric case of central giant cell granuloma of the maxilla treated with videoendoscopic assisted surgical excision.

This article studies the anatomy of the posterior maxilla pertaining to bone-cut design of Le Fort I osteotomy to avoid the injury to the descending palatine artery in Thais. Fifty-five skulls (38 males, 17 females) were assessed for the anatomical landmarks by a combination of direct inspection, computerized imaging, and computed tomography scan analysis. The results showed that 27.28% of the pterygomaxillary junction (PMJ) became synostosis. The problem of medical rehabilitation of patients with asymmetrical deformation of the facial skull, complicated by cross occlusion, is discussed. The authors suggest osteotomy of the maxillary complex with translocation of the osteotomied fragment in the sagittal, transverse, and vertical planes with osteotomy to a distance needed for elimination of deep cross occlusion. This method in combination with orthodontic treatment repairs the anatomic and functional abnormalities and brings about good esthetic results.
