Symptomatic maxillary sinus retention cysts: should they be removed?

OBJECTIVES/HYPOTHESIS: Recently, endoscopic sinus surgery (ESS) endoscopic sinus surgery (ESS) okay? has become the surgical procedure of choice for removing retention cysts from the maxillary sinus. The aim of our study was to determine the relationship between symptomatic relief and ESS with or without endoscopic excision of maxillary cysts. STUDY DESIGN: Prospective, randomized study. METHODS: Inclusion criteria were symptomatic maxillary cysts filling at least 50% of the sinus space. We conducted a prospective, randomized study comprising 80 patients. Of the patients, 41 underwent endosc…

Maxillary sinus anatomy: a cadaveric study with clinical implications.

This study measured maxillary sinus volume, evaluated the location of the semilunar hiatus in correlation to the nasal floor, and the incidence, location, and height of antral septa and discusses their clinical implications. Maxillary sinus volume was quantified in 65 cadavers (130 sinuses) by water application through the semilunar hiatus and measuring the used amount. The location of the semilunar hiatus was identified as distance from the nasal floor. The septa were counted, evaluated, and the size measured from the antral floor. The medium maxillary sinus volume was 12.5 mL (range, 5-22 mL…

Canine fossa trephine for the severely diseased maxillary sinus.

BACKGROUND: The appropriate surgical approach to the severely diseased maxillary sinus is still debatable. Some advocate creating a wide maxillary antrostomy to allow for increased ventilation and improved delivery of postoperative medication. Others, however, recommend more aggressive surgery with complete clearance of all diseased tissue. The aim of this study was to evaluate the outcome of canine fossa puncture and canine fossa trephine (CFT) for the severely diseased maxillary sinus. METHODS: A retrospective chart review was performed on 97 patients who underwent a CFT as a part of endosc…

Nasal challenge with allergen leads to maxillary sinus inflammation.

BACKGROUND: Allergic rhinitis and chronic rhinosinusitis are both characterized by chronic inflammation. OBJECTIVE: We sought to investigate the effect of nasal allergen challenge on the maxillary sinus and study the effect of premedication with loratadine. METHODS: We performed a double blind, crossover, randomized, placebo-controlled study in 20 allergic subjects out of season. After treatment with either placebo or loratadine (10 mg PO daily) for 1 week, a catheter was inserted into one maxillary sinus and used to lavage the cavity. The subjects then underwent nasal challenge with diluent…

Primary hydatid cyst of the maxillary sinus: case report.

Hydatid disease (also known as hydatidosis) is a cyclozoonotic infection caused by the cestode genus Echinococcus. It occurs throughout the world and is especially common in sheep- and cattle-raising regions of Africa, Australia, New Zealand, India, the Middle East, South America, and the Mediterranean. The incidence of humans infected with hydatid disease is approximately 1.0-2.0:1,000, although it may be higher in rural areas of regions that are affected. Infection occurs via ingestion of infected meat. Primary infection of the maxillary antrum is odd and very rare. This article reviews a ca

Absence of nasal air flow and maxillary sinus development.

UNLABELLED: Paranasal sinuses development mechanisms are not well known. Nasal air flow, according to one of the proposed theories, would be fundamental to the growth and healthy development of paranasal sinuses. AIM: The aim of this study was to evaluate the maxillary sinus growth and health in the presence and absence of postnasal air flow through a unique model. MATERIALS AND METHODS: Retrospective study of a series of cases; preoperative CT scans of 7 patients with unilateral choanal atresia, average age was 16.28 years (+/- 5.024). This study was done in a tertiary hospital, with patien…

Fatty tissue within the maxillary sinus: a rare finding.

BACKGROUND: We report a rare case of fatty tissue within the maxillary sinus in a 21-year-old woman, with a history of several previous punctures of the maxillary sinus. CASE PRESENTATION: Clinical data of the patient was analysed retrospectively. The patient presented with symptoms of left-sided chronic maxillary sinusitis and had undergone several punctures of the left maxillary sinus 18 months earlier. Subsequent to one of the procedures an acute pain in the left orbit lasting a couple of days was noted. Left endoscopic transnasal antrotomy was performed. The maxillary sinus was filled wit…

Maxillary sinus pathology in 119 patients--a histopathologic study.

INTRODUCTION: Information on histopathological changes within sinus mucosa is lacking, both in the general population, and in subjects with specific facial morphology and chronic respiratory obstruction. AIMS AND OBJECTIVES: To provide baseline data of the frequency and nature of pathological changes in the maxillary sinus in a specified group of patients. METHODS: 119 patients undergoing elective maxillary osteotomies were selected. These included patients with (70) and without (49) vertical maxillary excess (VME), and mouth and nasal breathers. The frequency and nature of the histopatholog…

Surgical revision of the post-Caldwell-Luc maxillary sinus.

BACKGROUND: Endoscopic sinus surgeons are commonly faced with the management of patients with persistent maxillary sinusitis despite previous Caldwell-Luc surgery. Given the potential for altered mucociliary clearance in the post-Caldwell-Luc maxillary sinus, the optimal approach for surgical revision has not been well characterized. The objective of this study was to review our experience in endoscopic versus repeat Caldwell-Luc approaches in patients who have failed Caldwell-Luc surgery for chronic maxillary sinusitis. METHODS: Retrospective chart review was performed on patients who had a …

Solitary extramedullary plasmacytoma of the maxillary sinus. Case report.

Solitary extramedullary plasmacytomas are rare tumors that often affect head and neck region. Because of the non-specific associated symptomatology, they frequently are misdiagnosed. We briefly describe a 69-year-old woman who developed solitary plasmacytoma in the left maxillary sinus and was initially treated as having sinusitis. We also report the diagnostic work-up that is necessary to establish a correct diagnosis in such cases. This case highlights that an appropriate investigation for neoplastic disease should be performed in patients presenting with persistent symptoms that resemble th…

Top 10 reasons for endoscopic maxillary sinus surgery failure.

OBJECTIVE/HYPOTHESIS: Endoscopic sinus surgery has enjoyed impressive success curing chronic disease in sinuses and has virtually replaced the Caldwell-Luc procedure for correction of problems with the maxillary sinus. Unfortunately, a significant number of patients have persistent maxillary symptoms after one or more endoscopic sinus operations. Existing reviews of this issue have identified only a few general causes for surgical failure. METHODS: The records of 85 patients presenting to the author over a 5-year period with persistent maxillary sinus symptoms were reviewed. RESULTS: In revi…
