Chronic sinusitis unresponsive to medical therapy: a case of maxillary sinus actinomycosis focusing on computed tomography findings.

Actinomycosis of the paranasal sinuses is a rare occurrence and its clinical presentation does not suggest a specific diagnosis. Therefore, actinomycosis should be included in the differential diagnosis of neoplasms and granulomatous lesions of the head and neck region. However, the differentiation from a malignant neoplasm is not easy because the radiological findings are frequently similar and positive cultures are difficult to obtain. This report highlights the clinical progress of paranasal actinomycosis associated with some computed tomography findings that can be extremely helpful in the

[Incidence of maxillary sinusitis in intensive care patients with nasal intubation--ultrasonic diagnosis as a screening test].

Acute maxillary sinusitis due to nasotracheal intubation during intensive care is seldom considered as a cause of fever of unknown origin. Forty six patients admitted to a post-operative intensive care unit had their maxillary sinuses examined using a mobile "A-scan" ultrasonic scanner. Follow-up examinations were performed regularly. As early as the 5th day of treatment 36 out of 46 patients who had undergone nasotracheal intubation showed pathological ultrasonic results. Bilateral involvement was most frequent. In unilateral findings at the first examination, the intubated side was three tim…

Drug Release and Pharmacokinetic Evaluation of Novel Implantable Mometasone Furoate Matrices in Rabbit Maxillary Sinuses.

BACKGROUND: Intranasal corticosteroid sprays (INCSs) used to treat chronic rhinosinusitis are suboptimal due to limited penetration into the middle meatus, rapid clearance, and poor patient compliance. A bioresorbable drug matrix, developed with the XTreoTM drug delivery platform, may overcome the limitations of INCS by providing continuous dosing over several months. OBJECTIVE: To evaluate the in vitro drug release and in vivo pharmacokinetics of novel mometasone furoate (MF) matrices in a rabbit dorsal maxillary osteotomy model. METHODS: XTreoTM matrices were formulated to consistently elu

The Modified Caldwell-Luc Approach with the Use of Collagen Material for Treatment of a Chronic Perforated Maxillary Sinusitis.

Oroantral fistula (OAF) often develops after extraction of posterior maxillary teeth and requires surgical closure. If it persists, OAF may result in maxillary sinusitis. This paper reports a case of an oroantral fistula, associated with chronic maxillary sinusitis. A 46-year-old female patient presented with a history of traumatic tooth extraction, which led to OAF formation. Three unsuccessful attempts were made to close it elsewhere. With one operation, we performed Caldwell-Luc surgery and closed OAF with a collagen membrane and plug and a buccal flap. The patient was assessed at 1-, 3-, 6…

Comparison of the nasal bacterial floras in two groups of healthy subjects and in patients with acute maxillary sinusitis.

The nasal bacterial flora was studied in 183 healthy men entering military service (entry group), 103 healthy recruits in service (service group), and 185 recruits with acute maxillary sinusitis. The 267 nasal and ipsilateral sinus aspirate findings in the same patients with acute maxillary sinusitis were compared pairwise. In the entry group presumed sinus pathogens were only rarely isolated from the nasal cavities: Haemophilus influenzae in 4%, Streptococcus pneumoniae in 1%, Branhamella catarrhalis in 3%, and Streptococcus pyogenes in 0%. The corresponding isolation frequencies in the servi…

[Concentrations of antibiotics (ampicilline, doxycycline) in the chronic-hyperplastic alterated mucosa of the maxillary sinuses (author's transl)].

Surgical treatment of chronic sinusitis is usually preceeded by the attempt of curing the disease by antibiotic and antiphlogistic therapy. Since no studies on the concentration of antibiotics in chronic-hyperplastic mucosa of the maxillary sinuses could be found in the literature, two groups of each 20 patients were subjected to treatment with ampicilline (Binotal) and doxycycline (Vibramycin) respectively and the concentrations of the antibiotics in the maxillary sinus tissue gained during operation were measured. It was shown that therapy with doxycycline produces higher tissue levels than
