Patient specific finite element model of the face soft tissues for computer-assisted maxillofacial surgery.

This paper addresses the prediction of face soft tissue deformations resulting from bone repositioning in maxillofacial surgery. A generic 3D Finite Element model of the face soft tissues was developed. Face muscles are defined in the mesh as embedded structures, with different mechanical properties (transverse isotropy, stiffness depending on muscle contraction). Simulations of face deformations under muscle actions can thus be performed. In the context of maxillofacial surgery, this generic soft-tissue model is automatically conformed to patient morphology by elastic registration, using skin

Temporomandibular Joint Disorders and Pain Confounders: An Awareness Study.

OBJECTIVES: Temporomandibular disorders (TMD) are the most common nonodontogenic cause of orofacial pain, leading to morbidity and impairment. TMD presents a diagnostic challenge due to many aetiologies that exhibit comparable symptoms and refer pain to the temporomandibular joint (TMJ) region. Patients may be referred to dental specialists without accounting for all pain sources. This study aims to identify radiographic confounders (RCs) that can be mistaken for TMD in patients undergoing TMJ assessment using cone-beam computed tomography (CBCT). MATERIALS AND METHODS: A review of 369 CBCT o

Full-Thickness Oral Mucoperiosteal Defects: Challenges and Opportunities.

Regenerative engineering strategies for the oral mucoperiosteum, as may be needed following surgeries, such as cleft palate repair and tumor resection, are underdeveloped compared with those for maxillofacial bone. However, critical-size tissue defects left to heal by secondary intention can lead to complications, such as infection, fistula formation, scarring, and midface hypoplasia. This review describes current clinical practice for replacing mucoperiosteal tissue, including autografts and allografts. Potentially paradigm-shifting experimental regenerative engineering strategies for mucoper

Muscle Regeneration of the Tongue: A Review of Current Clinical and Regenerative Research Strategies.

Various abnormalities of the tongue, including cancers, commonly require surgical removal to sequester growth and metastasis. However, even minor resections can affect functional outcomes such as speech and swallowing, thereby reducing quality of life. Surgical resections alone create volumetric muscle loss whereby muscle tissue cannot self-regenerate within the tongue. In these cases, the tongue is reconstructed typically in the form of autologous skin flaps. However, flap reconstruction has many limitations and unfortunately is the primary option for oral and reconstructive surgeons to treat

A Pilot Study Assessing the Incidence of Chronic Sinusitis Following Le Fort I Osteotomy in Maxillofacial Surgery.

Orthognathic surgery utilizing a Le Fort I osteotomy is performed regularly by oral surgeons to correct midface and dental occlusal abnormalities, yet little has been written discussing the impact these operations may have on sinonasal function. The objective of this study was to assess the incidence of objective sinonasal inflammation and subjective sinonasal symptoms following the use of Le Fort I osteotomies for maxillary advancement surgery.Thirty-eight subjects who previously underwent Le Fort I osteotomies for purposes of elective orthognathic surgery were enrolled retrospectively to ass

Pituitary Tumors in Maxillofacial Radiology and Daily Practice: A Scoping Review.

Background: Lateral cephalometric radiographs and large-field cone-beam computed tomography (CBCT) routinely used in orthodontics and maxillofacial radiology can reveal incidental pituitary tumors in the sellar region. Given the regular use of these imaging modalities, a structured overview of how pituitary tumors present on dental radiographs and how often they occur is clinically relevant. Methods: A scoping review was conducted according to PRISMA-ScR. MEDLINE via PubMed, Livivo, and Google Scholar were searched up to 20 January 2026 using MeSH terms and keywords for pituitary tumors and de

Orthognathic Surgery Using 1-Desamino-8-D-Arginine Vasopressin (DDAVP) in a Patient With Von Willebrand Disease: A Case Report.

Von Willebrand disease (vWD) is a bleeding disorder with autosomal-dominant inheritance caused by quantitative and qualitative abnormalities in von Willebrand factor (vWF), which plays a role in primary hemostasis. In such cases, it is necessary to evaluate hemostatic function prior to surgery and take appropriate measures to manage bleeding. There are few reports on orthognathic surgery, which is a highly invasive procedure in the oral and maxillofacial regions, and all of them use blood products. We report a case of orthognathic surgery using 1-desamino-8-D-arginine vasopressin (DDAVP), a sy

A case of extreme skeletal class III Malocclusion beyond the envelope of discrepancy, managed effectively by a modified ortho-surgical protocol.

Correction of severe anteroposterior skeletal discrepancy, as described in this case of Extreme Skeletal Class III Malocclusion, can be challenging and fraught with difficulties. Conventional, single stage Bi-jaw Orthognathic surgery, with pre-and post-surgical orthodontics is associated with drawbacks such as risk of relapse and an unsatisfactory outcome, with persisting occlusal discrepancies and skeletal abnormalities, especially when the magnitude of skeletal correction is large. Excessive mandibular setback restricts tongue space, narrows the posterior airway and pharyngeal spaces, and is

Maxillary Distraction Osteogenesis in a Patient With Osteogenesis Imperfecta.

Osteogenesis imperfecta (OI) is characterized by brittle bones, premature hearing loss, blue sclera, dental abnormalities, and short stature. Maxillofacial pathology is marked in many OI patients and includes a high incidence of class III malocclusion secondary to a retrusive maxilla relative to both the mandible and cranial base.Review of literature shows that most of the orthognathic surgeries performed in the setting of OI are double jaw surgeries, in the form of maxillary advancement and mandibular setback. However, severe maxillary hypoplasia is usually not correctable with single-stage m

Discovery and characterization of spontaneous mouse models of craniofacial dysmorphology.

Craniofacial abnormalities are among the most common features of human genetic syndromes and disorders. The etiology of these conditions is often complex, influenced by both genetic context and the environment. Frequently, craniofacial abnormalities present as part of a syndrome with clear comorbid phenotypes, providing additional insight into mechanisms of the causative gene or pathway. The mouse has been a key tool in our understanding of the genetic mechanisms of craniofacial development and disease, and can provide excellent models for human craniofacial abnormalities. While powerful genet

Craniofacial development in cleft lip and palate children related to different treatment regimes.

Outcome of treatment in 85 children with unilateral cleft, lip, alveolus, and palate who had been treated according to four regimes was studied using lateral skull radiographs, extra oral photographs, intra oral radiographs and orthopantographs. The treatment regimes differed with regard to: the inclusion of presurgical orthopaedics in one group, the type of lip closure, the type and timing of palatal closure, and timing of bone grafting to the alveolus. A method of pooling subgroups and comparing lateral skull radiographs from different centres was developed to obtain more meaningful comparis
