A study on posttraumatic experience of road traffic accident afflicted maxillofacial trauma patient at tertiary hospital.

OBJECTIVES: Patients are usually left in a vulnerable state after an accident. Because of this, they long for a good encounter when they are brought to the hospital. Physical impairment and psychological morbidities are some of the complications that can occur to them. Traditionally, surgeons tend to pay little attention to a patient's emotional and psychological perspective. The aim of this study was to understand the experience of oral and maxillofacial trauma patients due to road traffic accident right from immediate after the accident till the end of definitive treatment. Delayed treatment of zygomatic tetrapod fracture.

Since maxillofacial injury is frequently accompanied by other diseases, its evaluation and treatment are open delayed. When the evaluation is delayed, the surgical treatment can be difficult or impossible. A 21-yr-old man presented with right facial swelling and deformity after injury. We planned immediate surgical repair for his right tetrapod fracture, but the operation was delayed for two months due to severe hyperthyroidism. During the operation, we reducted and fixed the deviated bone after refracture of the zygomatic arch with an osteotome to achieve mobility. The facial deformity and di

Ophthalmic consequences of mid-facial trauma.

Fractures of the mid-face are commonly accompanied by injury to the visual system. Three hundred and sixty three patients who had sustained mid-facial fractures were assessed prospectively for evidence of such injury. The data have been used to devise a scoring system for the maxillofacial surgeon in order to help identify those patients at risk of eye injury. The scoring system has been evaluated on a further cohort of 100 patients with a similar spectrum of injury and the sensitivity and specificity of the system have been determined. The results of these studies have been reported in the ma

Surgical treatment of painful inferior alveolar nerve injuries following endodontic treatment: a consecutive case series of seven patients.

PURPOSE: Injuries of the inferior alveolar nerve (IAN) related to endodontic treatment are being increasingly reported. However, consensus on the preferred intervention and the timing of and indications for surgical treatment is lacking. Here, we describe our experience with painful IAN injuries arising from endodontic treatment and requiring prompt microsurgical treatment. METHODS: Seven consecutive patients with painful IAN injuries were referred to the Maxillofacial Surgery Unit of San Paolo Hospital in Milan. All patients had undergone root canals endodontic treatment in the mandibular mo

Endoscopic repair of anterior table frontal sinus fractures.

Frontal sinus fractures account for 5 to 15% of all maxillofacial injuries. The majority of these fractures are the result of high-velocity injuries such as motor vehicle accidents, assaults, and sporting events. The treatment algorithm for complex frontal sinus fractures is controversial due to the associated risks of brain injury, meningitis, cerebrospinal fluid fistula, and mucocele formation. However, mild to moderately displaced anterior table fractures carry a relatively low risk of long-term morbidity and are generally treated as aesthetic deformities. Unfortunately, the coronal approac…

Analysis of Surgical Complications in Mandibular Fractures in the Center of Italy: A Retrospective Study.

Mandibular fractures are the second most common maxillofacial fractures. The prevalent treatment for this kind of fractures is either a surgical procedure such as maxillomandibular fixation or open reduction and internal fixation or a combination of both. Sometimes the patients might develop locoregional postoperative complications. The most frequently reported complications are trigeminal V3 injury and surgical site infection. A 4-year retrospective study was performed at the Department of Oral and Maxillofacial Surgery, Policlinico Umberto I of Rome, Italy, and the postoperative complication

Nerve harvesting procedures.

Autogenous nerve grafts currently set the standard for reconstruction of motor and sensory nerve injuries that cannot be repaired by mobilization, advancement, and approximation, and suturing without tension. Donor nerves such as the GAN and SN are easily accessible and frequently used in maxillofacial microneurosurgery. Other nerves (MACN, LCNF, LTN) may serve well in special nerve reconstruction situations. In the future, availability of biodegradable nerve conduits with neurotrophic factors may render obsolete the reconstruction of the short-span nerve gap (< 3 cm) by autogenous nerve graft

Principles and clinical application of micronerve surgery and nerve transplantation in the maxillofacial area.

Modern microsurgery techniques using perineural interfascicular-nerve suturing permit successful nerve reconstruction in tumor defects and injuries of the maxillo-facial area. Since 1972 we have concentrated on nerve grafts and reconstructed the facial, accessory, hypoglossal, lingual, and mandibular nerves. In all cases, exclusively autologous nerve grafts were used; as donor nerves we found the great auricular and sural nerves especially suitable. Along with typical examples of the different nerve defects in the maxillofacial surgery, our technique in these special nerve transplantation proc

Frontal Sinus Fractures: Evolving Clinical Considerations and Surgical Approaches.

Frontal sinus fractures are an uncommon injury of the maxillofacial skeleton, and account for 5-15% of all maxillofacial fractures. As the force of impact increases, fractures may extend beyond the anterior table to involve adjacent skull, posterior table and frontal sinus outflow tract (FSOT). Fractures at these subsites should be evaluated independently to assess the need for and type of operative intervention. Historically, these fractures were managed aggressively with open techniques resulting in obliteration or cranialization. With significant injuries, these approaches are still indispe

[Management of craniofacial fractures with titanium mini-plate osteosynthesis and primary bone transplantation].

The frontobasal traumatic lesions are often combined with several type of midface fractures therefore the authors adopted the craniofacial injury definition. In the years 1995-1997, 534 patients with head trauma were operated on by maxillofacial surgeon and neurosurgeon of which 35 (6.5%) cases showed concomitant combined frontonaso-orbital-skull base involvement. In the complex management of the severe craniofacial lesions the cooperative team-work of maxillofacial surgeon, neurosurgeon, otolaryngologist and ophthalmologist is required. Stabilized midface and the "watertight" dura closure or

Preoperative Nutritional Laboratory Values, Demographics, and Wound Healing Following Facial Trauma Surgery: Which Variables Predict Postoperative Complications?

In individuals who have sustained maxillofacial trauma, inadequate nutrition is often a sequela and may lead to complications. The purpose of this study was to investigate the association between preoperative laboratory values and postoperative complications in patients with maxillofacial trauma requiring surgical intervention. A retrospective cohort study of patients with maxillofacial trauma requiring surgical repair from 2014 to 2020 was performed at a single academic Level I Trauma Center. The primary predictor variables were preoperative laboratory values including serum albumin, white bl

Epidemiology of maxillofacial trauma in elderly patients receiving oral anticoagulant or antithrombotic medication; a Swiss retrospective study.

BACKGROUND: The percentage of elderly trauma patients under anticoagulation and antiplatelet agents has been rising lately. As newer agents are introduced, each comes with its own advantages and precautions. Our study covered elderly patients admitted to the ED with maxillofacial trauma while on anticoagulation (AC) or antiplatelet therapy (APT). We aimed to investigate the demographic characteristics, causes, and types of maxillofacial trauma, along with concomitant injuries, duration of hospitalisation, haemorrhagic complications, and the overall costs of care in the emergency department (ED

The Reconstruction of Mandible Defects in War Injuries: Systematic Review and Meta-Analysis.

STUDY DESIGN: Systematic Review and Meta-Analysis. OBJECTIVE: There has been an increasing trend in maxillofacial injuries associated with combat trauma. Within the maxillofacial complex, the mandible is the most likely structure to be damaged during combat. The structural deficits as a result can be reconstructed with many options. These include vascularised bone grafts (VBGs), non-vascularised bone grafts (NVBGs), alloplastic implants, reconstruction bars and distraction osteogenesis. This study aimed to determine the common modality and efficacy of mandibular reconstruction in combat traum…

Surgical Treatment of Complex Craniofacial Fractures.

OBJECTIVE: Severe craniofacial fractures may present different needs in treating intracranial lesions over facial injuries. This paper examines the results of our strategy, consisting of a single-stage combined neurosurgical-maxillofacial treatment. METHODS: A retrospective review was conducted of 33 consecutive patients with complex fractures of the anterior cranial fossa and facial skeleton, who required elective surgery for craniofacial reconstruction. Patients who required emergency surgery for intracranial clots or penetrating wounds were excluded. In all cases, all or almost all the ant

World trends in publication for maxillofacial and dental trauma in the elderly.

Aging may increase the risk of maxillofacial and dentoalveolar trauma (MDT). This review assessed global trends in scientific publications on MDT in the elderly. Six databases were searched. Studies about MDT in the elderly (≥60 years old) were included in two text mining stages (S1 and S2). In S1, all studies with elderly subjects were included, while in S2 only studies in which the elderly were assessed in a separate set of data or in a subanalysis by age groups were included. In S1, relevant data were mined using VantagePoint™ software. In S2, data were descriptively analyzed. There were 22
