Thinking in three dimensions: a web-based algorithm to aid the interpretation of tuberculin skin test results.

BACKGROUND: The tuberculin skin test (TST) is the most widely used test for detecting tuberculosis (TB) infection. Accurate interpretation of TST requires consideration of three dimensions-the size of the skin reaction, the positive predictive value (PPV) and risk of disease. METHODS: We developed a web-based algorithm incorporating epidemiological, medical and radiographic risk factors to help in the interpretation of positive TST results in adults (http://www.meakins.mcgill.ca/meakins/NEW TST Calculator/homeE.htm). We used summary estimates from published reviews on the prevalence of latent

Standard Elements in Studies of Adverse Events and Medical Error (SESAME): explanation and elaboration.

Studies that evaluate adverse events and medical error often suffer from incomplete and inadequate reporting, which affects the interpretability and reproducibility of their findings. To address this, we developed the 44-item Standard Elements in Studies of Adverse Event or Medical Error (SESAME) checklist using a rigorous process following the recommendations of the Enhancing the QUAlity and Transparency Of health Research Network. The aim of this manuscript is to explain the scope and aim of the checklist by providing a detailed explanation of each item in the SESAME checklist, illustrated b

Impact of the new European legislation regarding vocational driving on London Transport Medical Service: the doctor's role.

The outcome of the first 1000 medical examinations undertaken by London Transport Medical Service (LTMS) to comply with the regulations for non-passenger carrying public carriage vehicle licence holders was analysed to assess where the primary role of the examining doctor affected the final outcome. It was found that the primary involvement of the doctor possibly altered the outcome in only two cases. In 49 cases, the medical practitioner had an interpretative role in the final assessment, or in the pursuit of further information, such as clarification of past history from other sources. In th

Efficacy of neuroleptic medication in pervasive developmental disorders of childhood.

A number of studies pertaining to the use of antipsychotic medication in the treatment of pervasively disordered children are reviewed. The drugs examined are chlorpromazine, haloperidol, thioridazine, thiothixene, trifluoperazine, lithium, and other miscellaneous agents. Medication side effects are also reviewed with particular attention to extrapyramidal symptoms and tardive dyskinesia. Although methodological problems complicate the interpretation of the studies, the results support the use of antipsychotic medication in this population. The agents do not have the specificity for psychotic

[Chinese expert consensus on colonic and anorectal manometry (2023 edition)].

Colonic and anorectal manometry includes anorectal manometry and colonic manometry. Anorectal manometry is a common method to evaluate anorectal function, which can objectively reflect the pathological and physiological abnormalities of outlet obstructive constipation and fecal incontinence, as well as the impact of anorectal surgery on continence. Colonic manometry is a new type of colon motility detection method developed in recent years. It can record the peristalsis and contraction of the whole colon through a pressure measuring catheter, which helps physicians further evaluate various col

Well prepared for work? Junior doctors' self-assessment after medical education.

BACKGROUND: Apart from objective exam results, the overall feeling of preparedness is important for a successful transition process from being a student to becoming a qualified doctor. This study examines the association between self-assessed deficits in medical skills and knowledge and the feeling of preparedness of junior doctors in order to determine which aspects of medical education need to be addressed in more detail in order to improve the quality of this transition phase and in order to increase patient safety. METHODS: A cohort of 637 doctors with up to two years of clinical work exp

A physics primer on photon-counting detectors in CT: Physics, signal formation, and performance.

Photon-counting detector CT (PCD-CT) is moving from laboratory development to clinical deployment. However, its threshold-bin data arise from a tightly coupled detector-readout chain that can be difficult to understand as a whole. These data are not a direct readout of photon energy, but the outcome of a sequence of physical and electronic processes that begins with x-ray interaction and charge creation and ends with threshold decisions applied to processed electrical pulses. Understanding this chain is essential for interpreting detector behavior, evaluating performance, and identifying the o

Beyond "Time to Surgery": a structured evidence review and multi-clock framework for emergency cranial neurosurgery.

Timing is often treated as a decisive variable in emergency cranial neurosurgery, but studies frequently use different temporal anchors, intervention endpoints, and outcomes under the same label of "early" intervention. This structured evidence review with narrative synthesis proposes a multi-clock framework for interpreting timing-to-intervention evidence across acute epidural hematoma, acute subdural hematoma, severe traumatic brain injury requiring decompressive surgery, and aneurysmal subarachnoid hemorrhage. We reviewed clinical guidelines, randomized trials, comparative-effectiveness stu

Performance of large language models in answering frequently-asked questions on celiac disease.

OBJECTIVES: Celiac disease (CeD) is a common autoimmune condition requiring lifelong adherence to a gluten-free diet (GFD). Patients and caregivers increasingly seek information online, and large language models (LLMs) have emerged as potential educational tools. However, their reliability in CeD remains uncertain. This study aimed to evaluate the performance of three popular LLMs in answering frequently asked questions (FAQs) about CeD and GFD management. METHODS: We conducted a cross-sectional comparative evaluation in which 12 FAQs were submitted to three LLMs: ChatGPT-4 (OpenAI), Gemini F

Standardizing the Seated Saline Suppression Test for Primary Aldosteronism.

BACKGROUND: Primary aldosteronism is the most common secondary endocrine form of hypertension. Specific treatment directly against aldosterone excess improves control of (and may cure) hypertension and reduces long-term cardiovascular and renal morbidity in patients with primary aldosteronism. Optimal treatment is guided by the results of adrenal venous sampling, with unilateral adrenalectomy being the preferred management for those with lateralizing forms and medical treatment with a mineralocorticoid antagonist for those with bilateral forms. Seated saline suppression testing, being highly s

World Sleep Society recommendations for the use of wearable consumer health trackers that monitor sleep.

Wearable consumer health trackers (CHTs) are increasingly used for sleep monitoring, yet their utility remains debated within the sleep community. To navigate these perspectives, we propose pragmatic, actionable recommendations for users, clinicians, researchers, and manufacturers to support CHT usage and development. We provide an overview of the evolution of multi-sensor CHTs, detailing common sensors and sleep-relevant metrics. We advocate for standardized 'fundamental sleep measures' across manufacturers, distinguishing these from proprietary exploratory metrics with future potential. We o

Potential Risk to Brain Health after Surgical Interventions: Biomarkers to Predict the Occurrence of Cognitive Decline.

As the global population ages, the number of people living with dementia is projected to increase significantly. Estimations indicate that over 150 million people worldwide will be living with dementia by 2050, with Alzheimer's disease being the most common cause. Elderly people are also at greater risk of undergoing surgery, either elective or emergency, escalating the associated likelihood leading to cognitive decline, especially if accumulative. However, the relationship between surgery and dementia development remains controversial. The cause seems to lie in the heterogeneous preoperative

Assessment of Treatment Response in Patients With Diffuse Large B-cell Non-Hodgkin Lymphoma Using the Deauville Criteria: A Case Report and Literature Review.

Diffuse large B-cell lymphoma (DLBCL) represents the most prevalent subtype of non-Hodgkin lymphoma in the adult population and is typically associated with an aggressive clinical trajectory. Despite significant therapeutic advancements, a proportion of patients exhibit resistance to first-line chemotherapy, underscoring the need for accurate and early assessment of treatment response to inform prognosis and optimize therapeutic strategies. We describe the case of a 64-year-old male with a medical history notable for ischemic heart disease and metabolic syndrome who presented with abdominal pa

Wisconsin Card Sorting Task-64 performance among HIV+ Black/African American and Latinx adults compared to normative samples and by sociocultural and health variables.

Many people living with HIV experience cognitive impairment, and HIV disproportionately affects racial/ethnic minority groups. Independent of HIV, racial/ethnic minority individuals perform worse than White individuals on cognitive tasks, even after accounting for education. Our goals were to (1) compare WCST-64 scores between HIV+ Black/African American (Black/AA) (n = 45) and Latinx (n = 41) urban-dwelling adults; (2) compare our total sample to the WCST-64 manual's normative (N) and clinical normative (CN) groups; and (3) explore relationships between WCST-64 performance and sociocultural/h
