Gait kinematics and kinetics in patients with different grades of meniscus injury: A cross-sectional study.

Meniscal injuries are common and can alter knee biomechanics, increasing the risk of osteoarthritis. This study investigated the effects of unilateral meniscal injuries of different Stoller grades on gait kinematics and kinetics. A total of 158 participants were stratified by MRI into three groups: control group(Grade 0, n = 51), Grade I-II (n = 54), and Grade III (n = 53). Three-dimensional motion capture synchronized with force platforms was used to assess peak sagittal-plane joint angles, joint moments, and ground reaction forces. Multivariate analysis of covariance was applied to adjust fo

Anatomic All-Inside Repair of the Anteroinferior and Posterosuperior Popliteomeniscal Fascicles for Hypermobile Lateral Meniscus.

Hypermobile lateral meniscus refers to abnormal mobility of an otherwise morphologically normal meniscus without obvious tears, causing lateral knee pain due to catching or locking of the lateral meniscus during flexion. The most common cause is rupture or elongation of the popliteomeniscal fascicles (PMFs), which connect the lateral meniscus to the joint capsule around the popliteus tendon. Hypermobile lateral meniscus is most commonly managed with simple, minimally invasive all-inside meniscal repair techniques or inside-out suture techniques. However, nonanatomic suturing not only results i…

Clinical Outcomes of Hidden Lesions of the Medial Meniscus Treated With a Flexible All-Inside Device.

Purpose Ramp lesions are characteristic injuries associated with anterior cruciate ligament (ACL) rupture and involve disruption of the meniscus-synovium interface near the posterior horn of the medial meniscus. Among these, tibiofibular side-only tears at the bursal attachment, so-called hidden lesions, are difficult to visualize from the standard anterior portal, making their diagnosis and treatment challenging. This study examined the outcomes of suturing hidden lesions using an all-inside technique with the FAST-FIX device from the anterior portal, with a focus on clinical results and the …

Ligament of anterior horn of medial meniscus of knee joint: case reports and literature review.

BACKGROUND: The meniscus is essential for knee joint biomechanics, contributing to load distribution and stability. Variations such as the anterior medial meniscofemoral ligament (AMMFL) originating from the anterior horn of the medial meniscus (AHMM) are rare but significant. These variations may predispose individuals to medial meniscus injuries and knee dysfunction, though their biomechanical implications remain unclear. The AMMFL disrupts normal anchorage of the AHMM to the tibial plateau, increasing meniscal mobility and potentially altering load transmission and predisposing to degenerat…

The influence of peripheral vision on driving performance in patients implanted with an inverted meniscus intraocular lens.

The purpose was to analyze and compare the influence of peripheral vision on driving while performing secondary visual-manual tasks in patients implanted with two types of intraocular lenses (IOLs): a standard monofocal IOL and a new inverted meniscus intraocular lens (ArtIOL). This study included 17 participants implanted binocularly with a standard monofocal IOL (control group) and 15 participants implanted binocularly with the ArtIOL. Visual acuity and contrast sensitivity were tested at 40 deg of eccentricity. Driving performance was assessed using a driving simulator. At some points of th…

A case study of a nonsurgical patient with a medial meniscus tear: A role for gluteal muscle physiotherapy.

The aim of this case study is to report how treatment and rehabilitation for meniscus tears can exacerbate the underlying condition or improve function. In addition, the objective of the study is to compare quadriceps physical therapy in a patient to gluteal muscle therapy. There are different types of meniscus tears, and each rehabilitation plan depends on the location and type of meniscus injury. Treatment options include conservative, nonsurgical therapy and surgical repair or replacement therapy. Typically, strengthening the quad muscles is the focus of physiotherapy. Few studies have asse…

Wrisberg variant of the lateral discoid meniscus in children: review of the literature and presentation of case series.

INTRODUCTION: The Wrisberg variant of the discoid lateral meniscus is a very rare disorder and is characterized by the hypermobility and instability of the meniscus caused by the absence of its posterior tibial attachment, with only its meniscofemoral junction (Wrisberg's ligament) maintained, and inserted in the posterior horn of the meniscus. As a result, the posterior horn of the lateral meniscus is mobile; often subluxing into the joint. MATERIALS AND METHODS: A total of eight skeletally immature patients with symptomatic Wrisberg variant of the discoid lateral meniscus were included in t…

In vitro method to quantify and visualize mechanical wear in human meniscus subjected to joint loading.

The mechanical wear and tear of soft connective tissue from repetitive joint loading is a primary factor in degenerative joint disease, and therefore methods are needed to accurately characterize wear in joint structures. Here, we evaluate the accuracy of using a structured light 3D optical scanning system and modeling software to quantify and visualize volume loss in whole human meniscus subjected to in vitro joint loading. Using 3D printed meniscus replicas with known wear volumes, we determined that this novel imaging method has a mean accuracy of approximately 13 mm3, corresponding to a me…

Editorial Commentary: Surgical Management of Degenerative Meniscus Lesions Is a Second- But Not a First-Line Treatment.

The 20-year progression of osteoarthritis (OA) after arthroscopic partial meniscectomy (APM) in patients aged between 50 and 70 bears a long-term risk of conversion to total knee arthroplasty of 15.7%. Negative predictors at the time of surgery are the degree of knee OA, lateral meniscectomy, age at surgery, and malalignment. This confirms the evolution of the natural history of knee OA, but most importantly, it provides arguments to further restrain indications of APM in degenerative meniscus lesions (DMLs). An improved understanding of the consequences of APM for DMLs allows to increasingly …

The relationship between a Segond fracture and meniscus injury in patients with anterior cruciate ligament tears.

BACKGROUND: Segond fracture may be identified when an anterior cruciate ligament (ACL) tear is diagnosed, and it has historically been considered a sign of significant knee trauma indicating intra-articular injury. There are few studies on the correlation between a Segond fracture and meniscus injury. The purpose of this study was to evaluate the relationship between a Segond fracture and meniscus injury in patients with ACL tears. METHODS: A retrospective review of all patients undergoing acute ACL reconstruction (less than 1 month from the time of injury) between 2013 and 2020 was performed…

Editorial Commentary: Saucerization Is Superior to Total Meniscectomy in Patients With Symptomatic Discoid Lateral Meniscus.

Current treatment recommendations favor meniscal rim preservation through partial meniscectomy with repair when indicated in patients with s … erienced surgeon because visualization within the lateral joint space may be limited by a thickened meniscus and the small size of the pediatric knee. To preserve a stable peripheral rim, various meniscal repair methods should be used for stabilizing the reshaped meniscus on the capsule based on repair location, tear type, and surgeon preference.

Diagnosis and Management of Articular Cartilage and Meniscus Pathology in the Posterior Cruciate Ligament-Injured Knee.

Posterior cruciate ligament (PCL) injuries commonly occur in association with participation in sporting or recreational activities or due to a direct trauma. Cartilage and meniscal lesions are prevalent in PCL-injured knees with increasing likelihood and severity based on extent and duration of trauma to the knee. As such, comprehensive diagnostics should be performed to ascertain all related pathology, and patients should be thoroughly educated regarding treatment options, likely sequelae including posttraumatic osteoarthritis, and associated outcomes. Treatments should address the joint as a

Radial Width of the Lateral Meniscus at the Popliteal Hiatus: Relevance to Saucerization of Discoid Lateral Menisci.

BACKGROUND: A discoid lateral meniscus (DLM) is a congenital anomaly of the knee in which the lateral meniscus has an "O" shape and contains irregular, abnormal collagenous tissue. A DLM can cause mechanical symptoms and pain. Treatment of a symptomatic DLM is arthroscopic saucerization to reshape the meniscus to a more normal contour. Enough tissue must be removed to eliminate mechanical symptoms but not too much to create instability. The residual width of the meniscus is crucial at the popliteus hiatus because here the peripheral rim is unattached at the capsule. Reports in the literature r…
