Astasia, asymmetrical asterixis and pretectal syndrome in thalamo-mesencephalic hemorrhage.

We report a patient who presented with sudden onset instability and diplopia. On neurological examination he had asymmetrical asterixis, predominantly in the left hand, and ocular findings consistent with a pretectal syndrome. He was also unable to stand or even sit up unassisted, with a tendency to tilt his body and rotate his trunk axis to the left. Brain MRI showed a hemorrhage in the right thalamo-mesencephalic junction. This region involves important structures for the control of postural stability, motor control, ocular movements and vestibulo-ocular integration, not yet well understood.

MRI in non-alcoholic Wernicke's encephalopathy.

Wernicke's encephalopathy (WE) is a potentially reversible disorder. It is often not considered in non-alcoholic patients unless MRI demonstrates lesions in the appropriate sites. However, specific MRI sequences only highlight some areas of abnormality and hence WE may not be considered unless a more complete study is performed and this is highlighted in the case described herein. The neuropathological basis for the imaging findings is also discussed.

Microsurgical anatomy of the mesencephalic veins.

OBJECT: The mesencephalic veins drain crucial brainstem areas. Due to the narrowness of the tentorial notch, these veins can become obstructed as a result of herniation or surgery, leading to hemorrhage and severe consequences for the patient. There is little in the literature about the mesencephalic veins. The aim of this study was to perform an exact analysis of their microanatomy. METHODS: Fifty-two cadaveric hemispheres were examined under an operating microscope, and measurements were made with a digital caliper. The authors focused on the basal vein (BV), pontomesencephalic vein (PMV),

MRI characteristics of the MLF in MS patients with chronic internuclear ophthalmoparesis.

OBJECTIVE: The authors imaged the medial longitudinal fasciculus (MLF) in 58 patients with MS and chronic internuclear ophthalmoparesis (INO) to determine which MRI technique best shows the characteristic lesion associated with this ocular motor syndrome. METHODS: Using quantitative infrared oculography, the authors determined the ratios of abduction to adduction for velocity and acceleration, to confirm the presence of INO and to determine the severity of MLF dysfunction in 58 patients with MS and INO. Conventional MRI techniques, including proton density imaging (PDI), T2-weighted imaging,

Cavernous dilatation of mesencephalic Virchow-Robin spaces with obstructive hydrocephalus.

We describe two patients with mild ventricular dilatation, shown to have cystic spaces in the midbrain, which we interpreted as greatly enlarged Virchow-Robin spaces. We discuss the pathophysiology and the possible relations to the mild hydrocephalus.

[Isolated lesion of the Edinger-Westphal nucleus in topographical relation with a post-traumatic mesencephalic hematoma].

INTRODUCTION: From the relevant literature, it would seem that the commonest single cause of lesion of the third cranial nerves is indirect, accompanying intracranial traumas. From multiple clinical observations however, it seems that many of these cases may be due to lesions of the mesencephalum which nevertheless have rarely been identified by current imaging techniques. Clinical case. We describe the clinical observation of isolated pupil involvement, attributed to a lesion of the Edinger-Westphal nucleus as a consequence of a mesencephalic haematoma in the context of closed craneo-encephal

[Midbrain hemorrhage presenting with trochlear nerve palsy--a case report].

A case of midbrain hemorrhage with isolated trochlear nerve palsy was reported. A 70-year-old female noticed vertical diplopia of insidious onset. Ouclar motility was normal. But her diplopia increased with right head tilt and in the left downward gaze, and disappeared with left head tilt. She had no other neurological symptoms and signs. CT scan revealed high density spot in the left tectum of lower midbrain. This lesion decreased over months but small high density spot existed five months later, which means calcified lesion, probably vascular anomaly. This is the first stroke case of trochle

Histochemical distribution of zinc in the brain of the zebra finch (Taenopygia guttata).

The distribution of zinc was studied in the brain of the zebra finch (Taenopygia guttata) by means of the selenium histochemical method. A s … uclei. Only the lateral nucleus of the habenula was stained, and with strong intensity. Most of the mesencephalon stained rather uniformly with a moderate to strong intensity. The most intense staining was seen in the substantia grisea centralis, the substantia grisea et fibrosa periventricularis, the torus semicircularis and the nucleus intercollicularis. The …

Mesencephalic hemorrhage--a report of 3 cases.

We studied three patients with spontaneous mesencephalic hemorrhages. All presented with some degree of impaired consciousness and abnormal ocular movements. One patient had a convergence-retraction nystagmus with a unilateral hemorrhage confined to the dorsal midbrain. Another patient showed left third nerve palsy due to a unilateral hemorrhage that involved ventral midbrain. The third patient revealed left internuclear ophthalmoplegia with ataxia due to a pinpoint hemorrhage in the tegmentum of the left midbrain. The patients survived and major neurologic deficits recovered, but abnormal ocu

Perimesencephalic hemorrhage: a nonaneurysmal and benign form of subarachnoid hemorrhage.

We studied 28 patients with subarachnoid hemorrhage and normal angiograms. On early CT (within 5 days) in 13 cases, blood was seen mainly or only in the cisterns around the midbrain. This pattern of hemorrhage was found in only 1 of 92 patients with a ruptured aneurysm. None of the unexplained perimesencephalic hemorrhages was associated with intracerebral hematoma or intraventricular hemorrhage. The clinical features also differed from those of aneurysmal hemorrhage; loss of consciousness was rare, and after 3 months, all 13 patients had returned to normal life. The cause of this benign disor

Computed tomography of the brain stem with intrathecal metrizamide. Part I: the normal brain stem.

In summary, the anatomic details of the brain stem are exquisitely demonstrated with metrizamide CT. The obex is the zone of transition from … a few millimeters lower than the isthmus. The latter constitutes the junction of the pons with the mesencephalon. The colliculi divide the midbrain into two levels of axial cross section: the inferior collicular level, which may or may not show the trochlear nerve, and the superior collicular level, which features anteriorly the suprasellar cistern and its con …

Decerebrate rigidity in humans.

Decerebrate rigidity (DR) in humans results from a midbrain lesion and is manifested by an exaggerated extensor posture of all extremities. It is characterized by shortening and lengthening reactions and can be modified by tonic neck, labyrinthine (Magnus-de Kleijn), and phasic spinal reflexes. These criteria, and not extensor posture alone, reflect the observations of Sherrington and should form the basis of the clinical examination; however, the experimental-anatomical lesion and physiological findings can never be reproduced exactly in humans. "Tonic and cerebellar fits" are not the equival

Hormones and midbrain mediation of courtship behavior in the male ring dove (Streptopelia risoria).

The nucleus intercollicularis (ICo) region of the midbrain has been shown to concentrate testosterone and to be involved in the neural control of avian vocal behavior. The present study investigated the possible role of the ICo region in the androgen-dependent courtship behavior of male ring doves, with the use of radio-frequency lesion and intracranial hormone-implant techniques. In the first experiment, bilateral lesions in the ICo region reduced nest-cooing but did not specifically alter other courtship behavior of males tested with stimulus females. No effect on gonadal state could be disc
