Two groups of patients from the same era were retrospectively studied in Cincinnati and Chicago. In Cincinnati two subgroups were studied. The first group received myringotomy with insertion of a 'T'-shaped ventilating tube (75 patients, 140 ears, 147 insertions). Fifty-eight ears still had the tube in place, 31 had healed after spontaneous extrusion, 17 had healed after removal of the tube, 20 had persistent perforation after the tube was gone, 2 had a cholesteatoma, and 4 patients (7 ears) were lost to follow-up. Accepted treatment of secretory otitis media, in the presence of nasopharyngeal carcinoma treated with radiotherapy, includes the insertion of a grommet. This study analyses the incidence of otorrhoea following this treatment as compared to a control group. Results show that persistent otorrhoea occurred frequently (68%) in the study group and was refractory to antibiotic therapy. Alternative methods of management of the secretory otitis, such as a hearing aid, should be considered to avoid the complications of grommet insertion in this group of patients.

Office pediatric otology requires the management of some of the most common acute and chronic conditions of childhood. Some of these conditions include eustachian tube dysfunction, otorrhea, granulation polyps, extruded tubes, otitis media, trauma, otalgia, and sudden hearing loss. An approach that recognizes the concerns of the child and parent facilitates the examination and treatment with minimal discomfort.

This retrospective study looks at the incidence and nature of ear disease in 50 adolescent patients who had cleft palates repaired in infancy. Half of these patients had a history of grommet insertion. We found that most patients had normal hearing (81%) and middle-ear pressures (86%), although about half had tympanic membrane abnormalities.
