Adverse mental health outcomes and alcohol misuse among UK Armed Forces personnel: fourth phase of a 20-year cohort study of military personnel who served during the Iraq and Afghanistan conflicts.

OBJECTIVES: Twenty years since the start of UK Armed Forces participation in the Iraq and Afghanistan conflicts post-2001, the extent to which these deployments continue to impact mental health outcomes and alcohol misuse in UK military personnel is unknown. This is the reporting of the fourth phase, cross-sectional study of a longitudinal cohort study that has assessed the health and well-being of UK serving and ex-serving personnel since 2004. METHODS: Participants were eligible for the most recent phase (2022-2023) if they took part previously (2014-2016) and consented to recontact. Primar

Identification of a subgroup of post-9/11 veterans with unique combat experiences and worse clinical outcomes.

OBJECTIVES: Prior studies have examined the influence of military deployments and combat exposure on post-9/11 Veterans' health and well-being. Fewer studies, however, have focused on the extent to which specific combat experiences relate to clinical outcomes in this Veteran cohort. We sought to identify distinct groups of post-9/11 Veterans based on their combat experience profiles and examine the relationship between combat experiences and clinical outcomes. METHODS: We used data from 21,995 deployed, combat-exposed, Iraq/Afghanistan-era Veterans who enrolled in the VA Million Veteran Progr…

Predeployment neurocognitive functioning predicts postdeployment posttraumatic stress in Army personnel.

OBJECTIVE: The Fort Campbell Cohort study was designed to assess predeployment biological and behavioral markers and build predictive models to identify risk and resilience for posttraumatic stress disorder (PTSD) following deployment. This article addresses neurocognitive functioning variables as potential prospective predictors. METHOD: In a sample of 403 soldiers, we examined whether PTSD symptom severity (using the PTSD Checklist) as well as posttraumatic stress trajectories could be prospectively predicted by measures of executive functioning (using two web-based tasks from WebNeuro) ass

Contribution of the Mission in Afghanistan to the Burden of Past-Year Mental Disorders in Canadian Armed Forces Personnel, 2013.

OBJECTIVE: The purpose of this study was to estimate the contribution of the mission in Afghanistan to the burden of mental health problems in the Canadian Armed Forces (CAF). METHODS: Data were obtained from the 2013 Canadian Forces Mental Health Survey, which assessed mental disorders using the World Health Organization's Composite International Diagnostic Interview. The sample consisted of 6696 Regular Force (RegF) personnel, 3384 of whom had deployed in support of the mission. We estimated the association of past-year mental health problems with Afghanistan deployment status, adjusting fo

Chronic Nonallergic Rhinosinusitis Associated With Military Burn Pit Exposure.

Millions of previously deployed individuals nationwide have been affected by military toxic exposures and could be predisposed to a variety of associated health conditions. We present a case of a 38-year-old veteran with chronic rhinosinusitis with nasal polyps (CRSwNP) for two years and explore its possible association with burn pit exposure. He had a history of military burn pit exposure during his deployments to Iraq and Afghanistan, which spanned a total of 20 months. Physical examination revealed pale and enlarged inferior nasal turbinates. CT of the sinuses revealed pansinus mucosal thic

Dental care provision to UK military personnel serving on Operation Herrick in Afghanistan. Part 2: aetiology and management.

AIMS: To evaluate the nature of tooth-related morbidity in military personnel deployed on Operation Herrick and to compare the management of dental emergencies to that in non-deployed personnel. METHOD: Data were collected prospectively for UK Service personnel on Operation Herrick (OpH) during an 18-month period and measured against prospectively collected 'home base' (HB) control data. RESULTS: Molar teeth were responsible for 64.1% (2,089/3,259) of presentations on OpH and 69.5% (323/467) at HB, causing 81.4% (622/764) of cases of pain lasting for >60 minutes and/or waking the patient on

Outcomes Following Closed Axillary Nerve Injury: A Case Report and Review of the Literature.

We report a case of a 43-year-old male who sustained an axillary nerve injury secondary to a glenohumeral joint dislocation at a young age, and who has served over 20 years in the military with near normal shoulder function. In addition, we review the literature for the natural history of axillary nerve injury. A 43-year-old male sustained a left anterior glenohumeral dislocation in a motor vehicle accident as an 18-year-old. Following prompt manual reduction and subsequent physical therapy, the patient developed a permanent axillary nerve palsy. Despite the development of complete atrophy of

Outbreak of Cutaneous Leishmaniasis in Peruvian Military Personnel Undertaking Training Activities in the Amazon Basin, 2010.

Military personnel deployed to the Amazon Basin are at high risk for cutaneous leishmaniasis (CL). We responded to an outbreak among Peruvian Army personnel returning from short-term training in the Amazon, conducting active case detection, lesion sample collection, and risk factor assessment. The attack rate was 25% (76/303); the incubation period was 2-36 weeks (median = 8). Most cases had one lesion (66%), primarily ulcerative (49%), and in the legs (57%). Real-time polymerase chain reaction (PCR) identified Leishmania (Viannia) braziliensis (59/61 = 97%) and L. (V.) guyanensis (2/61 = 3%).

Eating disorder in a young active duty male.

Eating disorders can have atypical presentations, be challenging to diagnose, and often result in treatment delay, as illustrated here. Bulimia nervosa is characterized by binge eating and inappropriate compensatory behaviors, and is ten times more common in females. Studies show increased prevalence over the past decade, with similar prevalence in young military members and civilians. Risk factors include dieting, gender preference, life-altering events, and history of a psychiatric condition. Relatively little research has focused on eating disorders among military males, but factors unique

Nonpharmacological Therapies for Musculoskeletal Injury in Military Personnel: A Systematic Review/Meta-Analysis.

INTRODUCTION: Musculoskeletal (MSK) injury is an inherent risk for military personnel that can potentially impact job performance, productivity, and military readiness. Evidence is needed to show the efficacy of nonpharmacological, self-managed therapies to reduce MSK symptoms at common injury sites that are feasible for use during expeditionary operations and home stations. This systematic review and meta-analysis identified, summarized, and synthesized available evidence from randomized and non-randomized trials on the effectiveness of self-managed, home-use therapies to improve pain, muscle

Ethical Readiness: A Framework for Integrating Ethical Competence into Military Graduate Medical Education.

INTRODUCTION: Military medicine in armed conflict presents unique ethical challenges, especially under the resource constraints and extended evacuation timelines anticipated in future large-scale combat operations. Currently, there is no standardized military medical ethics (MME) curriculum within U.S. military Graduate Medical Education (GME) programs, leaving a significant portion of the medical force unprepared for high-intensity conflicts and at increased risk of moral injury, thereby compromising operational readiness. The purpose of this study was to characterize the current landscape of

Concordance Study of Diagnoses and Therapeutic Management for Military Personnel Evacuated for Medico-psychological Reasons: From the Theater of Operations to the Military Training Hospital.

INTRODUCTION: The evolving global landscape has led to increased involvement of the French armed forces, exposing military personnel to operational challenges that can affect their mental well-being. As a result, psychiatry has become the second most common reason for Medical Evacuation (MEDEVAC). In war zones where specialized medico-psychological consultations may not be readily available, medical officers play a vital role in providing initial care. Therefore, there is a growing emphasis on the precise evaluation of these practices. MATERIALS AND METHODS: In this retrospective observationa

An Analysis of the U.S. Department of Defense's Military Health Readiness Assessments.

Congress mandates that the Department of Defense (DoD) assess and monitor the health readiness of the armed forces. Accordingly, DoD implements a suite of health assessments to monitor service members' health readiness. One annual and four additional deployment-related health assessments screen for issues with physical and behavioral health at specified intervals throughout the deployment cycle to facilitate early intervention and any medical care required to maintain force readiness. The content of many of the items in these assessments overlap, and the required time frames for assessment com

Gender Differences in Machine Learning Models of Trauma and Suicidal Ideation in Veterans of the Iraq and Afghanistan Wars.

Suicide rates among recent veterans have led to interest in risk identification. Evidence of gender-and trauma-specific predictors of suicidal ideation necessitates the use of advanced computational methods capable of elucidating these important and complex associations. In this study, we used machine learning to examine gender-specific associations between predeployment and military factors, traumatic deployment experiences, and psychopathology and suicidal ideation (SI) in a national sample of veterans deployed during the Iraq and Afghanistan conflicts (n = 2,244). Classification, regression
