Economic Impact of Periodontitis: Global, Regional and National Estimates of Periodontal Expenditure, With Forecasts to 2050.

AIM: To estimate the global, regional and national economic impact of periodontitis in 2021 (including expenditure on prevention, diagnosis, treatment, maintenance and rehabilitation), and to forecast expenditures through 2050 under two utilisation scenarios: continuation of current treatment coverage trends and an illustrative expanded coverage scenario. METHODS: A cohort-based gross-costing Monte Carlo model was developed to estimate global, regional and national expenditure on periodontitis in 2021. Model estimates were validated against known national periodontal spending and aggregate de

Sample pooling approaches simulated under resource scarcity, lapses in testing capacity, and rapid processing demands for surveillance testing: a data-driven performance comparison.

BACKGROUND: Sample pooling is a critical strategy to meet increased testing demand and conserve resources in surveillance testing. Much of its effectiveness depends on how well optimized the pool size is to the prevalence of infection in the sampled population, which can be difficult to anticipate in many circumstances. Multiple methods exist to better optimize pooling, with unique trade-offs. METHODS: Pooling optimization methods were simulated to examine trade-offs between surveillance priorities and operational characteristics using SARS-CoV-2 surveillance data and workflows generated by th

Modelling Skin Pigmentation Using the Monte Carlo Technique: A Review.

The impact of skin pigmentation on the accuracy of optical biomedical devices has gained increased attention since the COVID-19 pandemic, particularly following evidence of oximetry measurement bias in dark-skinned individuals. Meanwhile, many computational models utilising the Monte Carlo (MC) technique have been developed as a cost-effective and scalable method for investigating these effects. Hence, this review explores the application of the MC technique in modelling skin pigmentation, focusing specifically on how melanin in the epidermis is represented across different studies. First, the

MLDT: an open-source multilayer diffusion theory Python implementation.

SIGNIFICANCE: Diffuse reflectance spectroscopy and imaging are powerful noninvasive tools for quantifying tissue biomarkers such as hemoglobin, melanin, and water. A theoretical model of light propagation is required to interpret these reflectance measurements and translate them into quantitative biomarker concentrations. Although Monte Carlo (MC) simulations offer the gold-standard in accuracy, their high computational burden renders them impractical for real-time iterative inverse solving. Conversely, standard analytical approximations can be efficient but often oversimplify tissue as a semi

Quantifying uncertainty in food waste across an urban area: The role of industrial, commercial, and institutional sectors.

Industrial, commercial, and institutional (ICI) food waste is a large, poorly quantified component of municipal solid waste. Landfill disposal of food waste results in greenhouse gas emissions and lost resources. Here, we apply a spatial-probabilistic modelling approach for estimating sector-specific ICI food waste generation and demonstrate this technique for Montreal, Canada. We identified 14,508 ICI establishments from municipal food inspection records and classified them into 14 subsectors. Point locations were matched to building footprints to acquire areal estimates for each establishmen

Modeling Individual-Level Uncertainty From Missing Data in Multifactorial Breast Cancer Risk Prediction.

PURPOSE: Multifactorial breast cancer (BC) risk prediction models use a range of predictors to estimate an individual's chance of developing BC. Data on risk factors are often incomplete, and point estimates calculated when data are missing can mask considerable uncertainty. Quantifying this uncertainty is critical for effective risk communication. METHODS: We used Monte Carlo simulation methods to estimate the distribution of 10-year BC risk for individuals with missing data, using the BOADICEA multifactorial model as an example. Multivariate imputation by chained equations with large repres

Towards transparent and interpretable screening: multi-biofluid FTIR spectroscopy with LLM-Augmented explainability for pancreatic cancer detection.

Early detection of pancreatic cancer remains a critical challenge in oncology, with current diagnostic methods often failing to identify the disease until advanced stages. However, diagnostic accuracy alone may be insufficient for clinical adoption as regulatory frameworks and clinical workflows increasingly demand transparent, interpretable AI systems. This study investigates Fourier Transform Infrared (FTIR) spectroscopy combined with machine learning for non-invasive pancreatic cancer detection using urine and blood biofluids, augmented by a language-model-assisted transparency framework to

Bayesian Random-Effects Meta-Analysis of Aggregate Data on Clinical Events.

To investigate intervention effects on rare events, meta-analysis techniques are commonly applied in order to assess the accumulated evidence. When it comes to adverse effects in clinical trials, these are often most adequately handled using survival methods. A common-effect model that is able to process data in commonly quoted formats in terms of hazard ratios has previously been proposed for this purpose. In order to accommodate potential heterogeneity between studies, we have extended the model by Holzhauer to a random-effects approach. The Bayesian model is described in detail, and applica

Simulation and inference methods for non-Markovian stochastic reaction networks.

Stochastic models of reaction networks are widely used to capture intrinsic noise in complex systems in the life sciences. Typical formulations of these models are based on Markov processes for which there is extensive research on efficient simulation and inference. However, there are complex processes in biology, such as gene transcription and translation, that introduce history dependent dynamics requiring non-Markovian processes to accurately capture the stochastic dynamics of the system. This greater realism comes with additional computational challenges for simulation and parameter infere

Monte Carlo assessment of a treatment planning system for intraoperative radiotherapy in the presence of tissue heterogeneities.

BACKGROUND: Radiance is a Monte Carlo-based treatment planning software integrated into the Intrabeam system for 50-kVp X-rays intraoperative radiotherapy (IORT). PURPOSE: This study evaluates Radiance accuracy in calculating absorbed dose in IORT of the breast including the effect of tissue heterogeneities. METHODS: Dose distributions from IORT were calculated in 88 virtual phantoms of different tissue-equivalent materials. Dose was calculated independently using the penEasy code for the PENELOPE Monte Carlo engine and compared to Radiance. Depth dose curves (DDCs) were compared through gam

The Role of Dendritic Spines in Water Exchange Measurements With Diffusion MRI: Time-Dependent Single Diffusion Encoding MRI.

Time-dependent diffusion MRI (dMRI) using single diffusion encoding (SDE) is sensitive to water dynamics in biological tissues, yet interpreting its signals requires careful consideration of underlying microstructure. While prior work has focused on restricted/hindered diffusion and membrane permeation, additional exchange mechanisms such as diffusion-mediated exchange between dendritic shaft and spines in gray matter (GM) remain understudied. Here, we hypothesize that water diffusion within impermeable spiny dendrites can produce time-dependent SDE signals indistinguishable from those arising

Monte Carlo modeling of the Elekta Synergy linear accelerator platform with Agility MLC using TOPASMC for secondary dose calculation.

Monte Carlo simulations offer the highest accuracy for calculating dose distributions in radiation therapy (RT). Precise modeling of the Elekta Synergy linear accelerator (LINAC) with Agility multi-leaf collimator (MLC) is essential for accurate dose verification. The 6 MV flattened (FF) and FF-free (FFF) Elekta Synergy LINAC with Agility MLC was modeled using TOPASMC, providing an additional verification tool for patient dose distributions. An initial model was developed based on vendor specifications. The 160-leaf MLC was constructed from a computer-aided design mesh of a single leaf. Propri

Bayesian Model Averaging of Parametric Coalescent Models for Phylodynamic Inference.

Bayesian phylodynamic models have become essential for reconstructing population history from genetic data, yet their accuracy depends crucially on choosing appropriate demographic models. To address uncertainty in model choice, we introduce a Bayesian model averaging (BMA) framework that integrates multiple parametric coalescent models-including constant, exponential, logistic, and Gompertz growth-along with their "expansion" variants that account for non-zero ancestral populations. Implemented in a Bayesian setting with Metropolis-coupled Markov chain Monte Carlo, this approach allows the sa

Entry skin dose reduction in an inline MRI-linac using an electron contamination deflector coupled with a helium volume.

BACKGROUND: A unique feature of inline MRI-linac prototype systems is the changes to entry surface or skin doses. For the high field (1 T) Australian MRI-linac prototype, a strong surface dose increase is observed. This is a by-product of secondary electron contamination being focused down the MRI bore due to the magnetic field being parallel with the x-ray beam direction. It has been anticipated that future clinical treatments will utilize a 2 cm thick water equivalent bolus layer in air above the patient's skin to absorb the electron contamination that causes the large dose hot spot. However

Multi-objective optimization formulation for Alzheimer's disease trial patient selection.

OBJECTIVE: Clinical trial recruitment faces critical challenges with screen failure rates exceeding 80% in Alzheimer's disease (AD) trials. Traditional patient selection relies on expert consensus without systematic evaluation of trade-offs between statistical power, recruitment feasibility, safety, and cost. We developed a multi-objective optimization framework to systematically identify optimal eligibility criteria configurations that balance competing objectives in AD clinical trial design. METHODS: We implemented the Non-dominated Sorting Genetic Algorithm III (NSGA-III) to optimize patie

Neutron capture-enhanced proton beam therapy outside the irradiation field: in vitro experiments.

OBJECTIVE: To evaluate the cell-killing effects of neutron capture-enhanced particle therapy (NCEPT) outside the proton beam irradiation (PBI) field. RESULTS: Human osteosarcoma cells (MG63) were divided into control and NCEPT groups. Four hours before irradiation, the NCEPT group was exposed to 10B-boronophenylalanine. PBI was performed by targeting a gel bolus, with tissue culture plates placed outside the PBI field adjacent to the gel bolus. The bolus was irradiated with 12, 36, or 72 Gy. Cell survival fractions were calculated for wells adjacent to the bolus. Furthermore, the neutron flue

Electron dose optimisation based on tumour thickness and shape for photon multi-leaf collimated megavoltage electrons.

This study developed an optimisation method that considers tumour thickness for modulated electron radiation treatment (MERT) at a treatment distance of 60 cm. It comprises forming a tumour bed matrix from which a transformed tumour bed matrix is derived. From the discrete tumour bed depth data, electron beam segments were extracted, which were further decomposed into sub-beams. The EGSnrc-based Monte Carlo codes BEAMnrc and DOSXYZnrc were used to model a linear accelerator and to score 3-D dose data for various field sizes (sub-beams) in a water phantom model. The use of different energy and

Computationally efficient collimator-detector response compensation in high energy SPECT using 1D convolutions and rotations.

Objective. Modeling of the collimator-detector response (CDR) in single photon emission computed tomography (SPECT) reconstruction enables improved resolution and accuracy, and is thus important for quantitative imaging applications such as dosimetry. The implementation of CDR modeling, however, can become a computational bottleneck when there are substantial components of septal penetration and scatter in the acquired data, since a direct convolution-based approach requires large 2D kernels. This work proposes a 1D convolution and rotation-based CDR model that reduces reconstruction times but

Comparison of methods for tuning machine learning model hyper-parameters: with application to predicting high-need high-cost health care users.

BACKGROUND: Supervised machine learning is increasingly being used to estimate clinical predictive models. Several supervised machine learning models involve hyper-parameters, whose values must be judiciously specified to ensure adequate predictive performance. OBJECTIVE: To compare several (nine) hyper-parameter optimization (HPO) methods, for tuning the hyper-parameters of an extreme gradient boosting model, with application to predicting high-need high-cost health care users. METHODS: Extreme gradient boosting models were estimated using a randomly sampled training dataset. Models were se
