Opisthotonus: Revisiting a classic movement disorder.

Opisthotonus is characterized by backward arching of the trunk, associated with retrocollis, caused by involuntary contractions of extensor paraspinal muscles. Classical etiologies include tetanus, strychnine intoxication, dystonia (particularly tardive dystonia and neurodegeneration with brain iron accumulation), seizures, and functional movement disorders (MDs). Inborn errors of metabolism, epileptic encephalopathies and other gene mutations should be considered in newborns and infants presenting with opisthotonus. We surveyed 1216 consecutive patients presenting for evaluation in a tertiary…

Classifying neurogenic dysphagia as a movement disorder.

Swallowing is a complex sensorimotor task critical for maintaining nutrition, hydration and quality of life. Given the widespread neural inv … ation. In this review, we explore neurogenic dysphagia through classical sensorimotor hierarchy and movement disorders terminology. We provide comprehensive evidence of dysphagia signs and symptoms in pure cases of movement disorders and discuss nuances related to assessing signs of neuropathology responsible for disordered oropharyngeal swallowing biomechanics. App …

Multidisciplinary Treatment for Functional Movement Disorder.

Functional movement disorders such as tremor, dystonia, jerky movement, limb weakness, and gait abnormalities are a common clinical concern for neurologists. This article is a practical guide for neurologists to manage these conditions. The authors describe how the principle of "rule-in" diagnosis, which involves demonstrating the difference between voluntary and automatic movement, can be carried through to explanation, triage, and evidence-based multidisciplinary rehabilitation therapy. New predictive processing models of functional movement disorder can be combined with an understanding of …

[The yips: a movement disorder among golfers].

BACKGROUND: Although the yips occurs commonly in golfers, this neurological movement disorder is a relatively unknown phenomenon among physicians. CASE DESCRIPTION: A 47-year-old golf player developed a disturbing, neurological movement disorder that manifested when playing golf: the yips. These jerks occurred when putting, but not during practice rounds. The patient's father suffered from similar yips. CONCLUSION: The yips is a task-specific dystonia, which can also manifest in players of other sports. Multiple factors, such as genetic predisposition and anxiety seem to be involved in its p

The History of Movement Disorder Brain Surgery.

The first surgical procedures for abnormal movement disorders began in the 1930s, when surgeons first proposed ablative techniques of the caudate nucleus or transection of motor (pyramidal) pathways to reduce involuntary movements in patients with Parkinson's related tremor. During the 50-year interval between 1945 and 1995, the development of precise intracranial guiding devices, brain maps, and advanced imaging led to the refinement of appropriate deep brain targets affecting extrapyramidal pathways. Lesional surgery and subsequent neuroaugmentation using deep brain stimulation extended the …

Stereotyped movement disorder in ICD-11.

According to current proposals for ICD-11, stereotyped movement disorder will be classified in the grouping of neurodevelopmental disorders, with a qualifier to indicate whether self-injury is present, similar to the classification of stereotypic movement disorder in DSM-5. At the same time, the WHO ICD-11 Working Group on the Classification of Obsessive-Compulsive and Related Disorders has proposed a grouping of body-focused repetitive behavior disorders within the obsessive-compulsive and related disorders (OCRD) cluster to include trichotillomania and skin-picking disorder. DSM-5 has taken

The management of psychosis in movement disorder patients.

Psychosis may be seen with several movement disorders. As pharmacological treatments can sometimes worsen movement disorders, psychosis in these situations can be complex for clinicians to manage. This review covers the management of psychosis in three different movement disorders: Parkinson's disease, dementia with Lewy bodies and Huntington's disease.

Periodic limb movement disorder in children.

Periodic limb movement disorder (PLMD) has been reported in children and is more prevalent in certain conditions such as attention deficit hyperactivity disorder. The International Restless Legs Syndrome Group has recently developed the criteria for diagnosis of PLMD in children. These criteria include both specific clinical features and polysomnographic findings. Exclusion of other causes especially sleep-disordered breathing is essential. Several medications including benzodiazepine and dopaminergic medication have been used in children, although these medications have not been adequately st

Eye movements in a familial vestibulocerebellar disorder.

Eye movement abnormalities consisting of poor or absent smooth pursuit and vestibulo-ocular reflex suppression, gaze-paretic and rebound nystagmus, slow build-up of optokinetic nystagmus, mildly hyperactive vestibulo-ocular reflex, and a high incidence of strabismus were inherited in an autosomal dominant fashion in 10 members of a non-consanguineous English caucasian family. The onset was in early childhood, but was not congenital. In 7 cases there was no tremor, dizziness, consistent ataxia, or other cerebellar signs that are often associated with these ocular motor deficits, and apart from

The movement disorder of reflex sympathetic dystrophy.

We present 43 patients with reflex sympathetic dystrophy (RSD) who manifested abnormalities of movement. The patients have focal dystonia, weakness, spasms, tremor, difficulty initiating movement, and increased tone and reflexes. These motor signs and symptoms may precede other manifestations of the illness by weeks or months. They most frequently, but not invariably, occur concomitantly with sudomotor or vasomotor changes and pain. Lioresal is effective in reducing spasms. Early in the course of RSD, the motor manifestation may be alleviated by intense sympathetic blockade or sympathectomy. I

Deutetrabenazine in Hyperkinetic Movement Disorders: VMAT2 Modulation From Choreiform Disorders to Hyperkinetic Dystonic Syndromes.

Hyperkinetic movement disorders, including dystonia and choreiform syndromes, are pathophysiologically linked to dysregulated dopamine metabolism within the basal ganglia-thalamo-cortical (BGTC) circuitry. Mounting research highlights dopaminergic hyperactivity as a driver of aberrant signaling and motor symptoms. Vesicular monoamine transporters (VMATs) package monoamines into synaptic vesicles, yet they remain pharmacologically underexplored compared to plasma membrane transporters. Recent advances in VMAT2 inhibitors offer a promising therapeutic avenue for hyperkinetic movement disorders. …

Sleep-related rhythmic movements and rhythmic movement disorder beyond early childhood.

INTRODUCTION: Sleep-related rhythmic movements (SRRMs) are common in young children and become less prevalent with increasing age. When SRRMs significantly interfere with sleep and/or affect daytime functioning, potentially resulting in injury, rhythmic movement disorder (SRRMD) is diagnosed. OBJECTIVE: The aim of our study was to assess clinical comorbidities, types of SRRMs, sleep stage/wakefulness distribution during night, and age-dependence of these parameters. MATERIAL AND METHODS: In sum, 45 patients (age range 1-26 years, mean age 10.56 ± 6.4 years, 29 men) were clinically examined f

Smooth pursuit eye movements and saccadic eye movements in patients with delusional disorder.

OBJECTIVE: This study used eye movement tests to examine whether frontal lobe dysfunction is present in delusional disorder. METHOD: Smooth pursuit and voluntary saccadic eye movements of 15 delusional patients, 40 schizophrenic patients, and 40 normal subjects were recorded and analyzed statistically. RESULTS: The schizophrenic patients differed significantly from the normal subjects in some smooth pursuit eye movement characteristics, whereas both the schizophrenic and the delusional patients showed more saccades than the normal subjects during the smooth pursuit test. The delusional patie

Dystonic Movement Disorder as Symptom of Catatonia in Autism Spectrum Disorder.

BACKGROUND: Catatonia is increasingly recognized as a comorbid psychiatric condition in autism spectrum disorder (ASD), but the overlap of behavioral characteristics between these disorders raises many diagnostic challenges. Moreover, recognizing symptoms in ASD patients with medium-low functioning might be difficult. Literature on this argument is poor, especially for children. Case presentation. We report the case of an ASD patient with low cognitive functioning, who presented a complex symptomatology, characterized by progressive regression with loss of autonomy and involuntary movements th

Misinterpretation of sleep-breathing disorder by periodic limb movement disorder.

We report a case of misinterpretation of sleep-disordered breathing due to periodic limb movement disorder. A 67-year-old man was diagnosed with sleep-disordered breathing and subsequently placed on treatment with nasal continuous positive airway pressure (CPAP). The initial diagnostic evaluation did not include measurement of anterior tibialis electromyogram. The respiratory disturbance index of the initial evaluation was 23. After a brief period of nasal CPAP use, the patient discontinued the treatment because no significant change in daytime alertness was noted and signs of CPAP-related ins
