PPE37 Is Essential for Mycobacterium tuberculosis Heme-Iron Acquisition (HIA), and a Defective PPE37 in Mycobacterium bovis BCG Prevents HIA.

Mycobacterium tuberculosis, one of the world's leading causes of death, must acquire nutrients, such as iron, from the host to multiply and cause disease. Iron is an essential metal and M. tuberculosis possesses two different systems to acquire iron from its environment: siderophore-mediated iron acquisition (SMIA) and heme-iron acquisition (HIA), involving uptake and degradation of heme to release ferrous iron. We have discovered that Mycobacterium bovis BCG, the tuberculosis vaccine strain, is severely deficient in HIA, and we exploited this phenotypic difference between BCG and M. tuberculo…

Causes of and clinical sequelae of delays in reporting species identification of mycobacteria other than Mycobacterium tuberculosis and Mycobacterium avium complex.

We tracked cultures submitted to a laboratory in 2004 in which mycobacteria other than Mycobacterium tuberculosis or Mycobacterium avium complex were grown (n = 11). We reviewed clinical information, other mycobacterial cultures, and radiologic findings. We measured the time from specimen submission to first growth and to final species identification. We assessed the response to this information based on notes, laboratory tests, consultations, and radiology studies. The median time to identification was 68 days (47-292 days). In 5 cases, the clinician failed to retrieve the data, and in 3 of t…

Enhancing the protective efficacy of Mycobacterium bovis BCG vaccination against tuberculosis by boosting with the Mycobacterium tuberculosis major secretory protein.

Tuberculosis continues to ravage humanity, killing 2 million people yearly. Most cases occur in areas of the world to which the disease is endemic, where almost everyone is vaccinated early in life with Mycobacterium bovis BCG, the currently available vaccine against tuberculosis. Thus, while more-potent vaccines are needed to replace BCG, new vaccines are also needed to boost the immune protection of the 4 billion people already vaccinated with BCG. Until now, no booster vaccine has been shown capable of significantly enhancing the level of protective immunity induced by BCG in the stringent …

Serological response in leprosy and tuberculosis patients to the 18-kDa antigen of Mycobacterium leprae and antigen 85B of Mycobacterium bovis BCG.

Two enzyme-linked immunosorbent assays (ELISAs) for the detection of leprosy antibodies were developed. These assays were based on the recombinant 18-kDa protein of Mycobacterium leprae and the 85B antigen of M. bovis. Sera from leprosy and tuberculosis patients were analyzed, and a phenolic glycolipid-I (PGL-I) ELISA was used as a reference test. The 18-kDa ELISA reached a sensitivity of 70% on lepromatous leprosy sera. The corresponding results for the 85B and the PGL-I ELISAs were 93% and 96%, respectively. The sensitivity on tuberculoid leprosy sera was 72% for the 18-kDa antigen, 95% for …

Genetic diversity of clinical Mycobacterium avium subsp. hominissuis and Mycobacterium intracellulare isolates causing pulmonary diseases recovered from different geographical regions.

Mycobacterium avium complex (MAC) infections are increasing annually in many countries. MAC strains are the most common nontuberculous mycobacterial pathogens isolated from respiratory samples and predominantly consist of two species, Mycobacterium avium and Mycobacterium intracellulare. The aim of this study was to analyze the molecular epidemiology and genetic backgrounds of clinical MAC isolates collected from The Netherlands, Germany, United States, Korea and Japan. Variable numbers of tandem repeats (VNTR) analysis was used to examine the genetic relatedness of clinical isolates of M. avi…

Mycobacterium chelonae-Mycobacterium abscessus complex clear corneal wound infection with recurrent hypopyon and perforation after phacoemulsification and intraocular lens implantation.

We report a clear corneal wound infection occurring in a 74-year-old man caused by a member of the Mycobacterium chelonae-Mycobacterium abscessus complex, presenting as crystalline keratopathy with recurrent hypopyon. This led to perforation after phacoemulsification with posterior chamber intraocular lens implantation. Only after corneal biopsy of the incision was the causative organism isolated and found to be sensitive to clarithromycin and ciprofloxacin. Despite aggressive therapy, a full-thickness corneal perforation developed, requiring emergent cyanoacrylate glue to preserve ocular inte…

Polymerase chain reaction detection of Mycobacterium tuberculosis complex and Mycobacterium avium organisms in formalin-fixed tissues from culture-negative ruminants.

In the US eradication program for bovine tuberculosis, a definitive diagnosis depends on the isolation of Mycobacterium bovis. However, in some cases bacterial culture is unsuccessful, even though the tissue is considered suspicious by histopathology because granulomatous lesions and acid-fast organisms are present. The purpose of this study was to determine if polymerase chain reaction (PCR) tests on formalin-fixed tissue would successfully identify the organisms observed in suspect lesions from culture-negative animals. Diagnostic laboratory records were used to select paraffin blocks of tis…

Nontuberculous mycobacterium seroprevalence in Japanese patients with RA.

OBJECTIVE: Although newly developed immunosuppressive therapies have markedly enhanced disease control in patients with RA, they are associated with an increased risk of various infections, including nontuberculous mycobacterial (NTM) disease and tuberculosis (TB). The anti-glycopeptidolipid (GPL) antibody (Ab) test is used to screen for NTM disease, and the interferon-gamma release assay (IGRA) is used to screen for TB. However, detailed analyses of positive anti-GPL Ab test results from RA patients remain scarce. Therefore, this study examined anti-GPL Ab testing and IGRA positivity in RA pa

Fred Goldberg machines in the pathogenesis of Mycobacterium tuberculosis.

Regulated proteolysis is found in all forms of life, from bacteria to humans, and plays essential roles in all areas of physiology. The bacterial pathogen Mycobacterium tuberculosis has three, chambered ATP-dependent proteases needed to cause lethal infections in animals, making them attractive targets for drug development. Here, I review the latest developments in what is known about how caseinolytic proteases and proteasomes function to contribute to the virulence of one of the world's deadliest pathogens.

Transmission of Mycobacterium tuberculosis in four prisons in Colombia.

Our study aimed to describe the transmission dynamics and genotypic diversity of Mycobacterium tuberculosis in people deprived of liberty (PDL) in four Colombian prisons. Our cohort study included 64 PDL with bacteriologically confirmed pulmonary tuberculosis diagnosed in four Colombian prisons. The 132 isolates were genotyped using 24-mycobacterial interspersed repeated units-variable number tandem repeats (MIRUs-VNTR). A cluster was defined when ≥2 isolates from different PDL had the same genotype. Tuberculosis acquired in prison was considered when ≥2 persons were within the same cluster an…

A soft tissue lesion caused by Mycobacterium malmoense.

We report a rare case of a soft tissue infection caused by Mycobacterium malmoense in an 88-year-old woman, a former farmer. The patient presented with a non-healing lesion on her right forearm, which had been increasing in size over 2 months. Ultrasound and magnetic resonance imaging (MRI) showed subcutaneous nodular lesions, and the patient was referred to the multidisciplinary soft tissue tumour meeting at our institution. A biopsy was performed, and the histopathological examination revealed granulomatous inflammation with abundant Ziehl-Neelsen-positive microorganisms, indicative of a myc…

Mycobacterium tuberculosis biology, pathogenicity and interaction with the host.

Since the release of the first Mycobacterium tuberculosis genome in 1998, major advances have been made in understanding the biology of this pathogen, the leading infectious cause of death in modern human history. In this Review, we outline the physiological and metabolic features thought to underpin the survival, evasion and subversion strategies employed by M. tuberculosis as it drives a cycle of transmission, infection and disease in its obligate human host. We also consider adaptations to key host innate immune effectors, including the roles of granulocytes, phagosomal damage and repair, a…

Metabolic rewiring of isoniazid sensitivity in Mycobacterium tuberculosis.

Isoniazid (INH) inhibits mycolic acid synthesis in Mycobacterium tuberculosis (Mtb) and is a cornerstone of treatment regimens against this deadly pathogen. However, over 10% of Mtb infections are INH-resistant. The compound C10 can sensitize clinically relevant INH-resistant mutants to killing by INH. Thus, understanding the mechanism of action for C10 could aid in designing new strategies for circumventing drug resistance. We find that C10 treatment reroutes carbon flux toward valine, drawing carbon away from gluconeogenesis and the TCA cycle. As a result, C10 decreases cell envelope capsule…

On macrophage response to primary Mycobacterium tuberculosis in humans.

Tuberculosis stands as the leading cause of death worldwide, driven by infection from a single bacterial agent, and has been recognized as a global public health concern by the World Health Organization. Recent studies highlight that the innate immune response has a central role in controlling the initial spread of Mycobacterium tuberculosis (Mtb) within the host, and triggers adaptive immune response. We developed and analyzed a model examining the interactions among macrophages, innate cells, and Mtb to determine whether the infection is controlled by the innate immune response or whether a …

The role of TRIM proteins in the pathogenesis of mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb) is an intracellular pathogen responsible for tuberculosis (TB), a disease that remains the leading infectious cause of death worldwide. Several host cell proteins have been reported to play a significant role in regulating Mtb infection and replication. Among these, TRIM proteins are a large family of E3 ubiquitin ligases that have been described as being involved in host defence against Mtb, by controlling two important cellular processes: autophagy and the inflammatory response. Autophagy is a highly conserved cellular process deputed to the degradation of da…

A Novel, Nonaquatic Zoonotic Transmission of Mycobacterium marinum.

Introduction: Mycobacterium marinum was first described in humans in 1954, known to infect fish species and contaminate water and fish products. Inoculation to humans occurs through injured skin resulting in the formation of a solitary nodule known as "fish tank granuloma." Disseminated infections have been reported in the immunocompromised and can present with extracutaneous manifestations such as arthritis and osteomyelitis. Nonaquatic transmission has not been previously reported. Case Presentation: A 63-year-old lady with rheumatoid arthritis and bronchiectasis was referred to dermatology …
