Knowledge of the anatomy and function of the nail apparatus is essential when performing the physical examination. Inspection may reveal localized nail abnormalities that should be treated, or may provide clues to an underlying systemic disease that requires further workup. Excessive keratinaceous material under the nail bed in a distal and lateral distribution should prompt an evaluation for onychomycosis. Onychomycosis may be diagnosed through potassium hydroxide examination of scrapings. If potassium hydroxide testing is negative for the condition, a nail culture or nail plate biopsy should

A man, 58 years of age, presented with a 4 year history of painful lesions of his nails. His previous history included hypertension, diabetes mellitus and hyperlipidaemia. These were treated with enalapril, metformin and simvastatin respectively. He also had asymptomatic skin lesions for over 15 years that had worsened in the past 4 years. His father had similar nail lesions that had been diagnosed as onychomycosis.

Yellow nail syndrome is an acquired condition of unknown etiology, rarely seen in children, characterized by a triad of thickened yellow nails, primary lymphedema, and respiratory manifestations. We report an 8-year-old girl with this syndrome who showed improvement with Fluconazole, 200 mg once weekly and vitamin E, 1000 IU once daily.

A 45 year old male presented to a primary care physician with complaints of long term, bilateral knee and bilateral foot pain, without history of significant trauma. Physical exam demonstrated full range of motion without any erythema or edema. Fingernails were abnormal and hypoplastic. Bilateral knee radiographs demonstrated moderate degenerative changes of the right knee. Small, atrophic patellae were noted to be proximal to their expected location. Two months later the patient returned to his primary care physician for worsening of the knee pain related to cooler outside temperatures. An NS…

The yellow nail syndrome is a rare disease. The diagnosis is clinically based on three features: yellow discolouration on the nails, chronic lymphedema, and pleural effusions. The treatment of the yellow nails is controversial. Spontaneous healing is reported in 30% of the cases. Vitamin E can otherwise be tried. The syndrome is a benign disease but the literature describes an association to malign and autoimmune diseases. It is therefore important to investigate the patients with this syndrome. The present case story shows the development of yellow nail syndrome in a 73 year-old man.

Subungual malignant melanoma is a rare form of malignancy that can present at an advanced stage. We describe a case that was diagnosed after a presentation to the emergency department for a traumatic injury of the affected area. Initial presentations of malignant disease and its complications form a relatively low proportion of the caseload of emergency doctors. In this case, a patient presented after a minor injury that had failed to recover; subsequent investigation of this injury led to the diagnosis of subungual malignant melanoma in an otherwise asymptomatic patient. This is a rare case w

The diagnosis of pediatric nail populations is complicated because a large proportion of pediatric nail presentations involve parents bringing in their children with questions regarding nail appearance rather than the plain presentation of a disease or functional problem. Parents are concerned whether the nails are normal, if they are uncomfortable for the baby or child, if the nails will affect walking or other future function, or if the changes are of wider significance and suggest more sinister medical problems than are yet apparent. To address these concerns, the clinician needs a good gra…

The misdiagnosis of onychomycosis delays the proper treatment of the nail condition. Nail histomycology refers to the microscopic examination of histologic sections prepared from nail clippings for the purpose of detecting the presence of fungi. This laboratory method is the most sensitive procedure for the diagnosis of onychomycosis. It allows for the visualization of the precise location of the invasive fungus inside the nail apparatus. The method also distinguishes various shapes of the fungal cells, in particular filamentous fungi, yeasts, conidia, and sporodochia. The correct diagnosis pr

Common skin diseases often present with nail changes. This article reviews the nail manifestations of psoriasis, lichen planus, alopecia areata, and twenty-nail dystrophy. There is considerable overlap in the nail findings in these cutaneous conditions, but nail changes distinct to each do exist.

Nail pigmentation is defined by the presence of melanin in the nail plate. It most frequently has the appearance of a longitudinal pigmented band, called longitudinal mel-anonychia (LM). LM is an important aspect of ungual pathology because it can be the first sign of nail apparatus melanoma. The aim of this article is to help distinguish those types of melanonychia that are worrisome and should lead to biopsy from those that are reasonably reassuring and need only be followed clinically. Histology, differential diagnosis, and biopsy techniques also are considered.

Hair and nails are often stated to have much in common in relation to their origin, anatomical structures, and common involvement in many diseases. Hair and nails are predominantly epithelial structures derived from primitive epidermis and made up of keratinous fibrils embedded in a sulfur-rich matrix. It was first noted early in the 20th century that the nail unit was comparable in several respects to a hair follicle sectioned longitudinally and laid on on its side. The epithelial components of hair follicle and nail apparatus are differentiated epidermal structures that may be involved joint…
