THERAPY OF ENDOCRINE DISEASE: Central neck dissection: a step forward in the treatment of papillary thyroid cancer.

Since its introduction in the '70s and '80s, CND for papillary cancer is here to stay. Compartment VI should always be explored during surgery for papillary thyroid carcinoma (PTC) for obvious lymph node metastases. These can be easily spotted by an experienced surgeon or, eventually, by frozen section. No doubt, obvious nodal disease in the Delphian, paratracheal and subithsmic areas should be dissected in a comprehensive manner (therapeutic central neck dissection), avoiding the selective removal of suspicious nodes. Available evidence for routine prophylactic CND is not completely satisfact…

First report of elective selective neck dissection in the management of squamous cell carcinoma of the maxillary sinus.

Controversy remains about management of the neck in squamous cell carcinoma (SCC) of the maxillary sinus and we know of no reports of the use of elective selective neck dissection for management in this site. We retrospectively reviewed 18 consecutive patients with SCC of the maxillary sinus who were managed by primary operation with curative intent. A total of 13 patients had an elective selective neck dissection, which was invaded in one case 8%. Four patients had regional metastases, two with positive nodal disease confirmed after elective selective neck dissection, and two who developed re…

Prognostic significance of the number of lymph nodes in elective neck dissection for tongue and mouth floor cancers.

UNLABELLED: The presence of metastatic lymph nodes is a relevant aspect in the treatment of head and neck cancer, bringing about a 50% reduction in survival. OBJECTIVE: To assess the number of lymph nodes removed in the neck dissection and their relationship with the prognosis. METHODS: A retrospective study involving 143 patients with tongue and mouth floor epidermoid carcinoma, which histological exam showed no lymph node metastases. Among those, 119 were males and 24 females, with mean age of 54 years. As to the primary tumor site, 65 were in the tongue and 78 in the mouth floor. T stage …

Feasibility of preservation of the submandibular gland during neck dissection in patients with early-stage oral cancer.

BACKGROUND: The impact of submandibular gland (SMG) preservation during neck dissection on the survival of patients with early-stage oral squamous-cell carcinoma (OSCC) remains undocumented. METHODS: The medical records of all patients with early-stage OSCC (stage I and II) who underwent wide excision of the primary tumor and simultaneous neck dissection between 1999 and 2006 at our facility were retrospectively reviewed. RESULTS: We analyzed 408 patients, including 33 patients with and 375 patients without SMG preservation. The 5-year disease-free and overall survival rates were 78.8% and 9…

Discontinuous vs. in-continuity neck dissection in carcinoma of the oral cavity. Experience of two oncologic hospitals.

Many Authors have discussed the best indication and extension of neck dissection, but few have studied the surgical approach considering the continuity of neck dissection with the primary tumour. This retrospective study refers to patients submitted to major surgery between 1996 and 2001 for floor of mouth and oral tongue squamous cell carcinoma, at the Head and Neck Surgery Department of the Hospital "A.C. Camargo", São Paulo, Brazil and of the European Institute of Oncology, Milan, Italy. Patients were assigned to one of three groups: group I (in-continuity resection); group 2 (discontinuous…

Dissection from fundus to neck for ruptured anterior and middle cerebral artery aneurysms at the acute surgery.

It is generally believed that a ruptured aneurysm should be dissected from its neck to its fundus or that only the neck should be dissected. This study was conducted to clarify whether, during the acute stage, intra-operative bleeding occurs at the same site as the initial rupture point when aneurysms are dissected completely without clipping. The subjects were 170 patients with ruptured anterior or middle cerebral artery aneurysms who were surgically treated by day 7. These aneurysms were operated on through an interhemispheric or a pterional route, respectively. Most of the aneurysms were di

Minimally invasive selective neck dissection: a prospective study of endoscopically assisted dissection via a small submandibular approach in cT(1-2_N(0) oral squamous cell carcinoma.

BACKGROUND: Selective neck dissection (SND) in clinical N0 (cN0) cases of oral squamous cell carcinoma (SCC) has been performed by surgeons using a retroauricular or modified facelift approach with robotic or endoscopic assistance. However, these procedures provide cosmetic satisfaction at the cost of possible maximal invasiveness. In this prospective study, we introduced and evaluated the feasibility as well as surgical invasiveness and cosmetic outcome of endoscopically-assisted SND via a small submandibular approach. METHODS: Forty-four patients with cT1-2N0 oral SCC (OSCC) were randomly d…

Selective versus comprehensive neck dissection in the treatment of patients with a pathologically node-positive neck with or without microscopic extracapsular spread in oral squamous cell carcinoma.

The objective of this study was to compare the prognosis and complications between selective neck dissection (SND) and comprehensive neck dissection (CND) for patients with a pathologically node-positive neck in squamous cell carcinoma of the tongue and the floor of the mouth. This was a retrospective cohort study. There was no significant difference between the SND group and the CND group in 3-year neck control rate (86.2% vs. 85.9%, P=0.797) or disease-specific survival (DSS) rate (64.6% vs. 61.9%, P=0.646). Further analyses of the respective 3-year DSS rates in the SND and CND subgroups wer…

Matched-pair analysis of the outcome of two different neck dissection techniques for oral squamous cell carcinoma.

BACKGROUND: This study aimed to compare two different neck dissection (ND) techniques, conventional ND (CND) and functional ND (FND), in terms of lymph node yield (LNY) and regional recurrence (RR). FND is a concept based on dissection along the cervical fascial planes with en bloc removal of cervical lymph node levels (LNLs) while preserving the neurovascular and muscular structures, as described by Osvaldo Suárez. In contrast, CND involves the separate division of LNLs in situ without targeting the fascial planes. METHODS: This retrospective cohort study included previously untreated patien…

Clinical outcomes of bilobed platysma myocutaneous flap technique with neck dissection in lower lip squamous cell carcinoma.

BACKGROUND: Squamous cell carcinoma (SCC) in the lower lip is among the most frequently encountered types of cancer. As the size of this cancer increases, the need for reconstruction becomes evident, posing a significant challenge. Depending on the staging results in these patients, neck dissection may be recommended. OBJECTIVE: This study aims to demonstrate the successful application or feasibility of our Bilobed Platysma Myocutaneous Flap technique in patients with lower lip SCC. METHODS: The Bilobed Platysma Myocutaneous Flap was applied to all patients. Following TNM staging, supraomohy…

The role of prophylactic central compartment neck dissection in patients with T1-T2 cN0 papillary thyroid carcinoma.

BACKGROUND: It remains controversial whether prophylactic central compartment neck dissection (pCCND) is necessary in cases of stage T1-T2 cN0 papillary thyroid carcinoma (PTC). Some studies have demonstrated the benefits of pCCND on oncologic outcomes, whereas others reported that any advantages were insignificant. The purpose of this study was to investigate the effects of pCCND on cancer recurrence and its pattern in patients with T1-T2 cN0 PTC. METHODS: We retrospectively reviewed 2,902 patients with PTC who had undergone thyroidectomy between 2006 and 2012 at Seoul St. Mary's Hospital: 2…

Novel application of microdissection tungsten needle in total thyroidectomy with central neck dissection for papillary thyroid carcinoma.

BACKGROUND: Energy-based devices (EBD) have been popularized in thyroidectomy worldwide. Microdissection tungsten needle (MDTN) is characterized by the ultra-sharp tip providing safe and meticulous dissection with effective hemostasis. However, little study has applied MDTN in thyroidectomy. METHODS: This retrospective study compared clinical data of the patients who underwent total thyroidectomy (TT) with central neck dissection (CND) using MDTN, harmonic scalpel (HS), and conventional electrocautery (CE). We assessed outcomes related to surgical efficacy and safety. The injury degree of tis…
