Poorly differentiated sinonasal neuroendocrine carcinoma with skull base invasion: A case report.

BACKGROUND: Sinonasal neuroendocrine carcinoma is a rare head and neck tumor that represents only 5% of sinonasal neoplasms. This lesion has a high risk of invasion to adjacent structures such as the orbit, skull base, and soft tissues, with symptoms usually being nonspecific. Most cases are diagnosed in late stages, decreasing overall survival without treatment. To date, there is no consensus on management given its low prevalence; however, it has been shown that multimodal therapy, with the correct surgical approach as the mainstay, offers a better disease-free prognosis. CASE DESCRIPTION:

Pathologic Evaluation and Reporting of Intraductal Papillary Mucinous Neoplasms of the Pancreas and Other Tumoral Intraepithelial Neoplasms of Pancreatobiliary Tract: Recommendations of Verona Consensus Meeting.

BACKGROUND: There are no established guidelines for pathologic diagnosis/reporting of intraductal papillary mucinous neoplasms (IPMNs). DESIGN: An international multidisciplinary group, brought together by the Verona Pancreas Group in Italy-2013, was tasked to devise recommendations. RESULTS: (1) Crucial to rule out invasive carcinoma with extensive (if not complete) sampling. (2) Invasive component is to be documented in a full synoptic report including its size, type, grade, and stage. (3) The term "minimally invasive" should be avoided; instead, invasion size with stage and substaging of

Clinical applications of bladder magnetic resonance imaging: Beyond the VI-RADS® classification.

Bladder cancer is a cause for serious concern due to its high prevalence and the considerable challenges it poses in terms of morbidity, costs, and mortality. In recent years, magnetic resonance imaging (MRI) of the bladder has proved to be a useful tool with a significant impact on both local staging and the assessment of neoadjuvant treatment responses. This article reviews the main anatomical, histological and radiological characteristics of bladder neoplasms, describes bladder MRI, explains the VI-RADS® classification-which should only be used to classify malignant lesions and in studies u

Membranous expression of glucose transporter-1 protein (GLUT-1) in embryonal neoplasms of the central nervous system.

The human erythrocyte GLUT-1 is a transmembrane protein which facilitates transport of glucose in the cell in an energy-independent fashion. Neuroectodermal stem cells show strong membrane immunoreactivitry with this marker at early developmental stages in rodents. Membranous expression by undifferentiated neuroectodermal cells gradually decreases while GLUT-1 becomes confined to the endothelial cells, when these acquire blood-brain barrier function. We thus sought to determine whether GLUT-1 expression was limited to embryonal neoplasms of the central nervous system (CNS) which are presumably

The umbilicus: a rare site for melanoma. Clinical considerations in two cases. Case reports.

The umbilicus is a rare site for malignant melanoma. We report two cases of umbilical melanoma and analyse the mechanisms of diffusion of the neoplasm in relationship to the arterial, venous, and lymphatic anatomy of the region. Because of the peculiarity of these connections, we propose a revision of the concept of melanoma stage for the umbilical region.

[The early detection of stomach and lung cancer during the dispensary care of the population].

The system of early detection of cancer of the stomach and lung was developed on the basis of two-stage automated screening which involved questionnaires, instrumental and laboratory examination, and formation of high-risk and follow-up groups. Gastric and lung cancer were diagnosed in 1.5 and 1.3% of cases of high-risk groups, respectively. Stage I-II tumors made 80.8% of gastric cancer and 20.7% of lung malignancies. In the follow-up groups, all gastric tumors and half of lung neoplasms were diagnosed at early stages. The procedure described assures lower percentage of advanced tumors of the

Incidence trends and relative survival of colorectal neuroendocrine neoplasms: A population-based study using German cancer registry data.

Neuroendocrine neoplasms (NENs) of the colon and rectum are a heterogeneous group of epithelial neoplasms with neuroendocrine differentiation. They include well-differentiated neuroendocrine tumors (NETs), poorly differentiated neuroendocrine carcinomas (NECs) and mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs). Our aim was to calculate incidence, incidence trends and relative survival for colonic and rectal NETs, NECs, and MiNENs. We analyzed data covering the entire German population recorded between 2009 and 2021, calculating age-standardized incidence rates, annual percent chang

Clinicopathological and molecular features of SF3B1-mutated myeloproliferative neoplasms.

The introduction of next-generation sequencing has broadened the genetic landscape of myeloproliferative neoplasms (MPNs) beyond JAK2, MPL, and CALR. However, the biological role and clinical impact of most other mutations are not well defined. We interrogated 101 genes in 143 BCR-ABL1-negative MPNs in chronic phase from 2 large institutions. We detected SF3B1 mutations in 15 cases (10%) and set to investigate the clinical, morphologic, and molecular features of SF3B1 mutated (SF3B1+) MPNs in comparison to SF3B1 wild-type (SF3B1-) cases and to identify distinctive features with myelodysplastic

Apoptosis-related (survivin, Bcl-2), tumor suppressor gene (p53), proliferation (Ki-67), and non-receptor tyrosine kinase (Src) markers expression and correlation with clinicopathologic variables in 60 thymic neoplasms.

BACKGROUND: Our objective was to investigate the expression of survivin, Bcl-2, p53, Ki-67, and Src in thymic neoplasms and analyze their interrelationship with clinicopathologic variables. METHODS: A series of 60 thymic neoplasms was reviewed and classified according to the World Health Organization (WHO) scheme. Key clinical information, including Masaoka stage, recurrence-free survival (RFS), and overall survival (OS) was obtained. The percentage and staining intensity of listed markers were recorded. The correlation of markers and clinicopathologic variables was statistically analyzed usi

Surgical management of lung cancer with solitary cerebral metastasis.

Between 1964 and 1986, 19 patients underwent resection of both a primary lung cancer and the associated brain metastasis. One patient underwent resection of 2 separate primary lung cancers and the associated metastases. The 12 men and 7 women ranged in age from 42 to 67 years (mean, 54.6 years). The cell type was adenocarcinoma in 12 tumors, squamous or adenosquamous cell in 5, large cell undifferentiated or anaplastic in 2, and malignant carcinoid in 1 tumor. The types of resection were as follows: lobectomy for 12 neoplasms, pneumonectomy for 5, bilobectomy for 2, and wedge resection for 1 n

Redistribution, homing and organ-invasion of neoplastic stem cells in myeloid neoplasms.

The development of a myeloid neoplasm is a step-wise process that originates from leukemic stem cells (LSC) and includes pre-leukemic stages, overt leukemia and a drug-resistant terminal phase. Organ-invasion may occur in any stage, but is usually associated with advanced disease and a poor prognosis. Sometimes, extra-medullary organ invasion shows a metastasis-like or even sarcoma-like destructive growth of neoplastic cells in local tissue sites. Examples are myeloid sarcoma, mast cell sarcoma and localized blast phase of chronic myeloid leukemia. So far, little is known about mechanisms unde

Neuroendocrine tumors of the thymus.

Primary neuroendocrine tumors of the thymus (NETTs) are rare and biologically very aggressive neoplasms, usually located in the anterior mediastinal space. They are more frequently observed in males, in their fourth/fifth decades of life. In 50% of cases, NETTs are associated with endocrinopaties [Cushing's syndrome, acromegaly or Multiple Endocrine Neoplasia-1 (MEN-1) syndrome]. NETTs very often present with invasion of the surrounding mediastinal anatomical structures. Surgery, even in advanced stages, is the mainstay of treatment: a compete resection through a median sternotomy or a combine

Computed tomography in the preoperative evaluation of bronchogenic carcinoma.

Preoperative determination of the extent of bronchogenic carcinoma at presentation was assessed by CT using a 4th generation scanner with a 3-second scan time in 98 patients whose disease was later surgically staged. Prospective CT interpretations correctly staged 33 of 35 (94%) resectable lesions, and disease in 41 of 45 (91%) patients who had lesions that were not resectable for cure. No definite opinion was rendered on the scans of the remaining 18 patients. Retrospectively analysis using slightly modified criteria resulted in a correct prediction of resectability in 28 of 30 (93%) patients

Thoracic Manifestations of Genitourinary Neoplasms and Treatment-related Complications.

Genitourinary (GU) malignancies are a diverse group of common and uncommon neoplasms that may be associated with significant mortality. Metastases from GU neoplasms are frequently encountered in the chest, and virtually all thoracic structures can be involved. Although the most common imaging manifestations include hematogenous dissemination manifesting with peripheral predominant bilateral pulmonary nodules and lymphatic metastases manifesting with mediastinal and hilar lymphadenopathy, some GU malignancies exhibit unique features. We review the general patterns, pathways, and thoracic imagin

Laryngeal malignant neoplasms in children and adolescents.

An exhaustive literature review of the last two decades discloses 47 laryngeal malignant neoplasms in children and adolescents. The most frequent malignant neoplasm is the embryonal variant of rhabdomyosarcoma. The timely diagnosis of a laryngeal neoplasm depends on maintaining a high index of suspicion in a patient with progressive airway obstruction, dysphagia or dysphonia, and conducting an efficient work-up-including magnetic resonance imaging and direct laryngoscopy under general anesthesia in association with bronchoscopy-in order to define the extent of the lesion, rule out multiple les

[Value of colonoscopy in tumor after-care after colorectal carcinoma].

The significance of colonoscopic follow up is discussed controversially. Colonoscopy after resection of colorectal cancer offers the possibility of direct inspection and to take biopsies for the early detection of local recurrence or additional neoplasms. In a retrospective study we examined the benefit of regular colonoscopies. Between 1/1995 and 4/1996 237 colonoscopies were performed on 164 patients after resection of colorectal cancer. The evaluation was done due to the stage of the primary tumor, the age of the patients, the time after operation and the number of previous colonoscopies. 5

Unexpected Neoplasms in Lungs Explanted From Lung Transplant Recipients: A Single-Center Experience and Review of Literature.

INTRODUCTION: Lung transplantation is a common treatment for various indications, but undiagnosed neoplasms are found in 0.5% to 2.4% of explanted lungs. We report the largest single-institution series of patients with unexpected neoplasms in explanted lungs and compare rates of undiagnosed malignancies before and after the 2005 Lung Allocation Score (LAS) update. METHODS: We reviewed the medical records of patients who underwent lung transplantation at the Cleveland Clinic from 1990 to 2014. In cases of neoplasm discovered on explant, tumor type, pathological stage, recurrence, and date of d

Emergence of CA125 immunoreactivity in recurrent or metastatic primary ovarian mucinous neoplasms of the intestinal type.

Primary ovarian mucinous carcinomas are uncommon and usually present as unilateral stage 1 neoplasms. The vast majority are of the so-called intestinal or enteric type and arise from a preexisting intestinal-type mucinous borderline neoplasm. The overall prognosis is good. However, a minor proportion recurs or metastasizes, and this is associated with a poor prognosis. The vast majority of primary ovarian intestinal-type mucinous carcinomas and borderline tumors exhibit a variable degree of positivity with enteric markers and are CA125 negative. The primary purpose of this study was to describ

Fournier's gangrene as initial presentation of rectal cancer.

Fournier's gangrene is a necrotizing fasciitis of the perineum that can occur due to a loss of integrity of the gastrointestinal or urethral mucosa. Its appearance as the debut of a perforated rectal neoplasm is unusual, more uncommon in young patients, leading to an advanced stage of the disease. Therefore, we present the case of a 47-year-old male with Fournier's gangrene as the debut of a rectal cancer that, despite severe presentation, the pathological stage was early (pT2N0), highlighting the importance of considering neoplastic etiology in perineal gangrene and the need for an early mult

Thoracic Imaging Manifestations of Treated Lymphomas: Response Evaluation, Posttherapeutic Sequelae, and Complications.

Lymphoma is the most common hematologic malignancy comprising a diverse group of neoplasms arising from multiple blood cell lineages. Any structure of the thorax may be involved at any stage of disease. Imaging has a central role in the initial staging, response assessment, and surveillance of lymphoma, and updated standardized assessment criteria are available to assist with imaging interpretation and reporting. Radiologists should be aware of the modern approaches to lymphoma treatment, the role of imaging in posttherapeutic surveillance, and manifestations of therapy-related complications.
