Efficacy of IM dexmedetomidine in alleviating catheter related bladder discomfort and postoperative inflammatory response following percutaneous nephrolithotomy. A prospective randomized controlled study.

BACKGROUND AND AIMS: Catheter-related bladder discomfort (CRBD) causes significant distress to the patient and requires great attention to analgesia and increases morbidity in the postoperative period. This study evaluated the efficacy of intramuscular dexmedetomidine in alleviating CRBD following percutaneous nephrolithotomy (PCNL) and postoperative inflammatory response. MATERIAL AND METHODS: A prospective randomized double-blind study was conducted in a tertiary care hospital from December 2019- March 2020. Sixty seven of ASA I and II patients scheduled for elective PCNL were randomized an

Miniaturized Versus Standard-Tract Percutaneous Nephrolithotomy for Renal Calculi Larger Than 2 cm: Tract Caliber Determines Hemorrhagic Morbidity Without Compromising Stone Clearance.

Background Percutaneous nephrolithotomy (PCNL) is the treatment of choice for renal calculi larger than 2 cm, but much of its morbidity arises from the access tract rather than from stone removal itself. Whether tract miniaturization preserves clearance at this stone burden, where a larger working channel might be presumed necessary, remains incompletely defined outside randomized settings. We compared perioperative outcomes of mini-PCNL and standard PCNL in a high-volume tertiary cohort. Materials and methods We retrospectively analyzed 215 consecutive adults undergoing PCNL for renal calculi

Quantitative analysis of pleural effusion after prone percutaneous nephrolithotomy based on routine postoperative abdominal computed tomography: prevalence, risk factors, and clinical implications.

BACKGROUND: Detailed analysis of post-percutaneous nephrolithotomy (PCNL) pleural effusion (PE) and its relationship with respiratory symptoms is lacking. This study quantitatively investigates the prevalence, volume, and clinical significance of post-PCNL PEs detected on abdominal computed tomography (CT). METHODS: We conducted a retrospective analysis of 252 consecutive patients who underwent prone PCNL between January 2018 and April 2020. PEs and stone volume were quantified using three-dimensional (3D) segmentation of post- and pre-operative abdominal CT images, respectively. Respiratory

Robot-assisted laparoscopic pyelolithotomy versus multiple-tract mini-percutaneous nephrolithotomy for moderate-to-complex kidney stones: a propensity score-matched retrospective study.

Large and complex kidney stones remain difficult to treat, particularly when stones extend into several calyces. RLPL provides access to the collecting system without renal parenchymal puncture, whereas multiple-tract Mini-PCNL relies on several percutaneous tracts to reach different stone-bearing areas. We retrospectively analyzed patients with moderate-to-complex kidney stones treated with either procedure between January 2017 and December 2025. Propensity score matching was used to reduce baseline differences between groups. The main endpoint was stone-free status at 3 months; perioperative

Perioperative pain relief in percutaneous nephrolithotomy: A randomised trial of ultrasound-guided erector spinae plane block compared to peri-tubal anaesthetic infiltration.

OBJECTIVE: The aim of this study is to compare peri-tubal local anaesthetic (LA) infiltration with ultrasound-guided erector spinae plane block (ESPB) as an analgesic technique for percutaneous nephrolithotomy (PCNL) surgeries. The study hypothesised that patients receiving ESPB would require postoperative rescue analgesics later as compared to those receiving LA infiltration. METHODS: The study is a prospective, single-blinded, randomised clinical trial. Fifty-two adult patients undergoing unilateral PCNL were randomly allocated into the ESPB group (n = 26) or the LA infiltration group (n =

Enhanced Effectiveness of Extracorporeal Shock Wave Lithotripsy for Residual Stones Post-percutaneous Nephrolithotomy: A Comparative Study With Primary Stones of Similar Size.

INTRODUCTION: Renal stones are a common health problem worldwide, with rising prevalence and high recurrence rates. Extracorporeal shock wave lithotripsy (ESWL) and percutaneous nephrolithotomy (PCNL) are established treatments. While ESWL is well studied for primary renal stones, its effectiveness for post-PCNL residual stones is less well defined. OBJECTIVE: This study aims to compare the stone-free rate of ESWL in patients with post-PCNL residual stones versus those with primary renal stones of the same size (5-15 mm). METHODS: This prospective cohort study was conducted at the Sindh Inst

Analysis of stone-free rates and residual fragment sizes following standard percutaneous nephrolithotomy: mistakes in the treatment of non-opaque kidney stones.

Our presented study aimed to evaluate the possible effects of stone opacity, on both the success of percutaneous nephrolithotomy and the sizes of residual fragments following the procedure. Medical records of patients undergoing PCNL treatment for kidney stones at our clinic between July 2014 and May 2022 were evaluated in a retrospective manner. A total sample size of 304 patients with the required criteria was included. Patients were divided into two groups based on the radiopacity status of the stones assessed in the kidney-ureter-bladder graphy (KUB) [Group O (n = 211): opaque, Group N (n

Outcomes of Ureteroscopy vs Mini-Percutaneous Nephrolithotomy for Pediatric Upper Urinary Tract Calculi: Comparative Nonrandomized Outcomes from Two Tertiary Endourology Referral Centers.

Introduction: Pediatric upper urinary tract calculi can be treated by ureteroscopy (URS) or mini-percutaneous nephrolithotomy (mPCNL). We wanted to compare outcomes of URS and mPCNL from two tertiary referral centers that specialized in one of these treatments for pediatric stone disease. Materials and Methods: Data were collected from two tertiary centers for a 10-year period (2010-2019); one center specializing in URS and the other in mPCNL for consecutive patients ≤16 years undergoing either of these minimally invasive interventions. Upper urinary tract stones included stones in the kidney,

Laparoscopic ureterolithotomy, flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy for treatment of upper urinary calculi in patients with autosomal dominant polycystic kidney disease.

OBJECTIVES: Patients with autosomal dominant polycystic kidney disease (ADPKD) showed relatively high incidence of urinary stones. Enlarged kidneys occupied by cysts could make the stone-removal surgery relatively difficult. This study aimed to compare the efficacy and safety of retroperitoneal laparoscopic ureterolithotomy (RPLU), flexible ureteroscopic lithotripsy (FURL) and percutaneous nephrolithotomy (PCNL) in the ADPKD patients with upper urinary stones. METHODS: In this study, 45 patients with ADPKD who underwent RPLU, FURL and PCNL procedures were evaluated. Demographic and serum para

Perioperative application of dexmedetomidine for postoperative systemic inflammatory response syndrome in patients undergoing percutaneous nephrolithotomy lithotripsy: results of a randomised controlled trial.

OBJECTIVE: Our previous retrospective study demonstrated that perioperative dexmedetomidine (Dex) administration was associated with low systemic inflammatory response syndrome (SIRS) incidence. The present study was designed to investigate whether perioperative administration of Dex decreases the incidence of postpercutaneous nephrolithotomy lithotripsy (PCNL) SIRS in patients who undergo PCNL. DESIGN: A randomised controlled trial was designed. PARTICIPANTS: A total of 190 patients were randomly assigned to receive Dex (DEX group, n=95) or saline control (CON group, n=95) and completed the

A Randomized Controlled Trial of Preoperative Prophylactic Antibiotics Prior to Percutaneous Nephrolithotomy in a Low Infectious Risk Population: A Report from the EDGE Consortium.

PURPOSE: Single institution studies suggest a benefit of a week of preoperative antibiotics prior to percutaneous nephrolithotomy. These studies are limited by lower quality methodology, such as the inclusion of heterogeneous populations or nonstandard definitions of sepsis. The AUA (American Urological Association) Best Practice Statement recommends less than 24 hours of intravenous antibiotics but to our knowledge no other data exist on the duration or benefit of preoperative antibiotics. Using CONSORT (Consolidated Reporting of Trials) guidelines we sought to perform a rigorous multi-instit

[Acquired experience in anesthesia and perioperative intensive care in percutaneous nephrolithotomy. Current approach in the endoscopic treatment of lithiasis and pyelo-ureteral junction anomalies].

Percutaneous endoscopic treatment of the kidney retains a place in the treatment of renal stones (percutaneous nephrolithotomy--PCNL) and ureteropelvic junction abnormalities (endopyeloplasty). It requires anaesthesia ensuring surgical comfort and safety for the patients despite changes in position and the prolonged ventral supine position. The operation carries certain iatrogenic risks related to caliceal irrigation in patients with more or less documented episodes of infection and carries risks of haemorrhage and effraction of adjacent organs. 282 patients treated by PCNL between 1984 and 19

Renal vascular injury during percutaneous nephrolithotomy and migration of a double-J catheter fragment into pulmonary artery in the following nephrectomy--a case report.

A 60-year-old male underwent percutaneous nephrolithotomy (PCNL) for left renal stone at a community hospital. The surgery was, in general, unremarkable and a double-J ureteral catheter was placed before completion of surgery. Dyspnea, irritability, hypotension and flank pain developed in the recovery room. In addition, pleural effusion and elevation of the left hemidiaphragm were revealed on chest roentgenogram, and mild hypoxemia and respiratory acidosis were also detected by gas analysis. He was transferred to our hospital for further management. After arrival at our hospital, we decided to

Retrograde intrarenal surgery versus miniaturized percutaneous nephrolithotomy for renal calculi: a grade-assessed meta-analysis of randomized controlled trials with trial sequential analysis.

BACKGROUND: The optimal management of medium-sized (1-3 cm) renal stones remains a clinical dilemma, with retrograde intrarenal surgery (RIRS) and miniaturized percutaneous nephrolithotomy (mPCNL) serving as competing minimally invasive options. To overcome the selection bias inherent in previous meta-analyses that included observational data, we aimed to compare the efficacy and safety of both techniques by exclusively analyzing randomized controlled trials (RCTs). METHODS: A systematic literature search of five databases was conducted up to February 2026. Only RCTs comparing RIRS and mPCNL

Comparison of Stone-Free Rates and Systemic Inflammatory Response Between Intelligent Pressure-Controlled and Conventional Mini-Percutaneous Nephrolithotomy in Patients With Kidney Stones.

BACKGROUND: Elevated intrarenal pressure during mini-percutaneous nephrolithotomy (mini-PCNL) may worsen systemic inflammation and affect stone clearance. This study compared stone-free rates and incidence of systemic inflammatory response syndrome (SIRS) between intelligent pressure-controlled and conventional mini-PCNL after propensity score matching (PSM). METHODS: This retrospective cohort study included patients undergoing primary mini-PCNL for kidney stones (April 2022-April 2025). After PSM, patients were divided into a conventional mini-PCNL group (n = 136) and an intelligent pressure
