A case-control study of persons with elevated blood levels of dieldrin.

Twenty-seven pesticide workers with elevated blood levels of dieldrin (greater than or equal to 15 ppb) were involved in a case-control study which included history and physical examination, comprehensive neurological evaluation, laboratory tests, and psychological and psychomotor testing. No clinically important differences were found on history, physical, specialized neurological tests, or laboratory examination. The exposed group showed a statistically significant difference in five out of 58 psychological (P) and psychomotor (PM) tests--at least three would be expected by chance (p less th

Munchausen's disorder: a case of a factitious neurological emergency.

Few signs are more ominous than the dilated pupil which is unreactive to light. The deliberate use of this fact and the simulation of other neurologic signs to enhance the aura of a serious illness will often expose a patient to hazardous procedures. A patient with Munchausen's Disorder who made unsuccessful attempts to simulate a neurological emergency is presented. A thorough knowledge of the pathophysiology of neurologic disease will enable the physician to detect the possibility of malingering when confronted with this problem. A strong cholinergic solution will produce pupillary constrict

Gait Analysis in Neurologic Disorders: Methodology, Applications, and Clinical Considerations.

Gait and balance disorders are a leading cause of morbidity, mortality, and disability in central and peripheral neurologic disorders. Neurologic gait disorders are classically evaluated with a clinical examination and visual pattern recognition. Gait patterns such as a parkinsonian or ataxic gait may have distinct etiologies and are assessed using the neurologic physical examination and validated clinical scales. Technological advances have made gait analysis more accessible, allowing precise objective measurement of gait and balance deficits. Gait analysis may be more sensitive at detecting

Improving the detection and documentation of suspected cauda equina syndrome: a quality improvement project.

BACKGROUND: Cauda equina syndrome (CES) is a rare but critical neurological emergency. Prompt diagnosis and accurate documentation are essential to avoid permanent neurological impairment. This quality improvement project aimed to improve the accuracy and completeness of neurological examination documentation for patients presenting with suspected CES. METHODS: Using three Plan-Do-Study-Act (PDSA) cycles, a digitally prompted documentation tool '#CES' was developed and implemented in the electronic patient record system. The tool was designed in alignment with the 2019 GIRFT Spine Report and

Concomitant Occurrence of Acute Motor Axonal Neuropathy in Systemic Lupus Erythematosus.

Few case reports exist on the association of acute motor axonal neuropathy, a subtype of the Guillain-Barré syndrome (GBS), with systemic lupus erythematosus. Standard therapeutic guidelines for concomitant acute motor axonal neuropathy and systemic lupus erythematosus in the acute phase are not established, and no studies have reported physical medicine and rehabilitation perspective management in the plateau and recovery phases. A 50-yr-old woman with systemic lupus erythematosus presented with upper and lower limb weakness that progressed to an inability to walk. Neurological examination, r

Prospective association of general anesthesia with risk of cognitive decline in a Chinese elderly community population.

As life expectancy increases and the population grows, the number of surgeries performed each year is likely to continue to increase. We evaluated whether surgery with general anesthesia increases risk for cognitive impairment in a Chinese elderly community population. The current data was obtained from the China Longitudinal Aging Study (cohort 1) and Shanghai Brain Aging study (cohort 2). Cohort 1 included 1545 elderly people with normal cognitive function, who underwent a screening process that included physical examination, medical history, baseline and 1-year follow-up assessments of cogn

[A man with hanging eyelids].

A 73-year-old man came to the outpatient clinic because of diplopia and hanging eyelids. Neurological examination revealed fatigueable bilateral ptosis. The patient had a strong positive ice pack-test. Myasthenia gravis preached on top of the differential diagnosis. Subsequent blood tests showed antibodies against ACh-receptors and confirmed the diagnosis.

MDS-UDPRS-III in the diagnosis of idiopathic Normal Pressure Hydrocephalus and identification of candidates for Ventriculo-Peritoneal Shunting surgery. Results from a retrospective large cohort of patients.

OBJECTIVES: To evaluate the entity of extrapyramidal signs, characterize them and evaluate the dynamics of change by the mean of MDS-UPDRS-III in iNPH patients after the TT to determine if this tool may help the diagnosis of iNPH and the identification of candidates for Ventriculo-Peritoneal Shunting. MATERIALS AND METHODS: We retrospectively collected data from 120 patients with the initial diagnosis of possible iNPH; they underwent neurological examination by the means of MDS-UPDRS-III and other scales before and after Tap Test (TT). They were then classified as defined iNPH (57), probable

Relationship between clinical outcomes and nerve conduction studies before and after surgery in patients with carpal tunnel syndrome.

BACKGROUND: Nerve conduction study (NCS) is the only useful test for objective assessment of carpal tunnel syndrome (CTS). However, the relationship between pre- and postoperative NCS and clinical outcomes was unclear. This study aimed to determine whether pre- and postoperative (6 months) NCS could predict patient-oriented and motor outcomes (6 and 12 months postoperatively) in patients with CTS. METHOD: Of the 85 patients with CTS, 107 hands were analyzed from March 2011 to March 2020. All patients underwent open carpal tunnel release and were examined using the disabilities of the arm, sho

Comparison of Borderline Ultrasound and Nerve Conduction Studies for Carpal Tunnel Syndrome.

BACKGROUND: Choosing cutoff values for nerve conduction studies (NCS) and ultrasound cross-sectional area (CSA) in the diagnosis of carpal tunnel syndrome (CTS) is critical in determining the diagnostic accuracy of the tests. The goals of this study were to: (1) determine the sensitivity and specificity of various electrodiagnostic and ultrasound threshold values for diagnosis of CTS; and (2) determine the number of hands that underwent NCS and ultrasound that were within 10% of threshold values. METHODS: A total of 309 hands of 235 patients were included in this study. Diagnosis of median ne

Influence of infections on the quality of general movements in premature infants.

INTRODUCTION: The task of contemporary neonatology is not only to save the lives of children born prematurely, but also to provide them with the highest possible quality of life. The Prechtl method, one of several methods of assessing the quality of general movement patterns, enables early identification of immaturity or damage to the central nervous system. AIM: Dynamic assessment of the type and quality of general movement (GMs) in preterm infants to determine indications for early neurodevelopmental support and to identify the relationship between the occurrence of maternal and neonatal pe

An arrow that missed the mark: a pediatric case report of remarkable neurologic improvement following penetrating spinal cord injury.

Penetrating spinal cord injuries are rare in children but result in devastating impacts on long-term morbidity and mortality-with little known about the recovery capacity in this age group. We present the case of an eight-year-old child who sustained a penetrating injury through the right anterior thorax. Thoracic CT showed the arrow tip extending through the spinal canal at T6. Neurologic examination revealed no motor or sensory function below T6. The arrow was surgically removed without complications through an anterior-only approach. MRI on post-operative day (POD) 4 showed focal T2 hyperin…

Comparison of tactile sensitivity measured with a new digital esthesiometer (Beam Test) relative to Semmes-Weinstein monofilament analog esthesiometer.

The assessment of tactile sensitivity uses many tests, of which monofilaments are considered the best. The aim of this study was to develop a tactile sensitivity test, similar to the monofilament test, which eliminates the error risk related to manipulations by the observer, and to establish a correspondence scale. We studied 29 healthy subjects (18 women and 11 men) with a mean age of 27.8 years. The Semmes-Weinstein monofilament (SWM) analog esthesiometer and a digital beam esthesiometer (DBE) were used. We evaluated the tactile sensitivity threshold on the fingertip of each subject's domina
